History of Arthroscopy and Knee Surgery in Turkey 土耳其的关节镜和膝关节手术史

Arthroscopy is an innovative method used in the field of orthopedics to enter the joint by endoscopic method, to see, to take photos and to perform operations. Currently, arthroscopy functions in practice as diagnostic arthroscopy and arthroscopic surgery. The technological development and widespread use of arthroscopy, which is currently very advanced, has taken time. I tried to contribute to the initiation and development of arthroscopy studies in Turkey when I was working in Ege University Faculty of Medicine Orthopedics and Traumatology Clinic and in the “later period” of my professional life. Our aim is to briefly touch on the development of arthroscopy in the world and in Turkey from yesterday up to today, and also to share the information we have witnessed about arthroscopy in Turkey, emphasizing its importance especially in terms of patient and public health

The Association Between Femoroacetabular Impingement and Internal Fixation of Femoral Neck Fractures 股髋臼撞击与股骨颈骨折内固定的关系

Objective: Congenital deformities, slipped capital femoral epiphysis and Legg-Calve-Perthes disease have been suggested as possible causes for femoroacetabular impingement (FAI). The aim of our study is to describe the radiographic signs of FAI after femoral neck fractures and to compare these data with those of the contralateral hip. Method: Our institutional medical records database was retrospectively searched for patients 18-50 years old with a history of femoral neck fractures (OTA 31-B) between 2010-2019. Fifty-two patients were identified. After exclusion criteria, we detected 37 fractures in 37 patients. The mean age of 29 male and 8 female patients was 32,7 (range 18-48) years. The antero-posterior and cross- table lateral views of bilateral hip joints for all 37 patients were reviewed preoperatively and at final follow-up. The mean follow-up period was 48 months (range 6-98 months). In addition, postoperative CT-scans of these patients were also reviewed. Results: According to OTA classification subtypes, 2 subcapital (31B1), 25 transcervical (31B2) and 10 basicervical (31B3) fractures were detected. The mean alpha angle on lateral X-ray of the operated side was statistically significantly higher than the unaffected side. The mean alpha angle on CT was higher on the operated side than the unaffected side. In addition, the acetabular version angle on CT was higher on the unaffected side while acetabular depth on CT was higher on the operated side. The lateral CE angle on the AP X-ray was not different on the unaffected side compared to the operated side. Conclusion: Symptoms of impingement can be seen in patients undergoing internal fixation after femoral neck fracture, and a decrease in acetabular version and an increase in acetabular depth may be predisposing to femoral neck fracture.

Proximal Femur Metastasis Treatment With Fixation or Hip Replacement. Results of 47 Patients 股骨近端转移治疗与固定或髋关节置换术。47例患者结果

Objective: Purpose of the present study was to compare patients with proximal femur metastasis with actual or impending fractures who were treated by fixation or prosthetic hip replacement. Method: Twenty-seven patients underwent fixation treatment (IM nail, DHS), and 20 patients prosthetic (endoprosthesis or total hip arthroplasty) replacement. Data were analyzed regarding patient demographics, cancer type, localization and type of metastasis, actual or impending fracture, number of bone metastasis, presence of spinal or visceral metastasis and treatment data (ASA class, length of hospital stay or surgery or survival, cement usage, adjuvant treatment, postoperative walking status). Results: Fixation group (63 years) was younger than prosthesis group (70 years) (p: 0.03). Fixation was more preferred at subtrochanteric area (p˂0.001). Cementation of the lesion was more preferred and surgery time was longer at fixation group (p: 0.01). Greater number of complications (mostly medical) were more likely to be seen in the fixation group (6 ‘1 loosening’ vs 3 ‘1 dislocation’). Conclusion: It is not still clear whether one implant is clearly superior to other one, however it was revealed again that nailing was mostly preferred for the subtrochanteric area and tended to have more complications although mostly medical and unrelated to implant placement as previously reported

Post-Surgical Mortality Increases in Patients with Covid-19 Positive Hip Fractures Covid-19阳性髋部骨折患者术后死亡率增加

Aim: This study aims to evaluate the mortality and complication rates of patients surgically treated for geriatric hip fractures, and compare the results between groups with and without accompanying Covid-19 diagnosis. Method: 40 patients (22 males and 18 females), including 20 Covid-19 positive patients who underwent bipolar endoprosthesis due to hip fracture in our clinic between April 2020 and July 2021, and 20 Covid-19 negative patients who were matched paired in terms of age, gender, and American Society of Anesthesiologists (ASA) scores as the control group were included in the study. Demographic data of the patients, time to surgery, operative time, length of hospital stay, mobilization status, postoperative wound problems, 90-day mortality, hemoglobin, and albumin parameters were evaluated. Results: Ninety-day mortality, operative time, and hospital stay were significantly higher in the Covid-19 positive group compared to the matched Covid-19 negative control group in terms of age, gender, and comorbidity data (p=0.025, p=0.012, p<0.001, respectively). Conclusion: Patients diagnosed with geriatric hip fractures and accompanying Covid-19, face comorbidities related to both fracture and systemic Covid-19 infection. An early orthogeriatric examination, thorough planning, and prompt surgical intervention are mandatory to achieve favorable outcomes.

A Successful Treatment Of Femoral Neck Open Fracture In Middle-Aged Adult: A Case Report 成功治疗中年成人股骨颈开放性骨折1例

Background: We report an atypical case of a middle-aged adult male who wounds with gunshot injury of the proximal femur with femoral neck loss. Case presentation: We present a 41-year-old male patient who wounds with a high-energy ballistic injury and had also nerve injury due to blastic effect. We present our treatment stages by protecting the patient from infection and by complying with the damage-controlled surgery principles. Conclusion: Those gunshots involving major joints, especially the one on the hip, could be lethal. The comminuted femoral neck and periarticular soft tissue injuries made open reduction with internal fixation difficult. The nature of the high-energy ballistic injury increases the possibility of infection that may have contributed directly to prosthesis failure. As a result, we have treated the patient with three-stage surgery. The patient underwent total hip arthroplasty, and the patient lived a functional, satisfying life after surgery.
