Clinical randomized trial to compare the efficacy of Botulinum toxin injection versus occlusal splint for management of anterior disc displacement 比较肉毒毒素注射与咬合夹板治疗前椎间盘移位疗效的临床随机试验

Introduction: Anterior disc displacement is one of the major findings in Tempromandibular joint derangments TMDs, as well as the most common cause of Tempromandibular joint TMJ sounds uncoordinated function between the muscle bundles of the upper head and the superior part of the lower head of the lateral pterygoid muscle could lead to abnormal movements of the disc, therefore, causing a click . Aim: The aim of the present study was to investigate the effect of botulinum toxin type A injection in the lateral pterygoid muscle on temporomandibular joint clicking versus occlusal splint therapy. Patients & methods: The study enrolled forty patients divided into two equal groups. All patients were selected from the outpatient clinic, complaining of a painful TMJ click with muscle tenderness, associated with ADDR confirmed by magnetic resonance imaging. A BTX-A vial was diluted with normal saline to a concentration of 10 U per 0.1 mL for injection in a 1-ml insulin syringe. This was injected into the ipsilateral LP muscle with the guidance of an electromyogram device. Hard occlusal splint fabricated from transparent acrylic resin. Results : There was no statistically significant difference between pain, clicking and MMO in the two groups preoperatively. Group I showed the statistically significantly lowest mean pain scores. The mean MMO after 12 months showed statistically significantly higher mean value compared to pre-operative measurement in the two groups. Group I showed no clicking while Group II showed the highest prevalence of clicking. Conclusion: There was a significant increase in mouth opening from the start of treatment and till the end of the follow up period, concerning pain there is decrease in VAS values, and clicking improved postoperatively after 1 week in the first group while improved after first month in the second group, radiographic evaluation shows no change in MRI. to clinical criteria classifying TMJ disorders but is generally used to denote a mechanical fault in the joint interfering with its smooth movements, such as an abnormal positional relationship of the articular disc to the mandibular condyle and the articular eminence. Thus, the term has been used synonymously with disc displacement (2) . The disorders have been associated with characteristic clinical findings, including pain, joint sounds, headache, and irregular or deviated jaw functions, several attempts to identify the cause of disc displacement were reported (3) .

role of allograft bone gel in osseointegration of immediate loading implant in esthetic zone 同种异体骨凝胶在美学区即刻加载种植体骨整合中的作用

Introduction: Immediate implants loading permit the placement of a temporary restoration immediately after the insertion of the fixture that is certainly an aesthetic and comfort for the patient. Immediate nonfunctional loading of single-tooth implants in the upper anterior after augmentation with allograft bone gel can result in successful implant integration and stable peri-implant conditions. Aim: The aim of the study was to assess clinically and radiographically osseointegration of immediate loading implant with allograft bone gel. Patient and methods: involved fourteen immediate loading dental implants in 10 male patients. The age of the patients range from 22- 48 years old with average 35 years old. Fourteen Immediate implants were divided into two main groups: Group 1 (control group), A traumatic extraction of single rooted tooth or remaining root and immediate placement of dental implant into fresh extracted socket. Group 2 (test group), A traumatic extraction of single rooted tooth or remaining root and immediate insertion of the implant in extracted socket and sealed with allograft bone gel (c-blast putty). Results: test group showed significant higher bone density than control group (Independent samples t-test, P<.001) after two weeks ,three months and nine months. Conclusion: Allograft bone gel(C blast putty gel) shows effective role in implant osseointegration. Immediate non-functional loading implant design does not disturb or interfere with bone formation around implant in control or test group.

Effectiveness of Organic Versus Inorganic Analogues for Treating White Spot Lesions: An in Vitro Study 有机与无机类似物治疗白斑病变的有效性:体外研究

Introduction: Demineralization denotes the dissolution of the calcium and phosphate ions from the hydroxyl-apatite/tooth structure into the plaque and saliva caused by the acidic attack. White spot lesions start at the outermost layer of the teeth, damaging the dental hard tissues through the loss of mineral ions from the hydroxyapatite lattice at the surface. An ideal remineralizing agent should mimic the organization and micro-architecture of natural tooth structure mineral crystals to the greatest extent possible. Aim: S tudy aimed to evaluate and compare the effect of two remineralizing agents on remineralization of initial enamel carious lesions. Furthermore, the impact of both remineralizing agents and one infiltrating resin material on the color of demineralized enamel and their ability to mask white spot lesions was investigated. Subjects and Methods: 50 intact, freshly extracted permanent human anterior central incisors (extracted from diabetic patients) were selected and stored in distilled water at room temperature (22 0 C). Selected teeth were classified into two main groups (Control and Demineralized) according to the type of treatment. Results: Significant differences resulted between treatments for ΔE at p<0.001. Demineralized enamel showed the highest significant color change values (6.03±0.4). All treatments of demineralized enamel significantly restored the tooth color. Conclusion: Curodont Repair and MI Paste Plus can fill up defects and micropores on demineralized tooth structure, significantly restoring the tooth color.

Introduction : Glass-ionomer cements (GICs) may be the restoration of choice in patients with a high caries risk, owing to their anticariogenic and remineralizing capabilities. Regrettably, as compared to other restorative materials, it has lower flexural and tensile strength, fracture resistance, and a higher rate of wear, all of which have an implication on its survival rates when used in load bearing areas. Materials and methods: Sixty patients with high caries risk were selected. Class II occlusal slot cavities were prepared in the first permanent molar and restored randomly by two restorations, either; EQUIA ® Forte Fil or Zirconomer ® Improved. Restorations were evaluated according to FDI criteria in terms of functional properties at baseline, after six months and one year. Results : Chi-square and Mann-Whitney tests revealed that there was no statistically significant difference between both materials for the tested properties at base line. At six months follow up time, 92% of the EQUIA ® Forte Fil group and 68% of the Zirconomer ® Improved group were clinically successful. Meanwhile, at 12 months follow up time, 88% of the EQUIA ® Forte Fil group and 40% of the Zirconomer ® Improved group were clinically successful with significant difference between them at both six and 12 months. Conclusions : Glass ionomer with glass hybrid technology exhibited better clinical performance than zirconia-reinforced glass ionomer in class II slot cavities after six months and one year with high success rate in the purpose of restoring posterior permanent teeth.

Introduction : Dental fluorosis is a chronic condition of enamel hypomineralization resulted from excessive ingestion of fluoride during tooth development. Different treatment protocols were used to improve esthetic appearance of fluorosed teeth. Aim : To evaluate patient satisfaction on esthetic improvement of mild to moderate fluorosed teeth after treating with different minimal-invasive treatment modalities. Patients and Methods : Sixteen participants were randomly assigned in 8 treatment protocols with 20 teeth at each protocol ( n=20 ). ( P 1 ) Opalescence boost PF 40%. ( P 2 ) Opalustre. ( P 3 ) MI-Paste Plus. In ( P 4 ) teeth were treated with Opalustre + Opalescence boost PF 40%. In ( P 5 ) Opalescence boost PF 40% + MI-Paste Plus, while in ( P 6 ) Opalustre +MI-Paste Plus. Evaluation of Color Matching Ability of a Single Shaded Resin Composite Versus a Single Translucency Resin Composite With Different Teeth Shades 单一阴影树脂复合材料与不同齿色的单一半透明树脂复合材料的配色能力评价

Aim: This in-vitro study digitally evaluates the color matching ability of a single-shaded resin composite and a single-translucency resin composite system placed in teeth with three different teeth shades at two different cavity depths, and to compare both of them to a multi-shaded resin composite system. Methods: 90 extracted sound teeth were selected and their shade was recorded. Teeth were divided according to their shade into three groups (S 1 ,S 2 and S 3 ). In each group, Class V cavities were prepared at 0.5 mm depth (Subgroup D 1 ) for half of the teeth, while cavities in the other half were adjusted at 1.5 mm depth (Subgroup D 2 ). Each subgroup was further divided into three classes according to the restorative material placed in the prepared cavities (M 1 , M 2 and M 3 ). Shade was recorded after placement and ΔE was calculated. Results: Single-shaded resin composite (M 1 ) recorded the significant lowest ΔE values wih S 1 followed by S 2 and S 3 respectively (P ≤ 0.05). Multi-shaded resin composite (M 3 ), generally showed the best results with the three teeth shades compared to the other two materials. No significant differences were found at both cavity depths for all groups (P ≤ 0.05). Conclusions: The single-shaded resin composite showed more acceptable color matching at lighter teeth shades than at darker teeth shades. Multi-shaded resin composite showed superior color matching ability at the three different teeth shades. The selected cavity depths had no siginificant effect on the color matching ability of the different resin composite materials used. strength and longevity, and better esthetic properties (2) . “ Evaluation of the Effect of Two Different Concentration of Arginine on Fluoride Uptake by Demineralized Enamel Surfaces” in vitro study 两种不同浓度精氨酸对脱矿牙釉质表面氟吸收影响的体外研究

Introduction: Arginine is a natural component of human saliva, which has recently been incorporated in dentifrices treating teeth sensitivity and acts as a protective factor against caries. Aim of the study: This study evaluated the effect of two different concentrations of arginine; 2.5% and 8% on fluoride uptake by demineralized enamel surfaces. Methods: 80 specimens obtained from 40 human premolars were divided randomly into 4 equal groups (n=20 per group): (GI=negative control), (GII=positive control) treated by sodium fluoride (NaF500 ppmf), group III treated by combination of sodium fluoride solution (NaF 500 ppmf)+arginine solution (2.5%) and group IV treated by combination of sodium fluoride solution (NaF 500 ppmf)+arginine solution (8%). Microhardness was tested using Vickers microhardness test, in addition to imaging using environmental scanning electron microscope (ESEM) and minerals content was assessed using energy dispersive X-ray spectroscopy (EDX). Statistical analysis was performed utilizing Kolmogorov-Smirnov and Shapiro-Wilk tests for normality of data, Levene’s tests for homogeneity and one-way ANOVA with Bonferroni post hoc tests for the evaluation of statistical significance among the groups. Results: Highest mean microhardness was in group III (370.58±12.14) and group IV (370.22±8.24). An increase in mineral density was found in group IV followed by group III, then group II and group I as revealed by ESEM. Data analysis showed increase in fluoride concentration in group III followed by group IV, group II and group I with a mean value (17.59±1.33), (14.62±1.91), (13.57±2.29) and (14.32±2.13) respectively. Conclusion: Fluoride uptake of demineralized enamel is increased when using both concentrations of arginine (2.5% and 8%). Arginine modulates areas of mineralized deposits and overall refinement of enamel ultrastructure.

Introduction: periodontitis is an inflammatory disease of the supporting tissues of the teeth caused by specific micro-organisms or a group of specific micro-organisms, resulting in progressive destruction of the periodontal ligament and alveolar bone with pocket formation, recession, or both. Aim of the study: The present study aimed to assess the effect of hydroxyapatite and hydroxyapatite combined with platelet-rich plasma graft in treatment of intrabony periodontal defects. Subjects and Methods: 24 periodontitis patients with intra-bony periodontal defects participated in this clinical trial and treated with full thickness mucoperiosteal flap surgical technique and Hydroxyapatite bone graft (goup I) or full thickness mucoperiosteal flap surgical technique and Hydroxyapatite bone graft combined with platelet-rich plasma graft (group II ). Clinical outcomes included plaque index (PI), gingival index (GI), probing depth (PD) and clinical attachment level (CAL). Bone defect area was measured by digital periapical radiographs and bone density was calculated by Digora program. Measurements at baseline, 3-months and 6-months postoperatively were subjected to statistical analysis. Results : Both of hydroxyapatite bone graft (HA group) and hydroxyapatite bone graft with platelet-rich plasma (HA+ PRP group) resulted in statistically significant improvement in all clinical and radiographic parameters from baseline to 3-months and 6-months. But no significant difference was observed between both groups. After 3-months, HA group showed 5.05±.37 PD reduction and 6.65±1.11 CAL gain, while HA+PRP group showed 4.95±.96 PD reduction and 6.30±0.86 CAL gain. After 6-months HA group showed 3.50±.62 PD reduction and 4.60±1.17 CAL gain, while HA+PRP group showed 3.35±.67 PD reduction and 4.70±0.67 CAL gain. Conclusion : Both hydroxyapatite bone graft group and hydroxyapatite bone graft combined with platelet-rich plasma group showed significant reduction in clinical and radiographic outcomes after 6 months with no statistically significant difference between them . Both treatment modalities are successful procedures for treating intrabony defects

After 6-months HA group showed 3.50±.62 PD reduction and 4.60±1.17 CAL gain, while HA+PRP group showed 3.35±.67 PD reduction and 4.70±0.67 CAL gain. Conclusion : Both hydroxyapatite bone graft group and hydroxyapatite bone graft combined with platelet-rich plasma group showed significant reduction in clinical and radiographic outcomes after 6 months with no statistically significant difference between them . Comparative Clinical and Microbiological Study of Probiotic Strain Lactobacillus on the Count of Streptococcus Mutans in Saliva. 益生菌乳酸菌对唾液中变形链球菌计数的临床及微生物学比较研究。

Introduction: The concept of microbial ecological changes and probiotic ap-proaches is a promising concept for preventing dental disease and decreasing of oral cavity pathogens . Aim : to evaluate the effect of probiotic strain Lactobacillus on the count of Streptococcus mutans and Lactobacillus in saliva and the plaque formation rate. Materials and methods: This randomized clinical trial included 30 normal apparently healthy and cooperative children aged 6 to 11 years and selected from the Outpatient Dental Clinic of Pediatric dentistry Department, Faculty of Dentistry, Suez Canal University. Children were randomly divided into 2 groups as follows: Group I : included (15) children who brushed their teeth two times a day with fluoro toothpastes and Group II : included (15) children who instructed to take Periobalance G.U.M lozenges once daily at the evening immediately following brushing and floss-ing for 60 days. Clinical evaluation was performed by measuring plaque indices scores at all evaluation periods. Microbiological evaluation was performed by direct examination of bacterial cultures. Results: Probiotics showed a significant reduction in the mean Streptococcus mutans counts and increase the count levels of Lactobacilli but did not show a significant difference increase. There was decrease in the plaque index, with the lowest value after 60 days in both groups. Conclusion: Probiotic is a promising concept of preventing dental diseases. Until the mo-ment in clinical practice, dental caries is treated symptomatically although intensive focus on preventive strategies took place in many studies. These preventive measures either change or modify the factors associated with caries which include dietary, host, salivary and bacterial factors
