Primary benign fibrous histiocytoma of bone: A case report 原发性骨良性纤维组织细胞瘤1例

Benign Fibrous Histiocytoma is such a rare tumor that only a few cases have been reported in the literature. A patient with an apparently benign lesion of the distal femur presented at our Outpatient Department. Pain was the chief complaint. There were histological features of Non-ossifying Fibroma in the lesion. Because of its unusual radiological appearance and atypical clinical course, the lesion was diagnosed as Fibrous Histiocytoma. In this case report, we discuss about such a rare case. This 55 years old male patient with history of chronic kidney Disease came with complaints of pain over medial aspect of left knee, initially was managed elsewhere with conservative management. Referred to Orthopaedic surgery department for further management. After all necessary investigations, was planned for Incisional Biopsy. After the biopsy report, was planned for definitive management with Extended Curettage and Prophylactic fixation.

Study of outcome of fluoroscopic guided transforaminal lumbar epidural steroid injection in patients with low back pain with radiculopathy 透视引导下经椎间孔腰椎硬膜外类固醇注射治疗腰痛伴神经根病的疗效研究

Objectives: Evaluation of the outcome and the effects of fluoroscopic transforaminal epidural steroid injections in long term in patients having back pain with radicular leg pain. Background Data: Many studies have evaluated the efficacy of traditional trans sacral (caudal) or translaminar (lumbar) administration of epidural steroids, no studies have assessed specifically the therapeutic value of fluoroscopic transforaminal epidural steroids. Study Design: A prospective study that investigated the outcome of patients with radicular leg pain due to lumbar herniated disc who were injected with fluoroscopic transforaminal epidural steroid injections. Methods: All the patients included in our study received contrast-enhanced, fluoroscopically guided, transforaminal epidural injection of steroid at the documented pathological site. Patients’ evaluation was done by the independent observer. All the patients received sequential questionnaires pre-and postinjection, documenting pain, activity, and patient satisfaction. Results: 20 patients taken in this study and follow up taken for an average 52 weeks; 14 patients had a good outcome, reporting at least a >50% reduction between pre-injection and post-injection pain scores, as well as an ability to return to their activity of daily living after only 1.5 injections per patient. Out of 20 patients 16 were happy with their outcomes. Conclusions: Fluoroscopic transforaminal epidural injection of steroids are an effective conservative treatment option for patients with lumbar disc prolapse and radiculopathy when other conservative treatments are not so effective and before undergoing surgical intervention.

A study on incidence of early surgical site infections in postoperative orthopaedic cases 骨科术后早期手术部位感染发生率的研究

Background: Surgical site infection (SSI) has always been a major complication of surgery and trauma and has been documented for 4000-5000 years. The present study analysed the various organisms causing infections and to study the incidence of surgical site infections in postoperative orthopaedic cases in tertiary care hospital in India. Materials & Methods: The present study comprised of 100 orthopaedic cases of both genders in the department of Orthopaedics at a tertiary care centre. Patients with wound infection (pain or tenderness, localized swelling, redness or heat), a sample was taken of surgical site and transported to the microbiology laboratory for culture. Any surgical site infection (increased tempt, redness, discharge, suture gaping, and Southampton wound scoring system) was recorded. Follow up on post op day 3, 6, 14 and 30 days was performed. Southampton wound grading system was followed. Results: Open reduction internal fixation surgeries were performed the most (22%), followed by plating (20%), dynamic hip screw placement (14%) and fixation by nailing (10%). Arthroplasty and tendon repair were done in 8% patients each. Fixation using proximal femoral nail, total knee replacement and total hip replacement were done in 7%, 6% and 5% patients respectively. SSI positive was seen among 14% and SSI negative in 86%. Conclusion: The incidence of SSI in orthopaedics and trauma patients was comparable with the reported incidence in the literature.

Posterior malleolus fractures and their role in prognosis of malleolar fractures 后外踝骨折及其在外踝骨折预后中的作用

Introduction: The treatment of ankle fractures with involvement of the posterior tibial margin remains a subject of debate. We have evaluated the clinical, radiological, and functional outcome of such fractures in 25 patients, all of which were managed surgically over an average follow- up period of 6 months. The treatment options available for malleolar fractures, to attain a proper anatomical alignment and stability of ankle joint, can lead to rewarding outcome for the patient. Materials and Methods : 25 patients with posterior malleolus fractures were managed surgically between June 2019 to November2021 were included in this series. The mean age, gender ratio, mode of trauma, fracture union time, and complications were noted. The AOFAS Hindfoot scores were used for assessing the Ankle function. Results : Of the 25 patients in the study, 21 were males and 4 were females. The mean age was 38.12 years. The mean fracture union (radiological) time was 14.04 weeks (range: 10–18 weeks). However Ankle function was excellent in 17 cases (68%) on the AOFAS hindfoot score with a average AOFAS score of 94.8. Also platting gives a better result than using cannulated cancellous screws. Better syndesmotic stability is observed after surgically treating posterior malleolus fractures. Conclusion : Fixation of the posterior malleolus to reduce persistent fragment displacement, regardless of size, as well as to restore syndesmotic stability, may lead to improved outcomes. We conclude that posterior malleolar fractures encountered in clinical practice need thorough assessment and meticulous surgical intervention. We achieved stable fixation and performed early mobilization of the ankle joint, which limits the complications of mainly ankle stiffness and have achieved excellent clinical and functional outcomes of surgically fixing the posterior malleolus fractures.

Conclusion : Fixation of the posterior malleolus to reduce persistent fragment displacement, regardless of size, as well as to restore syndesmotic stability, may lead to improved outcomes. We conclude that posterior malleolar fractures encountered in clinical practice need thorough assessment and meticulous surgical intervention. Functional outcome following internal fixation of rare type IV capitellum fractures using varied fixation methods: A case series 采用不同固定方法内固定罕见IV型小头骨折后的功能结果:一个病例系列

Background: Capitellum fractures are very rare injuries, accounting for less than 1% of elbow fractures. These fractures are difficult to interpret on a plain radiograph and usually require a computed tomography scan to understand the complexity of the fracture. Treatment of such fractures vary from conservative treatment in the form of closed reduction and immobilization, excision of the fragment to open reduction and internal fixation with K wires, partially threaded 4mm cancellous screws or Herbert screws. In this paper we present the outcome of four cases of Type IV capitellum fractures that were managed operatively by different methods. Materials & Methods: Four patients were treated in our hospital from 2018 to 2019, for Type IV capitellum fractures. The mechanism of injury was a fall on the out stretched hand in three patients while the fourth patient was involved in a motor cycle accident. The fractures were equally distributed on either elbows, two on each side. The three males patients were aged between 18 to 30 years while the lone female was 40 years of age. All patients were operated in the supine position under general anaesthesia, using a tourniquet and with fluoroscopic guidance. Surgery was performed through an extended lateral Kocher’s approach. Definitive fixation in two patients was by using 2.7mm Herbert screws alone while in one with a lateral condyle humerus fracture, fixation was supplemented using plate and screws. K-wire fixation was used for one patient. Results: 2 patient attained a score of excellent in the Mayo Elbow Performance Score evaluation while 1 patient each had a good and a fair score. There was no evidence of arthritis, avascular necrosis or heterotopic ossification till 1 year of follow-up. Conclusion: Flawless reduction and fixation of the fragment with appropriate postoperative rehabilitation protocol are crucial to achieving optimal results.

Introduction: Osteoarthritis of the Knee is a Chronic debilitating disease excessive pressure leads to breakdown of the cartilage matrix, architectural changes in the subchondral bone, further altering the joint geometry. Aims and Objective: The effect of TBW with 2 cortical screws in cases of uni-compartmental O.A. Knee joint 32 cases were studied. Result: Lateral wedge osteotomy done 1.5 cm distal to joint margin to avoid fracture of tibial plateau intraoperatively. Height of wedge is taken dependent on varus angle calculated from x-rays, for each degree 1 mm height of wedge is taken. 2 cortical screws fixed distal to osteotomy site. Conclusion: It is a cost effective technique with use of minimum hardware and early post operative mobilization in patients who cannot afford Knee Arthroplasty in a rural set up.
