Combined cardiac surgery and extra-anatomic ascendant aorto-bifemoral bypass: Short-term results 联合心脏手术和解剖外上升主动脉-双侧旁路:短期结果

Background: Valvular or coronary artery disease associated with aortoiliac vascular disease is uncommon but very severe. We report the short-term results of 4 cases of single-stage surgery performed cardiac surgery and extra-anatomic ascendant aorta-bifemoral shunt. Methods: Retrospectively describe 4 cases of cardiac surgery combined with single-stage ascendant aorta-bifemoral artery bypass. Results: 1 case of valve replacement and 3 cases of coronary artery bypass without extracorporeal circulation accompanied by ascending aorta-bifemoral artery bypasses through the anterior peritoneal tunnel. Dacron Y-grafts with size 14/7 mm are used. Before surgery, all 4 were in Leriche’s classification stage 4 and TASC II grades D. Contrast angiography showed complete occlusion of the aorta-pelvis (just below the renal artery). There were no deaths or complications in the postoperative period. Follow-up up to 6 months and CT scan showed good patency of bypass, all the patient had no longer of pain in lower extremities and walking normally. Conclusion: Extra-anatomic ascendant aorta-bifemoral artery bypass is an effective, easy-to-implement, and safe solution in cases requiring cardiac surgery at the same time as an acute iliac aortic disease.

In-vitro comparison of TruNatomy and XP-endo Shaper on amount of dentin removal: A micro-computed tomography study 口腔切开术和XP-endo整形术在牙本质去除量上的体外比较:一项显微计算机断层研究

Objective: To use micro-computed tomography to assess the effectiveness of the TruNatomy versus the XP-endo Shaper for removing dentin from single-rooted permanent teeth with straight root canals. Materials and Methods: Ninety-eight extracted human single-root teeth were collected and divided into two equal groups: TruNatomy and XP-endo Shaper (XPS). Root canal instrumentation was done for each system. Pre-and post-instrumentation images were scanned by using Skyscan 1172 micro-CT scanner at a resolution of 13.73 µm to measure dentin removal, differences in dentin thickness, and percent change in root canal volume. The data were analyzed using SPSS. Results: The TruNatomy group and XP-endo Shaper had no significant difference regarding the amount of removed dentin. Conclusion: There was no significant difference in post-instrumentation canal volume and dentine removal between TruNatomy and XP-endo Shaper. Thus, both file systems are equally recommended for clinical use as a practice of minimally invasive endodontics.

Extrapyramidal syndrome - anaesthetist’s challenge: A case report 锥体外系综合征-麻醉师的挑战:1例报告

Extrapyramidal syndrome (EPS) is the condition commonly referred to as drug induced movement disorders, due to the side effects of drugs which the patients experience from dopamine receptor antagonists. The symptoms of EPS are debilitating, which interfere with the social functioning and communication, motor tasks and activities of daily living. This ultimately is associated with poor quality of life which may result in multiple hospital visits. This case is of a 28 years old female patient who was the known case of EPS and epilepsy and was on medications of antipsychotic drug Olanzapine, dopamine receptor agonist levodopa (100mg) plus carbidopa (25mg) and anti-epileptic drug levetiracetam who presented with an abscess in left loin region for 5 days which was insidious in onset and then gradually increased in size. It was associated with pain. This case study discusses how the patient with EPS with decreased swallowing reflexes, poor nutritional status and condition requiring surgery poses challenges to the anaesthesiologists in the peri operative period.

Coping with the COVID-19 pandemic: Anxiety, depression and stress: Results from a cross-sectional survey 应对COVID-19大流行:焦虑、抑郁和压力:来自横断面调查的结果

Introduction: The global burden of coronavirus disease (COVID-19) has affected mental health in many people, including anxiety, depression, and stress. Methods: Google online survey was posted about the COVID-19 experience on anexiety, stress and depression assessment. Results: A total of 306 responses were obtained. Around (76,8%) of the respondents were female, (73.9%) were with undergraduate education, and (29.4%) were from Makkah. Conclusion: Our study, which was conducted after the pandemic had settled down and the lockdown had been lifted, showed that anxiety, depression, and stress were at mild to moderate levels. From our results we advice to apply psychological protocols and help that can enhance any mental health defect related the COVID-19 pandemic.

Peutz-Jeghers Syndrome, A Rare Polyposis Syndrome: A Case Report Peutz-Jeghers综合征,一种罕见的息肉病综合征:1例报告

Peutz-Jeghers Syndrome (PJS) is an uncommon familial disorder that leads to the development of gastrointestinal and extragastrointestinal hamartomatous polyps, mucocutaneous pigmentation with an increased risk of malignancy. This polyposis syndrome is associated with variable clinical course with significant mortality & morbidity and is known to cause complications like intussusception, obstruction, anemia, and bleeding. Here, we report a case of a 21 years old young female suffering from PJS to increase awareness amongst clinicians regarding this rare syndrome. She presented to us with complaints of pain in the epigastrium and left hypochondrium, constipation and malena. She had hyperpigmented mucocutaneous macules of brownish-black colour on the buccal mucosa and lips and also had numerous sessile-pedunculated polyps in stomach, duodenum, jejunum and colon. She was decided to undergo double-balloon enteroscopy and polypectomy (more than 35 polyps were removed). After an interval of 4 weeks, colonoscopic polypectomy was also done. There was a history of jejuno-jejunal intussusception for which resection and anastomosis of proximal jejunal was done in past. Hence, it is advisable for young cases having a presentation of intestinal obstruction, intussusception or malena, in presence of mucosal pigmentation and other pathognomonic signs of PJS, to screen for PJS. In case of PJS, a surgical emergency like intussusception may be present which if missed may turn into bowel ischemia which may have a grave sequel. In PJS, early intervention (polypectomy) preferably by double-balloon enteroscopy method is important to prevent morbidity and mortality caused by PJS. Follow-up of gastrointestinal polyps and regular lifelong screening for malignancy is recommended in PJS.
