Open Notes and Closed-Loop Communication: A Case of Factitious Disorder in the Age of Transparent Medical Records 开放式笔记与闭环沟通:透明病历时代的人为障碍案例

A 31-year-old woman with a nursing background and loin pain-hematuria syndrome undergoes a renal autotransplant. During her recovery and prolonged hospitalization, her care team becomes increasingly concerned about a factitious disorder. With the roll-out of the 21st Century Cures Act Final Rule, which involves direct sharing of medical records with patients in real time, multiple inpatient care teams leave their clinical suspicion out of their progress notes for fear of damaging the therapeutic relationship. This case provides an opportunity to consider implications of the Final Rule policy of “open notes” for patients with sensitive psychiatric diagnoses, highlighting the need for effective communication and collaboration between inpatient teams and realistic approaches to the integration of documentation in patient care. Loin pain-hematuria syndrome is a rare condition characterized by chronic bilateral or unilateral flank pain and recurrent hematuria, hypothesized to have a psychogenic component [4]. While literature exists on the effects of the Open Notes Rule in the outpatient setting, little attention has been given to the changing dynamic in the inpatient care setting or to the effect on patient care when sensitive psychiatric information is relevant [5-7]. We will highlight the effects of Open Notes on team communication, treatment planning, and clinical outcome in this patient with factitious disorder, as well as potential strategies to address these unique dynamics.

Autism Spectrum Disorder (ASD) is a neurodevelopmental condition characterized by social skill and communication disability, accompanied by repetitive behavior that interferes with the individual’s verbal and non-verbal communication. An up-growing number of researches have put to evidence that ASD physiopathology is related with metabolic and glutamatergic transmission in the vertebrate brain alterations, which are directly affected by oxidative stress levels. The purpose of the present work was to perform a bibliographic survey about the potential application of N-acetylcysteine (NAC) as an alternative treatment for the behavioral disorders present in autism. As results, we observed that NAC emerges as a potentially prospective treatment in autism as a consequence of its capacity to decrease patient’s irritability when compared to other conventional treatments.

The recurrent nature of Bipolar Disorder (BD) is the main cause of disability associated with the illness. Despite the proliferation of drugs approved for the maintenance phase of BD, the relapse rate is still high. The combination of drugs, especially the potentiation of mood-stabilizers with second generation antipsychotics, may reduce the risk of relapse and rehospitalization. However, studies on the efficacy of specific combinations are scarce.

In the last few years, several studies have suggested the presence of high rates of psychopathology in the trans population and those with gender variability. The purpose of this paper has been to investigate the prevalence of psychiatric disorders in gender dysphoric and gender non-conforming people, as well as their relationship to the stigma and discrimination that they often suffer. For this purpose, this review includes 37 studies published in the last five years, which address the prevalence of various psychiatric pathologies while also addressing possible factors related to them. Among all the psychiatric disorders considered, the most frequent are depression, suicidal ideation, and anxiety.

A Scoping Review on Biological Factors Associated with Interpersonal Violence in Adults with Severe Mental Illness 成人严重精神疾病患者人际暴力相关生物学因素的研究综述

Introduction: Knowledge about the biological basis of violence in people with severe mental illness is needed to formulate strategies in the management of violence. The aim of this study was to identify and describe the biological factors associated with interpersonal violence in adults with severe mental illness. Methods: This scoping review was written with reference to the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist. The literature search was conducted in MEDLINE (via PubMed), PsycINFO, SciELO, ProQuest and Epistemonikos until October 2020. Primary and Secondary studies in English or Spanish were included without restrictions as to date of publication. The information charted during the process included: authors; publication year; country; type of research; study population; biological factors; outcomes and key findings. Results: Twenty-eight articles were included after the selection process. The most widely-studied population was people with schizophrenia. The biological factors studied in the articles are categorized into six groups: Serotonin function, genes, hormones, neuroactive steroids, brain function and anatomy, and immune factors. Several studies showed a relationship between different biological factors and interpersonal violence in people with severe mental illness. Conclusion: This scoping review describes current research, and provides evidence on a biological basis. Nevertheless, further studies are needed to carry out research into biological factors associated with interpersonal violence in people with severe mental illness.

The increasing use of the Electronic medical record (EMR) has been promoted to enhance communication between health care professionals and medical settings. A common criticism, however, has been the EMR's effect upon the doctor-patient relationship and patient satisfaction. Current literature on such effects of the EMR has primarily involved the outpatient setting. This study instead surveys patient attitudes toward the use of the EMR in an acute, inpatient psychiatric setting.

Objectives: Excessive smartphone use, sometimes also referred to as “smartphone addiction”, has increasingly attracted neuroscientific interest due to its similarities with other behavioral addictions, particularly internet gaming disorder. Little is known so far about the neural mechanisms underlying smartphone addiction. Here, we explored interrelationships between brain structure and function to specify neurobiological correlates of smartphone addiction on a neural system level. Methods: Gray matter volume and intrinsic neural activity was investigated in individuals with smartphone addiction (n = 20) compared to controls (n = 24), using multimodal magnetic resonance imaging and multivariate data fusion techniques, i.e., parallel Independent Component Analysis. Results: The joint analysis of both data modalities explored shared information between gray matter volume and intrinsic neural activity which were not revealed by previous modality-specific approaches. Two amplitude of low frequency fluctuations-based independent neural systems significantly differed between individuals with smartphone addiction and controls. A medial/dorsolateral prefrontal system exhibited lower functional network strength lower in smartphone addiction vs. controls, whereas the opposite pattern was detected in a parietal cortical/cerebellar system. Neural network strength was significantly related to duration of smartphone use and sleep difficulties. Conclusions: We show modality-specific associations of the brain’s resting-state activity with distinct and shared smartphone addiction symptom dimensions. In particular, the data suggest contributions of aberrant prefrontal and parietal neural network strength as a possible signature of deficient executive control in smartphone addiction.
