The Key Methods to Increase the Success Rate of Endoscopic Repair of Cerebrospinal Fluid Rhinorrhea 提高内镜下脑脊液鼻漏修补成功率的关键方法

Objectives: To evaluate the success rate of endoscopic repair of CSF leak and factors with potential effect on surgical outcome. Methods: A case series study based on review of medical records and follow up of patients admitted at three hospitals during a 5 year period. Results: Of 43 operated patients 38 (88.4%) had successful result after first surgical attempt. Defect size, number of graft layers, graft placement technique-underlay vs. overlay-lumbar drain placement or serial lumbar punctures did not show a statistically significant association with surgical outcome. Definite determination of defect site before or during operation had a relationship with surgical success with a P value of 0.06 there was a significant correlation between the number of layers on the defect site and immediate postoperative improvement. Conclusions: Endoscopic repair of CSF leak has been proven as a successful method with reported success rate of above 80 percent. Careful attempt to find the exact site of CSF leak is recommended. We do not suggest the routine use of lumbar drains, serial post op lumbar punctures and intrathecal fluorescine due to their potential complications and no evidence for their efficacy.

Meshoma, a Rare Complication of Abdomen and Hernia Repair-A Case Report 腹疝修补术中一种罕见的并发症——Meshoma 1例报告

Introduction: In this study, we report a case of meshoma with ventral hernia recurrence symptoms after the use of mesh. Case Presentation: A 52-year-old woman who had two ventral hernia surgery presented to our department. In the second surgery, a mesh is used to repair the position. After 6 months of the second surgery, she was refered to us with symptoms of acute cholecystitis. In pathologic examination, sections of mesh with interaction of a foreign body around it, including chronic inflammatory infiltration, bud cells of foreign body together with surrounding hyalinized irregular fibrotic fibers amidst the adipose tissue in the abdominal wall could be observed. Conclusions: Although using mesh is a suitable tool to repair abdominal wall defects, mesh shrinkage and meshoma could be considered as its rare complications while it is possible to regard it as the differential diagnosis in hernia repair with the mesh in the cases of abdominal wall masses.

The Effect of Roux-en-y Gastric Bypass Surgery on Fitness Parameters in Women with Morbid Obesity Roux-en-y胃旁路手术对病态肥胖女性健康参数的影响

Background: Obesity is widely considered as an important risk factor for impaired physical function and disability. Even mild to moderate weight loss has been shown to improve physical function. This study aimed to evaluate the effect of Roux-en-y gastric bypass (RYGB) surgery on some fitness parameters in women with morbid obesity. Methods: Seventytwowomenwithmorbidobesitybodywereincludedinthisstudy. TheywerefollowedforsixmonthsafterRYGB surgery. Somefitnessparametersincludingbodycomposition(usingbioelectricalimpedance),muscularstrength(legpressinlower limb and hand power grip in upper limb) and six minute walk test(6MWT) were evaluated before surgery and that at intervals of 1, 3 and 6 months after surgery. One way repeated measure ANOVA was applied to determine the data changes during the follow up period. Results: BMI and body weight of all samples declined constantly during 6 months follow up after surgery. Also, by the end of the study, the body fat percent decreased from 48.2 (3) to 36.4 (4) (P < 0.05). The distance which patients paced in 6MWT increased 22% during6monthsfollowup(P< 0.0001). Althoughhandgripandlegpressof theparticipantsdecreasedfollowingthegastricbypass surgery, the power grip/weight ratio and leg press /weight ratio improved significantly through the study (P < 0.0001). Conclusions: RYGB surgery may positively affect fitness parameters in morbid obese women. The endurance capacity, body composition and even muscle strength seem to be enhanced after gastric bypass surgery.

Observational Studies in Bariatric Surgery 减肥手术的观察性研究

: In medical literature, studies are divided into two categories; experimental and observational settings. Experimental studies, entitled randomized controlled trials could test the relationship between exposure and outcome experimentally via control group and random allocation. Observational settings include either analytical or descriptive studies. Descriptive studies consist of case reports and case series that are helpful in present the experience of a case or a series of cases with similar diagnoses in detail which results in hypothesis generation. Cross-sectional studies, as analytical designs, are not capable to survey the temporality of exposure and outcome as simultaneously exposure and outcome status are measured. In case-control studies, subjects follow back from outcome to exposure. The rare diseases are recommended to study using case-control setting to save expenses and time. Both exposure measurement and patient selection is before disease detection in cohort studies. Therefore, they are inefficient for rare diseases or diseases with long latency. Cohort studies are time consuming with high cost and loss to follow-up. This paper elaborately reviews the features, advantages, and disadvantages of different types of observational and experimental studies.

Endoscopic Removal of a Rare Case of Extranasopharyngeal Angiofibroma: A Case Report and Review Article 内镜下切除咽外血管纤维瘤1例:1例报告及综述文章

Introduction: Angiofibroma is a benign, un-encapsulated, and highly vascular tumor which typically arises from the pterygopalatine fossa in the young adolescent males. It predominantly extends into the nasopharynx and rarely develops extranasopharyngeally. However, the most common origins of extranasopharyngeal angiofibromas are the maxillary sinus, ethmoid sinus and nasal cavity. Case Presentation: Here, we report a 40-year-old male who presented with headache and epistaxis and was diagnosed as an extranasopharyngeal angiofibroma originated from the sphenoid sinus. The tumor was completely resected endoscopically. The 24-month follow-up showed no recurrence. Conclusions: Angiofibromas should be considered as a possible diagnosis in any unilateral vascular mass of the nasal cavity, even in the older patients without typical imaging patterns.
