Objective: We present a case report of an 89 years old male with a diagnosis of incarcerated left inguinal hernia, with surgical findings of a complicated sigmoid diverticulum and a tubular structure of 0.5 mm attached to the sigmoid colon, fixed to the inguinal ligament with a true lumen that could correspond to a duplicated appendix B4 type. Introduction: Colon diverticular disease is more frequent in western countries, present in 30% of 60 years old patients and in more than 50% in 80 years old patients, with 95% of the times located at sigmoid colon. The most frequent complication is the acute diverticulitis (25%) and in 30% of these patients, surgery is required. Discussion: Acute, complicated, diverticular disease is an infrequent pathology, even more, a colonic diverticulum included within the hernia sac. Of the appendix congenital malformations, a duplicated appendix is the rarest; usually found in asymptomatic adults as an incidental find during abdominal surgery or during contrasted abdominal scans. Conclusion: Awareness must be raised within first-contact health care professionals about diagnosing surgical entities with high mortality rates, in asymptomatic patients with unclear clinical findings, especially with the occurring of a demographic transition in most countries. In this specific patient, Hartman surgical procedure and a second time inguinal plasty were the best surgical options for this patient; even considering the mortality rates (25%) of a perforated diverticulum, considering added surgical site infection.

Gallbladder volvulus is a rare cause of acute cholecystitis and is due to the torsion of the gallbladder along its pedicle. The diagnosis of gallbladder volvulus is often made intra-operatively and therefore failure to recognize the condition pre-operatively may lead to delay in definitive management. This case report presents a 95-year-old female patient admitted to a regional hospital with acute right upper quadrant pain. The patient was taken to theatre and on laparoscopy was found to have a gangrenous gallbladder twisted along its pedicle. A laparoscopic cholecystectomy was performed and the patient was successfully treated. Awareness of this rare entity and prompt surgical intervention is required to limit the potential morbidity and mortality.

The condylomata acuminatum or genital warts are produced by the human papillomavirus. Infection typically occurs when host, basal cells are exposed to viral infection through a damaged epithelial barrier, such as during sexual intercourse or as a product of other minor skin abrasions. In a few occasions, they can evolve with a marked growth and cause the well-known Buschke-Löwenstein syndrome or Giant condyloma acuminatum. We present a patient with Giant condylomata acuminata in the inguinal and anal regions that were surgically treated with exeresis of the inguinal lesions and electro fulguration of the annals with satisfactory evolution, without tumor recurrence at three months.

Key Techniques Involved in Recovering Gingival Recessions: Abstract Introduction: Gingival recession can be defined as the apical dislocation of the gingival margin in relation to the amelio-cementary junction, or in relation to its original site, which provides a progressive exposure of the root surface. Its etiology is multifactorial, being more related to mechanical trauma, periodontal diseases and predisposing factors. Objective: To analyze the main techniques involved in root coverage of gingival recessions as well as to evaluate tissue response, according to different prognoses. Methodology: A bibliographic survey was conducted from 2006 to 2019 in the databases Lilacs, PubMed and Scielo. The search strategy used was “recession” and “gingival” and “gingival” and “recession” and “treatment”. Articles that did not elucidately portray root coverage methods or evaded the study objective were excluded. Results: The most frequently used techniques were “coronary repositioning flap” and “subepithelial connective tissue graft” because they had more satisfactory results. Conclusion: The literature points to the subepithelial connective tissue graft as the “gold standard”, which, being removed from the patient’s own organism, expresses better results in root coverage and also shows positive results in the Acellular Dermal Matrix (MDA), as it is a Allogeneic graft taken from tissue bank donors, minimizing patient discomfort.
