Osteosynthesis of femoral shaft fractures at Brazzaville University Hospital: Nail versus screw plate 布拉柴维尔大学医院股骨干骨折的骨合成术:钢钉与螺钉钢板

Introduction: Fractures of the femoral shaft are serious traumatic conditions frequently encountered in hospital. The aim of our study was to compare two different surgical techniques for their treatment: anterograde centromedullary nail and compression plate. Materials and Methods: From September 2018 to August 2023, 208 femoral shaft fractures were recorded out of 3236 hospitalizations, i.e. 6.42%. Two groups of patients were selected: group 1 with 122 patients treated with anterograde nails and group 2 with 49 patients treated with screw plates. These two groups were compared according to fracture type (AO/OTA), time to surgery, follow-up and consolidation, and complications. Data were processed in Excel and SPSS 19.0 with significant p-value when less than or equal to 0.05. Results: The time to surgery was 29.11±1.13 days for group 1 and 27.74±2.41 days for group 2 (p=0.0005). Most of the patients were treated in the middle third (78.68% in group 1 and 65.3% in group 2). Bone consolidation was achieved at 4.91±0.52 months for group 1 and 5.72±0.34 months for group 2 (p=0.000007). The main complication was limb length inequality in 6 cases (4.92%) in group 1 and 9 cases (18.36%) in group 2. Discussion: Although our study was monocentric, it was carried out in the largest hospital in the country. The middle third was the most frequent site for us, as it was for most authors. This is the area most exposed to high-energy trauma and torsional forces. Complications are due to delayed surgery and unfavourable operating conditions. Conclusion: Comparing the two main surgical techniques, the results are better for the anterograde femoral centromedullary nail. The choice of implant depends on the type of fracture, the surgeon's habits and the implants available.

Clinical and functional outcome of inter-trochanteric fractures in the elderly patients using Proximal Femoral Nail Anti-rotational-II (PFNA-II) 使用股骨近端抗旋转钉-II(PFNA-II)治疗老年转子间骨折的临床和功能疗效

Introduction: Intertrochanteric fractures of the femur constitute nearly half of all the fractures around the hip and are known to be associated with long-term functional impairment, nursing home admission and increased mortality. Intertrochanteric fractures are routinely operated, which enables early recovery and faster rehabilitation of the patient. The choice of implant used in the management of intertrochanteric fractures has been a subject of debate, especially with the development of various devices over the years. The aim of present study was to evaluate clinical and functional outcome of intertrochanteric fractures in elderly patients using proximal femoral nail anti-rotation-II (PFNA-II). Methods: The present study was carried out 30 patients to evaluate clinical and functional outcome of intertrochanteric fractures in elderly patients using proximal femoral nail anti-rotation-II (PFNA-II). On examination, presence of any tenderness, deformity, limb length discrepancy and range of motion were noted. Functional outcomes were assessed with the help of Harris Hip Score. Results: The mean time to full weight bearing was calculated to be 9.5 (range 6-12) weeks. At final follow-up, half of the patients reported no pain in the hip while 11 cases had slight, occasional pain The mean time to union in our study was calculated to be 11.3 weeks. The mean range of motion at 6 months follow-up was 226 (range 175-270) degrees. The average Harris hip score was 90.6 (range 85-100). In this study, according to Harris hip score all patients were graded excellent or good scores Conclusion: The present findings suggest that PFNA applied in elderly patients with inter-trochanteric fracture can get satisfactory effects, clinical outcome and a lower radiological complication rate.

Background: Osteoporosis, characterized by reduced bone mineral density, increased fracture risk and impaired quality of life, remains a significant global health concern. Collagen, serving as the foundational structure for bone mineralization, emerges as a crucial component in the armamentarium against osteoporosis. Objective: This expert opinion captures insights from a panel of experts, highlighting the role of specific collagen peptides in the management of osteopenia and osteoporosis. Methods: To achieve this consensus, a group of 13 orthopedics experts from across India came together in Mumbai on 15 th October 2023 to review the existing clinical data and discuss the role of specific collagen peptides in the management of osteopenia and osteoporosis. Results: Collagen, as the fundamental structural component for bone mineralization, emerges as a critical element in osteoporosis management. Specific Collagen Peptide, being a small chain of amino acids, is easily absorbed from the GI tract and, via the circulatory system, reaches the bone marrow, where it activates the formation of osteoblast from bone marrow stem cells plus also inhibits the activity of osteoclast through an increase in the OPG/RANKL ratio. Specific Collagen Peptides (SCP) demonstrated enhancements in Bone Mineral Density (BMD) by 4.2% in the spine and 7.7% in the femoral neck, coupled with alterations in bone turnover markers in 12 months. A clinically relevant increase in BMD was observed in the 4-year follow-up study; no fractures were reported by any patients, and no drug interaction and tolerability issues were noted in the study. Conclusion: The consensus among experts underscores the necessity of integrating once-a-day oral 5 gm specific collagen peptide supplementation (Nutra with a drug-like action) into standard osteoporosis management. SCP, with its dual action on both osteoblast and osteoclast (Along with Calcium and vitamin D), can be used stand-alone or can be used for its synergistic action when used along with anti-resorptive (Bisphosphonate/denosumab) as well as anabolic (teriparatide) treatment.

Collagen, serving as the foundational structure for bone mineralization, emerges as a crucial component in the armamentarium against osteoporosis. Objective: This expert opinion captures insights from a panel of experts, highlighting the role of specific collagen peptides in the management of osteopenia and osteoporosis. Methods: To achieve this consensus, a group of 13 orthopedics experts from across India came together in Mumbai on 15 th October 2023 to review the existing clinical data and discuss the role of specific collagen peptides in the management of osteopenia and osteoporosis. Results: Collagen, as the fundamental structural component for bone mineralization, emerges as a critical element in osteoporosis management. Specific Collagen Peptide, being a small chain of amino acids, is easily absorbed from the GI tract and, via the circulatory system, reaches the bone marrow, where it activates the formation of osteoblast from bone marrow stem cells plus also inhibits the activity of osteoclast through an increase in the OPG/RANKL ratio. Specific Collagen Peptides (SCP) demonstrated enhancements in Bone Mineral Density (BMD) by 4.2% in the spine and 7.7% in the femoral neck, coupled with alterations in bone turnover markers in 12 months. A clinically relevant increase in BMD was observed in the 4-year follow-up study; no fractures were reported by any patients, and no drug interaction and tolerability issues were noted in the study. Conclusion: The consensus among experts underscores the necessity of integrating once-a-day oral 5 gm specific collagen peptide supplementation (Nutra with a drug-like action) into standard osteoporosis management. A study of local corticosteroid injection versus dry needling in Lateral Epicondylitis (LE) 局部皮质类固醇注射与干针治疗外侧上髁炎(LE)的对比研究

Introduction: Lateral epicondylitis (LE) is an inflammation or micro-tearing of the tendons that join the forearm muscles on the lateral aspect of the elbow. The first line treatment for LE is topical and oral anti-inflammatory drugs from ice applications and brace used. If the first line treatment fails second line treatment generally invasive are offered and second-line therapeutic regimens include saline, corticosteroid or platelet-rich plasma injections. Dry needling is relatively new for treating the same. We hypothesized that dry needling would be as effective as second-line treatment for LE. The aim of this study was to compare the outcomes of second-line treatment local steroids and dry needling. Methods: In this study a total of 42 patients, clinically diagnosed with LE utilizing provocative tests and point tenderness at the insertion of the ECRB at the lateral epicondyle to compare the outcomes of second-line treatment local steroids and dry needling. Results: In group A (DN group), the mean PRTEE score before the start of therapy, at the 4 th week and 8-weeks follow-up was 68.96±6.89, 44.13±5.23 and 37.18±5.81 respectively. In group B (corticosteroid group), the mean PRTEE score before the injection, at the 4 th week and 8-weeks follow-up was 65.23±4.82, 51.08±6.32 and 43.72±4.12 respectively. Conclusion: Both the techniques have proven good results at defined intervals at regular follow ups. But as the PRTEE score we found both the treatment are equally effective. But due to less complications we preferred dry injection over local steroids. More Comparative studies should be conducted to compare dry needling with other treatment modalities.
