Microbiological Profile and Antimicrobial Resistant Pattern among Isolates from Bloodstream Infection in a Tertiary Care Hospital 某三级医院血液感染分离株微生物学特征及耐药模式

Background- Blood stream infection is a very important cause of morbidity and mortality worldwide, especially in resource limited countries. It ranges from transient bacteremia to life-threatening septic shock. Blood culture is a gold standard method of diagnosis of infectious agents present in the blood.Method- This study was conducted From August 2021 to October 2021 at Ruxmaniben Deepchand Gardi medical and Ujjain Charitable trust Hospital, Ujjain (M.P.). All received blood samples were processed, isolates were identified, and antibiotic susceptibility testing was performed using the Vitek 2 Compact (Biomerieux) system in the Microbiology laboratory.Results- A total of 74(17%) pathogens were isolated from 432 blood samples. Gram-positive bacteria 47(64%) were predominant organisms obtained, followed by gram-negative bacteria 25(34%) and non-albicans candida species 2(2.7%). The predominant pathogens were Staphylococcus aureus 40(54%), Klebsiella pneumoniae and Pseudomonas aeruginosa 6(24%) each. Among Staphylococcus aureus isolates 36 (90%) were methicillin-resistant Staphylococcus aureus (MRSA). And among gram-negative bacteria, all isolates of K. pneumoniae were extensively drug-resistant. Majority of drug-resistant organisms were isolated from neonatal intensive care unit (%).Conclusion- Successful treatment of bloodstream infection depends on early diagnosis and appropriate use of antimicrobial agents.

Achalasia cardia: A Case Report and a Brief Review 贲门失弛缓症1例报告及简要回顾

Achalasia cardia (AC) is one of the rarely seen disorders of esophagus motility which happens as a result of degeneration of neurons which is irreversible. Treatment is mainly of palliative nature, and no complete curative treatment is available as of now. The diagnostics and therapeutics in this direction has strengthened mainly due to high-resolution manometry and per-oral endoscopic myotomy, referred to as Peroral endoscopic myotomy (POEM).The characterization of type of achalasia cardia is made easier by high-resolution manometry as we have mentioned, which also has an important therapeutic role. Achalasia has to be managed for each patient individually, and the role of pneumatic balloon dilatation, POEM, or Heller’s myotomy needs to be reconsidered. In our case report and brief review, we discussed the main aspects of diagnosing an OPD-based patient and a brief review of achalasia cardia.

Undergraduate Medical Research: Perception of First MBBS Students 本科医学研究:第一届MBBS学生的感知

Background: Research is essential for the progress of the medical field. The research base should be developed from the undergraduate level itself. Though there are efforts taken by universities to fund Undergraduate research, the involvement of students is not up to the mark.Objective: The present study was undertaken to observe the perceptions of first-year medical students towards undergraduate research.Materials and methods: The present cross-sectional study was conducted at Alluri Sitaramaraju Academy of Medical Sciences (ASRAM) Medical College, Eluru, Andhra Pradesh. A total of 80 willing participants of first-year MBBS were included in the study. Informed consent was obtained duly from all participants. A standard questionnaire was given to students to answer by themselves.Results: 90% of students agreed that research helps them for better learning. 91% of students agreed that research upgrades their knowledge. 96% agreed that research is absolutely needed in the medical field. 23% reported that they will opt for research as a career option. 14% reported that they have knowledge about funding agencies of research. 70% of students reported that they need research to be inculcated in the curriculum.Conclusion: The study highlights that there is a strong need to increase awareness about medical research in undergraduate students and motivate them to apply for funding from agencies like Indian Council of Medical Research, Short Term Studentship and from university funding. Further detailed studies are recommended

Statistics may be defined as “Methods to plan a study scientifically, collect quality data, analyze appropriately, interpret analytical results correctly and derive implications accurately.” To be more specific, all methods of gaining knowledge are essential statistics. Statistics principles remain the same regardless of the area in which it is used. However, it is differentiated by area of its use, like biostatistics, while it is used in medical sciences, business statistics in business research, and agricultural statistics in agricultural sciences. A host of literature is available on various aspects of statistics, including its varying applications, uses and misuses. The present write-up aims to briefly emphasize the need of strong biostatistics components in medical colleges in India.

Bilateral Pneumothorax in a case of severe COVID-19 Pneumonia with Pregnancy 重症COVID-19肺炎合并妊娠双侧气胸1例

A 19-year old patient with 8th-month pregnancy was admitted in our hospital with severe COVID infection. She was put on oxygen support and managed as per COVID protocol. She had spontaneous vaginal delivery and the baby was found to be COVID-negative. She developed initially left and later right-sided Pneumothorax. She had prolonged hospitalization and was managed with intercostal drains, NIV, Remdesivir and chest physiotherapy. The pneumothoraxes resolved and she made a satisfactory recovery and could be discharged on room air.

Self-Reliance is the best strategy for development 自力更生是最好的发展战略

My association with R. D. Gardi Medical College is long. I was a classmate of Dr V. k. Mahadik from 1967 to 1971 as we graduated with MBBS. Later for different reasons I traveled to Zambia and worked for the Government of Zambia Ministry of Health at two districts, Mporokoso and Luwingu, both in the northern province. However, contact with Dr Mahadik was continuous. On my completion of contract in Zambia and on my way to the Bahamas, I visited Ujjain. With other friends, we had discussions about developing a healthcare institution in the form of a hospital. We wanted a cooperative hospital and even contributed funds to this end. However, politicians and individuals advised us to create a charitable trust and a hospital. There are many steps, hard work, and diplomacy, mostly by Dr Mahadik to achieve what is a medical college and other hospitals associated with this. Only a person with imagination, persistence, technical knowledge and patience knows how this could be achieved. In my view, Dr Mahadik has all these qualities.

Skull or scalp metastasis is commonly seen in carcinoma of breast, prostate, kidneys and lungs. Here we report a case of 80-year-old male who presented with scalp swelling for treatment. High resonance ultrasonography (HRUS) and contrast-enhanced computed tomography (CECT) of brain showed large soft tissue lesion on right occipito-parietal region with erosion of underlying bone, suggestive of neoplastic etiology. Subsequently, on further workup, his CT thorax showed lesion in right hilar and lower lobe of right lung with mediastinal lymph nodes. Patient underwent excision of the right occipito-parietal lesion which on histopathology was reported as epithelial malignancy, favoring metastatic moderately differentiated squamous cell carcinoma. This case is reported here to emphasize the importance of good clinical workup to rule out metastatic lesions which might be the initial presentation of an unknown primary at other site.

Antimicrobial Susceptibility Pattern of Klebsiella pneumoniae Isolated from Various Clinical Specimens in Ujjain City, Madhya Pradesh, India 印度中央邦乌贾因市不同临床标本分离肺炎克雷伯菌的药敏特征

Klebsiella pneumoniae is an important opportunistic pathogen that commonly causes nosocomial infections and contributes to substantial morbidity and mortality. We sought to investigate the sensitivity pattern of K. pneumoniae isolated from various clinical specimens. Methods- This cross-sectional study was conducted at Ruxmaniben Deepchand Gardi Medical College and Ujjain Charitable Trust Hospital in Ujjain City during March 2021 to September 2021 on total 463culture positive isolates from various clinical specimens. Identification of K. pneumoniae and antibiogram testing were performed by using VITEK-2 compact bacterial identification and monitoring system (bioMerieux, US). The data were analyzed by using NCSS statistical software 2021. Results- In total, 83 positive K. pneumoniae cultures from various clinical samples were examined. The isolates were more predominant in the females (n = 45; 54.2%) compared to males (n = 38; 45.8%). The antibiotic resistance rate of K. pneumoniae varied among different isolate clinical sample sources. Overall, high resistance rates were recorded for ampicillin (100%), amoxyclav (81.48%),  piperacillin/ tazobactam (73.68%), cefuroxime (94.74%), ceftriaxone (91.2%), cefotaxime (91.2%),  and cefepime (73.68%). A total of 83 K. pneumoniae isolates, 75 were ESBL (90.36%) producers. However, colistin (100%) showed the highest susceptibility rate against the isolates. Conclusion- the wide variety of MDR K. pneumoniae harboring β-lactams and virulence genes strongly suggest a necessity for the implementation of effective strategies to prevent and control the spread of antibiotic resistant infections.
