Lasers are one of the tooth hypersensitivity treatments. This study aimed to determine the effect of irradiation of Nd:YAG 1064nm and 980nm Diode lasers, used for hypersensitivity treatment, on the shear bond strength (SBS) of metal orthodontic brackets to enamel. Ethylenediaminetetraacetic acid (EDTA) was used to simulate sensitivity in 70 extracted human premolars. The teeth were radiated with 1w Nd:YAG, 1.5w Nd:YAG, 1w Diode, or 1.5w Diode. All samples were incubated at 37° for 24 hours, after bonding the metal brackets. SBS values and adhesive remnant index (ARI) for each tooth was recorded. One-way analysis of variance (ANOVA) and Kruskal-Wallis test were used to compare the mean SBS and the distribution of ARI scores between the study groups, respectively. The SBS mean from the highest to the lowest were in 1w Diode (25.71Mpa), 1w Nd:YAG (24.66Mpa), 1.5w Diode (23.08Mpa), control (21.68Mpa) and 1.5w Nd:YAG (21.53Mpa) groups. No statistically significant difference existed between different groups, in terms of SBS (p=0.211) and ARI distribution (p=0.066). The application of Nd:YAG and 980nm Diode lasers to treat tooth hypersensitivity did not change the SBS of metal orthodontic brackets to the enamel and thus, are harmless to use for orthodontic patients.

Adverse effects associated with ablative lasers when treating common dermatologic conditions: a systematic review of 946 patients 治疗常见皮肤病时与烧蚀激光相关的不良反应:对 946 名患者的系统性研究

CO2 and Er:YAG are the two most common types of ablative lasers in use today. Ablative lasers are known for their superior efficacy in penetrating deeper through the skin layers when compared to non-ablative lasers. This review compares the outcomes and adverse events associated with CO2 and Er: YAG lasers, in conjunction with other therapeutic modalities, in the treatment of common skin conditions. A PRISMA-compliant systematic review of PubMed and Embase databases was conducted from January 2012 to December 2022. Study types that were eligible for this review included clinical trials, randomized controlled trials, case reports, case series, cohort studies, and meta-analyses. The study protocol was registered (PROSPERO 2022: CRD42022348569). The search strategy produced 27 studies comprising 946 patients who were treated for acne scars, postacne hyperpigmentation, solar lentigines, facial rejuvenation, refractory vitiligo, and epidermal verrucous nevi. Erythema, pain, and hyperpigmentation were the most commonly reported adverse effects. A higher rate of adverse events was observed in CO2 laser therapies (n=610, 64.5%) when compared to Er: YAG and other laser cohorts (n=40, 4.2%). Both CO2 and Er: YAG lasers demonstrate similar efficacy in the treatment of benign cutaneous conditions. Further research is needed for parallel comparison of CO2 and Er: YAG lasers.

Use of laser for uncovering a mandibular retained tooth: case report 使用激光揭开下颌滞留牙:病例报告

This clinical study aimed to describe a case of lower impacted canine to demonstrate the advantages of diode laser for crown exposure during orthodontic treatment. The patient received a surgical intervention by an infra-red diode laser to uncover a retained lower canine. The intervention did not need anesthetic injection and suture apposition, the patient did not refer pain, and the healing process was completed after a week. The diode laser was safe, fast, and easy to use, and the avoidance of bleeding was reached due to its excellent absorption by hemoglobin: this is particularly important considering that, for obtaining a strong adhesion, a bracket must be bonded in a dry enamel surface. The use of an infrared diode laser is a proper approach in the surgery of retained teeth during orthodontic treatment.

Effective optical zone after corneal lenticule extraction with the CLEAR femtosecond laser application 应用 CLEAR 飞秒激光进行角膜扁平膜摘除术后的有效光学区域

Laser vision correction of myopia induces an effective optical zone (EOZ) smaller than the programmed optical zone (POZ) by 16 to 26%. We evaluated the EOZ after corneal lenticule extraction for myopia with astigmatism ≤1 diopter (D) with a new femtosecond laser application (CLEAR), compared to POZ in a retrospective, consecutive, comparative case series study. Forty eyes of 40 patients underwent lenticule extraction with the Ziemer CLEAR® application; the control group was composed of 40 eyes of 40 patients receiving myopic femtosecond laser in situ keratomileusis (LASIK); EOZ was calculated on difference tangential maps at 6 months. For lenticule extraction, mean preoperative spherical equivalent (SE) was -6.03±2.48 D; mean POZ was 6.43±0.27 mm; EOZ 5.55±0.45 mm; mean difference between POZ and EOZ was 0.88 ± 0.28 mm (p=0.00); the mean reduction of EOZ compared to POZ was 13.60%±4.75; a positive correlation between preoperative SE and percent reduction of EOZ was found (r=0.63). For LASIK, mean preoperative SE was -5.89±2.14 D; mean POZ was 6.57±0.34 mm; EOZ 5.16±0.53 mm; the mean difference between POZ and EOZ was 1.41±0.35 mm (p=0.00); the mean reduction of EOZ compared to POZ was 21.46%±5.20. The mean difference between EOZ of the 2 procedures was 0.39 mm (p=0.0008). The mean difference between the reduction in optical zone (POZ-EOZ) of the 2 procedures was -0.53 (p=0.00). In conclusion, in myopia with low astigmatism, the CLEAR application for lenticule extraction provided a limited reduction in EOZ, compared with existing platforms. A positive correlation exists between corrected SE and reduction of the EOZ.

Monitoring the effectiveness of nanosecond laser for the treatment of port-wine stain patients using non-invasive imaging systems 利用无创成像系统监测纳秒激光治疗葡萄酒港污渍患者的效果

A variety of lasers have been used to treat port-wine stain (PWS) birthmarks, however, all of them carry the risk of adverse effects. Our objective was to assess the effectiveness and safety of nanosecond (ns) laser treatment for PWS patients using non-invasive images. This study included 20 PWS patients, whose ages ranged from one to sixteen years. A nanosecond laser (λ=595 nm; τp=2 ns), with a spot size of 5 mm, and parameters of 0.2-0.3 J/cm2, 5 Hz, was used in three passes. The infrared thermal imager aided in determining the optimal laser dosage and preventing side effects. The primary efficiency was evaluated using a DermaSpectrometer, and patients were followed up after one week, four weeks, three months, and twelve months. The infrared thermal images of skin surface temperature assessment in the first ten seconds were recorded after the maximal energy density 0.3 J/cm2 treatment. The thermal wave equation and Penne’s bioheat transfer equation ranged from 32.4±0.1 to 33.8±0.2°C, 32.6±0.1 to 34.7±0.3°C (baseline skin surface temperature - 32.4±0.3°C) respectively. Transient hyperpigmentation appeared in 5% (n=1) of the 20 patients, but it resolved spontaneously within three months. There was no evidence of permanent hypopigmentation or scarring. A nanosecond laser (λ=595 nm; τp=2 ns), with a spot size of 5 mm, and parameters of 0.2-0.3 J/cm2, 5 Hz, was used in three passes. The infrared thermal imager aided in determining the optimal laser dosage and preventing side effects. The primary efficiency was evaluated using a DermaSpectrometer, and patients were followed up after one week, four weeks, three months, and twelve months. The infrared thermal images of skin surface temperature assessment in the first ten seconds were recorded after the maximal energy density 0.3 J/cm2 treatment. The thermal wave equation and Penne’s bioheat transfer equation ranged from 32.4±0.1 to 33.8±0.2°C, 32.6±0.1 to 34.7±0.3°C (baseline skin surface temperature - 32.4±0.3°C) respectively. Transient hyperpigmentation appeared in 5% (n=1) of the 20 patients, but it resolved spontaneously within three months. There was no evidence of permanent hypopigmentation or scarring. Alopecia areata (AA) were considered autoimmune non-scarring hair loss, found in 0.2% of the general population. Trichoscopy is a strong supporting diagnostic tool, especially for pediatric AA, which commonly presents in daily practice, and this case report aims to highlight its importance. A 4-year-old girl was brought to the outpatient clinic with a chief complaint of enlarging baldness for nine months. The lesion was initially coin-sized and gradually enlarged without itchiness. Supporting diagnostics revealed a reactive toxoplasma IgG with unremarkable trichoscopic findings, leading to the diagnosis of toxoplasma-related AA. Follow-up examination of trichoscopy after six weeks of therapy revealed significant improvement as indicated by short velus hair, pigtail hair, and upright regrowth. Prior reports have shown that there are several findings distinguishing adult and pediatric AA. Although these findings may be absent during the initial assessment, follow-up trichoscopy is essential to show hair regrowth indicating successful therapy. Trichoscopy serves as a valuable tool in the management of AA, with pediatric patients providing specific characteristics. Moreover, findings may differ depending on disease activity.

Endolesional ablation of xanthelasma using microfiber optic laser delivery 利用超光纤激光照射治疗黄斑瘤

Xanthelasma palpebrarum is the most common type of cutaneous xanthoma and is often a cause of psychological distress and aesthetic dissatisfaction. The extent, depth, or background skin type, intolerance to downtime, or cost, may restrict the treatment options, or contribute to a recurrence rate of up to 60%. 1470 nm microfiber laser is a recent clinical innovation that allows highly targeted delivery of Laser to deeper tissues through fibers as small as 150 μm in diameter, targeting fat and/or water chromophores. We report a retrospective data series on five patients (10 eyelids) treated with intralesional microfiber laser, where other treatment methods were inappropriate, contraindicated, or declined. Single-use tip firing microfibers (150-300 μm), were introduced into lesions under tactile and visible indicator light guidance (1-2 W; 250-500 Hz, LEED 1-2 Jcm–2, 1470 nm ). Results were followed up with before/after photography. The pain was measured using a prevalidated 1-10 Likert scale. Patients were followed up by remote consultation up to one year post-treatment. Xanthelasma size was (7 mm ± 4 mm, mean ±SD). The average time to complete resolution was 12±2.4 weeks ( All patients were normolipidemic pre-treatment. Sessions needed were 1.2±0.4 (mean ±SD). Maximum discomfort on a 1-10 Likert scale was 3±1/10 (mean ±SD), at eight weeks’ follow-up. No recurrences were reported up to 1 year’s follow-up. No patients had visible scarring. Most importantly, all patients reported minimal downtime and could continue normally with activities of daily life. 1470 nm microfiber laser is a promising method for the management of palpebral xanthelasma: within this case series was safe and effective in experienced hands. Further, larger studies are in hand to assess follow-up long-term outcomes and patient satisfaction.

Management interventions for amputation stump neuromas: evidence based review and cost-benefit analysis 截肢残端神经瘤的管理干预:基于证据的回顾和成本效益分析

Amputation is a common military and civilian surgery with high morbidity. Patients without prostheses due to neuroma pain lose productivity and lifelong contributions, which is often underestimated. The surgical and non-surgical treatment of painful stump neuromas is controversial. An evidence-based assessment and cost-benefit analysis of painful stump neuroma management modalities emphasizes institutional awareness and disruptive technologies. An Oxford Centre for Evidence Based Methodology critical appraisal and structured literature review were used in the research. We found 154 records using a reproducible literature search strategy that included electronic databases and references. A full review of 27 manuscripts after exclusion criteria yielded data for analysis. Surgical, injectable, and electromagnetic spectrum methods were used. Surgical interventions had longer follow-up times than injection and radiofrequency treatments, which affected outcomes. CEBM level 4 evidence dominated primary literature, indicating low quality. No therapy was superior, but the risks varied. Injection therapies like sclerosing alcohol had limited success and side effects. Despite limited evidence, electromagnetic spectrum modalities showed potential. Including direct and indirect costs, amputation stump refitting costs millions annually. Compared to outpatient non-surgical interventions, laser therapy could save a lot of money. According to the study, surgical interventions are common but expensive and have limited functional success. Low-risk non-surgical methods like co-ablation, pulsed radiofrequency, and transcutaneous laser therapy have mixed results. The short follow-up of all non-surgical studies seems to limit them. Follow-up duration is crucial to outcome assessment. Long-term, low-risk laser-induced thermotherapy is promising for future research. This study emphasizes the need for more research and the economic benefits of disruptive technologies in treating painful stump neuromas.

One of the most common menopausal symptoms is vaginal dryness. Menopausal sex hormone deficiency causes changes in the urogenital tract, where estrogens are the primary regulators of vaginal physiological functions. Many treatments have been developed over the years, but the majority of them are ineffective or have potential side effects. Actually, laser therapy has the highest efficacy with the fewest (or none) side effects. Thermal energy acting on the vaginal wall has been shown in studies to stimulate collagen synthesis, induce neovascularization, enrich the glycogen epithelium, improve vaginal lubrication, and treat urinary incontinence. However, not all lasers are created equal. The majority of published studies describe the data from CO2 laser therapy, Erbium laser therapy, and radiofrequency. Less is known about the new 1470 nm solid state laser. This is a retrospective study in which 16 women with vaginal dryness were treated with a 1470 nm solid state vaginal laser. All of the patients were cured of vaginal dryness with no pain, bleeding, scarring, or other side effects.
