Extensor mechanism sparing para tricipital posterior approach to the distal humeral shaft fractures: An alternative approach for fracture treatment 伸肌机制保留肱三头旁后路肱骨远端骨折:骨折治疗的另一种方法

Introduction: Generally Anterolateral and posterior surgical approaches are most commonly used approaches for fractures of middle thirds and distal third of the humerus. In posterior approach, usually splitting of triceps is done for exploring humerus. We studied an alternative, triceps reflecting para tricipital approach which spares the extensor mechanism. We are using it for the lower third extra articular humerus fracture to achieve good exposure of humerus so as to avoid muscle splitting and gaining the early ROM range of motion [1, 2] . Materials and Methods: We studied 15 skeletally mature distal humerus fractures at our center treated by Triceps sparing approach with minimum follow up for a period of 6 months. They were functionally assessed with MAYO score [3] , measurements of ROM and radiological union. Mean duration of follow up was 10 months with range from 6-14 months. Results: The average Mayo score was 95 indicating an excellent performance with a mean functional ROM 132 degree with range from 120-140 degree. Only 1 patient had complication of infection which was not related to approach. Conclusion: The triceps reflecting approach (TRA) is a valuable option for ORIF in distal shaft humerus fractures. The extensor mechanism-sparing Paratricipital approach is an invaluable approach for fixation of lower third fractures of the humerus without negative effects on triceps strength. The clinical outcome parameters of our series revealed excellent maintenance of strength compared to the contralateral side. This approach gives us liberty to mobilize elbow joint actively as well passively and also avoids complications related to triceps adhesions.

Assessment of proximal tibial fractures managed with locking compression plate 锁定加压钢板治疗胫骨近端骨折的疗效评估

Background: Proximal tibial fractures refer to fractures that occur in the upper part of the tibia, which is the larger bone in the lower leg. The present study was conducted to assess management of proximal tibial fractures by locking compression plate. Materials & Methods: 70 proximal tibial fractures of both genders were treated with MIPO and ORIF. Parameters such as side, type of fracture, type of reduction, principal, operative time, blood loss, hospital stay, healing time etc. were recorded. Results: Out of 70 patients, males were 40 and females were 30. Side was left in 32 and right in 38. Type of fracture was pure cleavage in 29, cleavage with depression in 15, central depression in 5 cases, medial condyle fracture in 6, bicondylar fracture in 8 and metaphysio diaphyseal dissociation in 7 cases. Method of reduction was MIPO in 48 and ORIF in 22. Principle of reduction was compression in 42, bridging in 18 and combined in 10 cases. The difference was significant ( p <0.05). The mean operative time (min) was 92.3, blood loss (ml) was 121.5, hospital stay (days) was 11.4, healing time (weeks) was 9.2 and time of recovery to work (days) was 11.4 days. Common complications were infection in 4, varus deformity in 2, knee instability in 1 and knee joint stiffness in 3 cases. The difference was significant ( p <0.05). Conclusion: Open reduction and internal fixation with LCP is an excellent technique for treatment of proximal tibia fractures.

Clinical outcomes of minimal invasive plate osteosynthesis in comminuted fracture of long bones 微创钢板内固定治疗长骨粉碎性骨折的临床效果

Background: MIPO technique, with less periosteal stripping, may facilitate bone healing by preserving soft tissue and promoting oxygen tension in the fracture hematoma. The present study was conducted to assess the functional and clinical outcomes of Minimal Invasive Plate Osteosynthesis in comminuted fracture of long bones. Materials & Methods: A total of 28 patient’s patients were selected for the study. After a patient with comminuted fractures of long bones was admitted to the hospital, all the necessary clinical details were recorded in proforma prepared for surgery. The cases were studied on the basis of gender, age, site of injury, classification and mode of injury. Postoperatively, patients were evaluated for radiological investigations and functional outcome. Results: The mean age of the patients was 38.03 years and 24 (86%) were males. Outcome was excellent in 16 patients (61.54%) followed by Good in 7 patients (26.93%) and Poor in 3 patients (11.53%) due to non-union. Conclusion: Minimally Invasive Plate Osteosynthesis (MIPO) technique using Locking Compression Plate (LCP) is a safer and better procedure than the conventional techniques used to manage comminuted fractures of long bones.
