Giant cell fibroblastoma is a rare intermediate grade fibrohistiocytic tumour of childhood and even rarer in adults. It is seen in first decade of life. It is known for its recurrence, but its metastasis has never been reported. Giant cell fibroblastoma requires attention as it was previously misdiagnosed as sarcoma. So histology is mandatory and remains the gold standard for its diagnosis. We present a case of 21 year old female who presented with complains of painless lesion over the dorsal aspect of left hand.

Delta bilirubin is albumin bound conjugated bilirubin that can be calculated by formula [Total bilirubin - (conjugated + unconjugated bilirubin)]. Newer technology like dry chemistry provide conjugated and unconjugated bilirubin fraction which is more specific and informative than direct and indirect bilirubin by conventional method in which only approximate values of conjugated bilirubin can be measured. In cholestasis conjugated bilirubin combines with albumin non enzymatically and form delta bilirubin in circulation and remains for long duration. The present review describes the usefulness of delta bilirubin in cholestasis, chronicity of liver diseases, in pediatric jaundice, transplant rejection and other jaundice condition.clinical correlation of delta bilirubin in all conditions have been summarized and their future perspectives are discussed.

Clinical manifestations of Mullerian duct abnormalities vary greatly and can range from isolated abnormalities of a single reproductive organ to multiorgan aplasia. An early and precise diagnosis is essential because untreated symptoms may result in pelvic endometriosis, retrograde tubal reflux, and infertility. MRI and ultrasonography are the imaging modality that accurately helps in the diagnosis of mullerian duct anomalies. Two interesting cases came to C. R. Gardi OPD with Mullerian duct anomalies. The first case came with dysmenorrhea and a lower abdominal palpable mass upto 14 wk size was present, which was diagnosed to be bicornuate uterus, obstructed hemivagina. It is a diagnostic dilemma because menses were regular. Another case came with severe cyclic dysmenorrhea and was found to be a hematometra in the non-communicating rudimentary horn on the left side.

Alcohol is the oldest recreational substance, and its consumption is increasing all over the world as it produces a pleasurable effect and also it allows people to relax and unwind. A large number of over the counter and prescription medicine interacts with Alcohol by Pharmacokinetic and/or Pharmacodynamic mechanisms. Alcohol consumption with drugs having CNS depressant property may lead to marked impairment in psychomotor performance and making the person’s injury prone. Alcohol consumption increases the systemic toxicity and organ dysfunction caused by certain drugs. Alcohol consumption may cause unpleasant manifestations with a few drugs/substances (didisulfiram-likeeaction). Alcohol may reduce the therapeutic effect of a few drugs. Awareness about the various interactions of Alcohol with drugs/medicines/food/herbals/occupational agents will ensure the safety of the people consuming Alcohol and may help the physician in clinical decision-making.

Background- Blood stream infection is a very important cause of morbidity and mortality worldwide, especially in resource limited countries. It ranges from transient bacteremia to life-threatening septic shock. Blood culture is a gold standard method of diagnosis of infectious agents present in the blood.Method- This study was conducted From August 2021 to October 2021 at Ruxmaniben Deepchand Gardi medical and Ujjain Charitable trust Hospital, Ujjain (M.P.). All received blood samples were processed, isolates were identified, and antibiotic susceptibility testing was performed using the Vitek 2 Compact (Biomerieux) system in the Microbiology laboratory.Results- A total of 74(17%) pathogens were isolated from 432 blood samples. Gram-positive bacteria 47(64%) were predominant organisms obtained, followed by gram-negative bacteria 25(34%) and non-albicans candida species 2(2.7%). The predominant pathogens were Staphylococcus aureus 40(54%), Klebsiella pneumoniae and Pseudomonas aeruginosa 6(24%) each. Among Staphylococcus aureus isolates 36 (90%) were methicillin-resistant Staphylococcus aureus (MRSA). And among gram-negative bacteria, all isolates of K. pneumoniae were extensively drug-resistant. Majority of drug-resistant organisms were isolated from neonatal intensive care unit (%).Conclusion- Successful treatment of bloodstream infection depends on early diagnosis and appropriate use of antimicrobial agents.

Background: Research is essential for the progress of the medical field. The research base should be developed from the undergraduate level itself. Though there are efforts taken by universities to fund Undergraduate research, the involvement of students is not up to the mark.Objective: The present study was undertaken to observe the perceptions of first-year medical students towards undergraduate research.Materials and methods: The present cross-sectional study was conducted at Alluri Sitaramaraju Academy of Medical Sciences (ASRAM) Medical College, Eluru, Andhra Pradesh. A total of 80 willing participants of first-year MBBS were included in the study. Informed consent was obtained duly from all participants. A standard questionnaire was given to students to answer by themselves.Results: 90% of students agreed that research helps them for better learning. 91% of students agreed that research upgrades their knowledge. 96% agreed that research is absolutely needed in the medical field. 23% reported that they will opt for research as a career option. 14% reported that they have knowledge about funding agencies of research. 70% of students reported that they need research to be inculcated in the curriculum.Conclusion: The study highlights that there is a strong need to increase awareness about medical research in undergraduate students and motivate them to apply for funding from agencies like Indian Council of Medical Research, Short Term Studentship and from university funding. Further detailed studies are recommended

A 19-year old patient with 8th-month pregnancy was admitted in our hospital with severe COVID infection. She was put on oxygen support and managed as per COVID protocol. She had spontaneous vaginal delivery and the baby was found to be COVID-negative. She developed initially left and later right-sided Pneumothorax. She had prolonged hospitalization and was managed with intercostal drains, NIV, Remdesivir and chest physiotherapy. The pneumothoraxes resolved and she made a satisfactory recovery and could be discharged on room air.

My association with R. D. Gardi Medical College is long. I was a classmate of Dr V. k. Mahadik from 1967 to 1971 as we graduated with MBBS. Later for different reasons I traveled to Zambia and worked for the Government of Zambia Ministry of Health at two districts, Mporokoso and Luwingu, both in the northern province. However, contact with Dr Mahadik was continuous. On my completion of contract in Zambia and on my way to the Bahamas, I visited Ujjain. With other friends, we had discussions about developing a healthcare institution in the form of a hospital. We wanted a cooperative hospital and even contributed funds to this end. However, politicians and individuals advised us to create a charitable trust and a hospital. There are many steps, hard work, and diplomacy, mostly by Dr Mahadik to achieve what is a medical college and other hospitals associated with this. Mahadik from 1967 to 1971 as we graduated with MBBS. Later for different reasons I traveled to Zambia and worked for the Government of Zambia Ministry of Health at two districts, Mporokoso and Luwingu, both in the northern province. However, contact with Dr Mahadik was continuous. On my completion of contract in Zambia and on my way to the Bahamas, I visited Ujjain. With other friends, we had discussions about developing a healthcare institution in the form of a hospital. We wanted a cooperative hospital and even contributed funds to this end. However, politicians and individuals advised us to create a charitable trust and a hospital. There are many steps, hard work, and diplomacy, mostly by Dr Mahadik to achieve what is a medical college and other hospitals associated with this. Only a person with imagination, persistence, technical knowledge and patience knows how this could be achieved.
