AbstractObjective. To regulate the doctor-patient communication record, reduce defects and errors in doctor-patient communication and reduce the occurrence of medical disputes, so as to guarantee the quality of medical treatment. Method. Randomly selecting 585 terminal medical records of January 2013 in our hospital, performing quality control checks on the doctor-patient communication records in the medical records, recording the effects of quality control in Excel and performing statistical analysis. Results. AbstractObjective. To standardize the filling of the first page of medical records and improve the information quality of the Hospital Quality Monitoring System (HQMS) network report. Methods. According to the filling requirements and selection principles for the main diagnosis of diseases as specified in the tenth edition of International Classification of Diseases (ICD-10), the quality of the first page is analyzed in accordance with the standards in the Filling Criterion of Medical Records in Guangxi and the data reporting requirements for the first page of medical records in the HQMS. AbstractObjective. To improve the archiving rate of discharge medical records. Methods. By applying the plan-do-check-action (PDCA) method to carry out statistical analysis on the archiving status of medical records in seven days from January to September 2013, so as to identify problems, draw up a plan, and then evaluate the rectification results. Results. The archiving rate of medical records in seven days has increased from 66% in January to 99.8% in September; the archiving rate in 3 days has also increased from 21.4% in January to 79.3% in September. Conclusion. By using the PDCA management method to monitor the medical record archiving, the understanding of its importance in hospital departments at all grades in the hospital are improved, the degree of standardization is further strengthened, the archiving rate is significantly improved, and meanwhile, the working efficiency and the service quality are improved.

AbstractObjective. To understand the current situation of misuse of unspecified codes for surgeries and operations of ICD-9-CM-3, analyze the causes, and find solutions. Methods. All the unspecified codes for surgeries and operations are summarized in line with the ICD-9-CM-3 (2011 version) and compared with the Peking Union Medical College Hospital (PUMCH)'s existing dictionary database of coding. All codes mentioned above are searched one by one in the PUMCH's management information system of medical record home pages in 2012, the number of error codes in the search is counted and the rate of error coding is calculated. Results. Referring to the latest version of the ICD-9-CM-3, a total of 162 unspecified surgical and operational codes are screened out, in which 58 out of 162 codes exist in the dictionary database of our hospital, and 60.3% (35/58) of the codes are in error after individual retrieval. AbstractObjective. To discuss the characteristics of the disease spectrum of hospitalized children. Method. Performing sequence and constituent ratio analyses of the disease spectrum, for items such as the principal diagnosis, sex and age of the hospitalized children, using data from hospitals which are secondary or higher, in Guangdong province in 2010. Results. The top five systemic diseases affecting the health of children were, respectively, respiratory diseases (38.0%), of which some cases (15.0%) originated in the perinatal period; certain infectious diseases and parasitic diseases (11.1%), digestive system diseases (11.1%), injury, poisoning and external factors caused some of the other consequences (6.7%); the top five diseases account for 81.9% of the diseases for which children are hospitalized. To analyze the constitution of systemic diseases and the causes of death in 154 cases of patients who died after operation in a hospital, and provide evidence for decreasing the post-operative death rate of patients. AbstractAccording to the Evaluation Criteria for the Tertiary General Hospitals (2011 Version) announced by the National Health and Planning Commission of P.R. China, a certain hospital took key surgeries as examples and conducted training on normalized filling of surgical information in the homepages of medical records according to the surgical classification principles, aimed at the professional characteristics of clinicians, including the awareness of key surgeries, the basic knowledge of surgical classification, the selection of main surgery, the key classification points for special surgery, as well as the work of communication and code expansion. In this way, the ICD-9-CM3 knowledge was popularized among the clinicians, the development of evaluation work was promoted, and the accuracy rate of disease classification coding was improved.

AbstractObjective. To investigate the situation and risk factors of outpatients with stroke at a large general hospital. Method. Outpatients aged 35 years or more between March 2012 and December 2012, are randomly chosen to receive an epidemiology questionnaire survey. Result. To investigate factors influencing the medical expenses of patients with rheumatoid arthritis and provide policy reference for the implementation of single disease payment standard for relevant departments. Method. To conduct a study on the medical expense data of 29,431 patients with rheumatoid arthritis, from 2007 to 2011, in the health insurance database of Guangzhou and use the nonparametric test, stepwise regression analysis method, to conduct statistical analysis. AbstractThe homepages of medical records are the core of the entire medical record information.
