Treatment of comminuted fractures of the distal radius bone by the traction-suspension technique is both simple and innocuous. The authors describe this old and satisfactory technique, slightly modified: a transmetacarpal pin is introduced and suspended to a giving set stand, and a counter-traction of 3 to 4 kg is exerted by means of a transulnar pin. After remodelling of the bone under general anaesthesia and radiographic control, the pins are sunk in plaster to be removed 6 to 8 weeks later. Excellent, often unexpected clinical and radiological results can be obtained, and complex traumatic problems can be solved by this technique which can be performed by non-orthopedic surgeons, even with little experience.

When primary and not part of a systemic autoimmune disease, Sjögren's syndrome has peculiar clinical, serological and genetic features. It exceptionally becomes malignant but evolves to a stage of pseudo-lymphoma. Numerous studies suggest hyperstimulation of B-lymphocytes that would result from destruction of suppressive T-lymphocytes, this destruction in turn being probably due to failure of immune complex clearing by the reticulo-endothelial system.

Lysozyme (LZM), immunoglobulins and beta 2-microglobulin concentrations were measured in the saliva of 79 patients with primary (n = 29) or secondary (n = 50) Sjögren's syndrome and in a control population. beta 2-microglobulin and immunoglobulins A, G and M concentrations were higher, and LZM concentrations lower than in controls. A significant correlation was established between LZM and immunoglobulins, but no correlation was found between immunoglobulins and beta 2-microglobulin, nor between the latter and LZM. These results indicate that salivary LZM concentration may be of value in the diagnosis of Sjögren's syndrome.

In addition to motor and sensory nerve conduction, monosynaptic triceps surae reflex, amplitude of median nerve voltage and response of extensor digitorum brevis and soleus muscles were investigated in 29 diabetic patients. These parameters were altered earlier than motor and sensory nerve conduction, which considerably increased the incidence of subclinical peripheral neuropathy (90% of the patients). It was also found that sensory fibers were affected earlier than motor fibers. The electrically detected neuropathy correlated with duration of diabetes and control of glycaemia.
