The Effects on the Cognitive Function of Healthy Volunteers of a Combination of Acetyl-L-Carnitine, Vinpocetine and Huperzine A Administered Over 28 Days 乙酰左旋肉碱、长春西汀和石杉碱a联合用药28天对健康志愿者认知功能的影响

This report on a 4 week, randomized, placebo controlled, parallel group, repeated measures, double-blind study clinical trial of a preparation containing Acetyl-L-Carnitine, Vinpocetine and Huperzine A administered to healthy volunteers has identified that the product significantly improves a well validated global score from six tests of working and episodic memory, with an effect sized improvement comparable to the current medications approved to treat Alzheimer’s disease.

Posterior Circulation Infarcts and Carotid Disease. Is There a Correlation? 后循环梗塞和颈动脉疾病。有相关性吗?

We present a case of a 66-year-old man who presented with a history of Right Hemianopia and Right sided facial droop and weakness. CT and MRI brain imaging demonstrated areas of infarction affecting the vascular regions supplied by Left Middle Cerebral Artery and Left Posterior Cerebral Artery circulation. Carotid Duplex Arterial imaging demonstrated moderate Left Internal Carotid Artery Stenosis and CT Angiography demonstrated a dominant Left Posterior Communicating Artery. The origin of the multi-territory Stroke as demonstrated on the MRI scan was his underlying Carotid disease. The patient underwent a successful Left Carotid Endarterectomy. This case highlights the fact that patients presenting with isolated Posterior Cerebral Arteryterritory stroke or mixed Posterior Cerebral Artery and Middle Cerebral Artery territory strokes and ipsilateral Internal Carotid Artery stenosis, should have a low threshold to be assessed with CT or MR Brain Angiography to determine if variants in Cerebral Circulation exist.

Positive Airway Pressure Compliance in Patients with Epilepsy and Obstructive Sleep Apnea 癫痫和阻塞性睡眠呼吸暂停患者的气道正压依从性

Obstructive sleep apnea (OSA) affects approximately one third of patients with epilepsy. Treatment with Continuous Positive Airway pressure (CPAP) is associated with improved seizure control, but is difficult for patients to tolerate. Patients with epilepsy and co morbid OSA may be at higher risk of recurrent seizures if they are noncompliant with CPAP. This study investigates short-term compliance with CPAP therapy in patients with OSA and epilepsy, as this can predict long-term adherence. Identifying factors influencing compliance rates in this population may help improve compliance and reduce seizure recurrence rate in this population. We also investigated the impact of epilepsy on sleep, and the possible effects this has on CPAP compliance.

Unawareness of Olfactory Dysfunction in Older Adults 老年人嗅觉功能障碍的无意识

Deterioration of olfaction is a common phenomenon observed in the senior population. A number of factors may cause this deficit including infections, aging and neurodegenerative diseases. The aim of this study was to evaluate the reliability of the self-report as a measure of olfactory function in seniors. A total of 93 community-dwelling participants (43 men and 50 women) from the Quebec NuAge cohort on Nutrition and Successful Aging participated in the Olfactory Response and Cognition in Aging (ORCA) study. The age range was 80-95 years and all subjects had a telephone mini mental state examination (t-MMSE) score > 18. Individuals were interviewed using a self-report (“do you suffer from smell problems?’’) and quantitative (University of Pennsylvania Smelling Identification Test (UPSIT)) olfactory tests. Based on the self-report, 81% of the participants claimed to have a normal sense of olfaction. However, based on the UPSIT, 95% of them showed different forms of microsmia. These results reveal that most senior citizens are unaware of their olfactory dysfunction and indicate that an self-report questionnaire is not a valid instrument to assess olfactory function in the aging population.

Cognitive-Functional Interaction in Patients with Vascular Dementia 血管性痴呆患者的认知功能相互作用

Background: There is a close interaction between cognitive and functional performance in the normal brain. It increases in cases of brain disease, where a worse initial cognitive performance is associated with subsequent functional decline. Objective: To describe the cognitive and functional performance profile and their interaction in a population of patients with Vascular Dementia (VD). Methods: We studied 79 patients (Group 1, G1) with VD and 80 healthy subjects without brain disease (Group 2, G2). The following tests were administered to all the study population: ADAS Cog., Trail Making Test (TMT), Mini Mental State Examination (MMSE), Basic and instrumental activities of daily living (ADL, IADL), Disability Assessment for Dementia (DAD), and Gottfries-Brane-Steen Scale (GBS). Scores were statistically evaluated usingthe ANOVA Test, Spearman’s correlation matrix and Principal Component Analysis (PCA). Results: The average age of G1 was 72.6 + 7.1 and their literacy level was 8.05 + 4.76 years. The cognitive profile showed a significant global decline in the MMSE (19.9 + 3.77 points) and in the ADAS Cog (29.2 + 8.80 points). In this group, the cognitive functions most compromised were Memory, Orientation and Recognition and, to a lesser degree, Attention, Language and Praxis. The functional profile showed the greatest impairment in IADL. The PCA showed a correlation of ADAS with TMT and Katz Index, and of MMSE and DAD. Conclusions: The results highlight the interaction between complex functional performance and cognition, suggesting an impact of cognitive impairments linked to executive function on functional performance, of potential therapeutic significance.

Prevention and Treatment of Basic Diseases: A Potential Direction to Intervene Alzheimer’s Disease 基础疾病的防治:干预阿尔茨海默病的潜在方向

As the life span of human beings increases, the number of dementia patients such as Alzheimer’s disease (AD) is increasing. AD may be one of the major diseases faced by the whole world in the future. It is an unresponsive for single drug according to the current stage of AD pathogenesis and drug research. Combined use of various drugs may be the inevitable trend of AD drug research in the future. Personalized treatment of metabolic diseases, vascular diseases and chronic inflammatory risk diseases is a necessary step to intervene in AD. During the middle age, to strengthen immunity and to reduce the external environment to stimulate the susceptible gene, which is beneficial to suppress the formation of AD. In a word, the early prevention of AD is better than the later treatment.

Diverse groups of ototoxic agents and gene mutation induce hearing disorders. Several studies showed that increased oxidative stress, inflammation, and glutamate are involved in the initiation and progression of hearing disorders. Therefore, reducing these biochemical defects before and after exposure to ototoxic agents may prevent and improve the management of hearing loss. Previous studies with a single agent have yielded variable benefits in hearing disorders. This review briefly presents evidence for the involvement of oxidative stress, inflammation, and glutamate in the initiation and progression of hearing disorders. It proposes a novel concept that simultaneous elevation of the levels of antioxidant enzymes, dietary and endogenous antioxidant compounds, and reduction in the levels of glutamate may be necessary for the optimal benefits in prevention and improved management of hearing disorders. Supplementation with micronutrients can elevate the levels of antioxidant compounds and reduce the levels of glutamate. However, increasing the levels of antioxidant enzymes is complex requiring an activation of the nuclear transcriptional factor-2 (Nrf2). This review briefly describes the regulation of activation of Nrf2, and proposes a micronutrient mixture that can simultaneously activate Nrf2, enhance the dietary and endogenous antioxidant compounds levels, and inhibit the release and toxicity of glutamate for prevention and improved management of hearing loss.

The use of marihuana is becoming more frequent in young people, the association between psychosis and cannabis has been described, especially in young people, we present a clinical case of psychosis associated with cannabis in a young man with normal Brain Magnetic resonance, but abnormalities in tractography mainly in the connectivity between cuadate nucleus and frontal lobe.
