[Clinical aspects of Neurofibromatosis type 1 seen in the Department of Dermatology at University Hospital Antananarivo, Madagascar]. [马达加斯加塔那那利佛大学医院皮肤科 1 型神经纤维瘤病的临床表现]。

Introduction: Neurofibromatosis 1 (NF1) is an inherited disease, in an autosomal dominant manner, with complex multi-system involvements. Prevalence varies from one country to another. However, little is known about neurofibromatosis in African countries, particularly in Madagascar.

Methodology: A descriptive retrospective study from 2014 to 2019 was conducted at the service of dermatology at University Hospital Joseph Raseta Befelatanana in Antananarivo, including all patients with neurofibromatosis according to National Institutes of Health Consensus Conference criteria for whom genealogical investigation could be made.

Results: Among 32 cases of NF1 seen during 6 years, 28 cases were included with a sex ratio M/F of 0.87. The mean age was 24 years ranging from 11 to 54 years. Seventeen patients presented sporadic forms. All patients had "café au lait" spots and cutaneous neurofibromatosis. Three cases presented plexiform neurofibromas which cause significant cosmetic and functional problems by their size and their displayed topography. Fifteen patients had Lisch nodules but no case of optic glioma was identified. Neurological symptoms such as learning difficulties, epilepsy and headache were frequent in our case series. However, access to medical imaging was very limited. Scoliosis was the most common orthopedic complication.

Conclusion: The clinical manifestations of NF1 are extremely variable. Although the possibility of systemic complications seems to be low, patients must be followed up.

[Giant cervical lipoma: Report on one case in Ouagadougou, Burkina Faso]. 巨大宫颈脂肪瘤:布基纳法索瓦加杜古1例报告。

Objective: To report on a case of rare giant anterior cervical lipoma.

Patients and methods: This was a 60-year-old male patient received in March 2020 for an anterior cervical swelling in progressive evolution since 20 years. The condition was negatively impacting the patient's quality of life with a feeling of cervical heaviness, discomfort, head movement limitations, with no sign of compression. Despite this significant discomfort, the patient first consulted several traditional practitioners with different traditional treatments without success, the mass having been taken for a goiter. It is in the face of the failure of traditherapeuts that the patient finally decided to consult in our care structure.

Results: Physical examination found a large left paramedian formation which appeared soft, mobile vis-a-vis the two plans, and sensitive; it measured 13 cm on its longer axis, and surrounding skin showed scarifications, witness to previous traditional treatments. Cervical CT eliminated goiter and made it possible to diagnose a giant cervical lipoma. A cervicotomy was performed under general anesthesia with a total one-block excision of an encapsulated mass. The evolution was favorable with a good healing. The histological examination of the mass identified a well-differentiated lipoma. There was no relapse up until 24 months later.

Conclusion: The giant anterior cervical lipoma is rare. The differential diagnosis is mainly posed with a large goiter or liposarcoma. CT or MRI make it possible to confirm the diagnosis. Its treatment is exclusively surgical and the examination of the operative piece by the pathologist confirms the diagnosis. An extended post-operative monitoring is recommended given the risks of relapse and of malignant degeneration.

A mobile care unit has been designed for infectious patients care in a humanitarian setting. Six individual compartments are grouped together in the main tent of 54 m2 under negative air pressure. The principle of walking from clean to dirty is respected. The main indications are bacterial or viral infections with airborne transmission and an epidemic potential. The operating situations of this prototype, which has not yet been tested in the field, and its constraints in an underdeveloped country are presented.

[The hazards of SARS-COV-2 for aquatic ecosystems]. [SARS-COV-2对水生生态系统的危害]。

Introduction: The current COVID-19 pandemic is due to a new emerging coronavirus SARS-CoV-2, belonging to the Coronaviridae family and to the Orthocoronavirinae subfamily. This virus was first reported in December 2019 in China. Although reported by several countries in several animal species, COVID-19 is a disease transmitted from human to human. Moreover, SARS-CoV-2 virus and its RNA were detected in body excretions besides saliva, such as urine and fecal matter discharged into sewage.

Bibliographic review: Within this framework, this article aims to synthesize the bibliographical reviews on SARS-CoV-2 in aquatic environment. It will underline the generalities on SARS-CoV-2, the possible sources of potential contaminations of SARS-CoV-2 in water environment, the viability of SARS-CoV-2 in aquatic environment, the receptive species and the impacts of SARS-CoV-2 on the aquatic ecosystems.

Conclusion: We compile key information about SARS-CoV-2 that are considered important to remember and highlight the importance of further research in this area in order to assess the hazards of SARS-CoV-2 on aquatic ecosystems.

Burkitt lymphoma in HIV-infected patients is seldom seen in the oral cavity, and they are rarely reported in the literature. The clinical manifestations may suggest other tumors or gingival infections, which delays diagnosis and management. Spinal cord involvement is exceptional and only a few cases are reported. We report a case of Burkitt lymphoma in the oral cavity and spinal cord, revealing HIV infection in a 44-year-old patient, who responded well to chemotherapy and antiretroviral treatment and is on remission.

Background: Envenomation by snakebite is a public health problem in developing countries. Neurovascular complications are uncommon. We report two cases of hemorrhagic stroke at the Sourô Sanou Teaching Hospital in Bobo-Dioulasso, which complicate an envenomation.

Clinical description: The first patient was a 60-year-old woman with no history, who had been admitted 2 hours after snakebite for gingivorrhagia and consciousness disorders. Clinical examination found coma (GCS 7/15) and tetraparesis. The second patient was a 50-year-old woman with no history, who had been admitted one week after snakebite with digestive bleeding. Clinical examination found right hemiplegia and meningeal syndrome. Both patients presented hemorrhagic stroke on brain CT-scan but hemostasis assessment was normal. They received polyvalent antivenom and symptomatic treatment. The outcome was favorable for survival but with sequelae like tetraparesis for first patient and hemiparesis for second patient.

Discussion - conclusion: The hemorrhagic strokes are rare complications of snake envenomation. Venomous toxins can directly lead to the rupture of intracranial vessels apart of abnormalities of hemostasis. However, the morbidity and mortality related to envenomation by snake bite remains high in our environment.

Neurovascular complications are uncommon. We report two cases of hemorrhagic stroke at the Sourô Sanou Teaching Hospital in Bobo-Dioulasso, which complicate an envenomation.</p><p><strong>Clinical description: </strong>The first patient was a 60-year-old woman with no history, who had been admitted 2 hours after snakebite for gingivorrhagia and consciousness disorders. Clinical examination found coma (GCS 7/15) and tetraparesis. The second patient was a 50-year-old woman with no history, who had been admitted one week after snakebite with digestive bleeding. Clinical examination found right hemiplegia and meningeal syndrome. Both patients presented hemorrhagic stroke on brain CT-scan but hemostasis assessment was normal. They received polyvalent antivenom and symptomatic treatment. The outcome was favorable for survival but with sequelae like tetraparesis for first patient and hemiparesis for second patient.</p><p><strong>Discussion - conclusion: </strong>The hemorrhagic strokes are rare complications of snake envenomation. Venomous toxins can directly lead to the rupture of intracranial vessels apart of abnormalities of hemostasis. [New oral polio vaccine: A turning point for the global polio eradication initiative?] 新型口服脊髓灰质炎疫苗:全球根除脊髓灰质炎行动的转折点?]

Launched in 1988, the Global Polio Eradication Initiative (GPEI) aims to eradicate polioviruses, which are the etiologic agents of poliomyelitis. Coordinated by the World Health Organization, this program relies on two pillars: mass vaccination campaigns that target children and active surveillance of the virus circulation. The GPEI has led to the eradication of two out of three serotypes of wild polioviruses and to the containment of the last serotype in two countries.Two polio vaccines exist: the injectable vaccine and the oral one. Both induce an efficient protection against poliomyelitis, but only the oral vaccine is able to stop poliovirus transmission chains. Therefore, the oral vaccine is essential to contain polioviruses and, finally, to eradicate them. In some contexts where the vaccine coverage is not sufficient, the attenuated strains contained in the oral vaccine can circulate for months and recover a pathogenic phenotype through genetic drift. In order to prevent this phenomenon, a new vaccine strain has been developed through genetic engineering: it has been designed to be as immunogenic as the historical vaccine strain, but more genetically stable to prevent the loss of its attenuation determinants. After being evaluated in vitro and through clinical trials, the novel strain has been rolled out in several African countries and in Tajikistan in 2021.

[Generalized old world leishmaniasis: first Moroccan case in an immunocompetent adult?] 广泛性旧大陆利什曼病:摩洛哥首例免疫功能正常的成人病例?]

Background: Post-kala-azar dermal Leishmaniasis (PKDL) is a rare skin syndrome observed after treatment of visceral Leishmaniasis (VL) with pentavalent antimonial organic salts, never described in Morocco before. Here we report a case in an immunocompetent adult.

Case: A 36-year-old-man from Tata in southern Morocco, with a history of visceral Leishmaniasis 2 years before and treated with meglumine antimoniate and amphotericin B with good clinical course, was hospitalized in dermatology for an erythematous papulo-nodular closet of the face. Six months ago, he presented oral mucosa involvement, then 3 months later, cutaneous lesions appeared on the face. The dermatological examination revealed a papulo-nodular erythematous closet extending to the nose and both cheeks, crusty and lupoid lesions on the forehead, around the eyes and chin, associated with an ulcerative and painless lesion on the heeL. The examination of the oral mucosa revealed an ulceration of the posterior third of the tongue and a papillomatous aspect of the soft palate. The skin biopsy and smear found some amastigote forms of Leishmania bodies. ITS1 PCR was positive (genus Leishmania). The HIV serology was negative. The diagnosis of PKDL was then evoked. The patient received intra-muscular injections of meglumine antimoniate with good progress.

Conclusion: To our knowledge, this is the first case of generalised leishmaniasis suggesting PKDL reported in a Moroccan immunocompetent adult.

Fake vaccine trafficking is a recent but growing phenomenon, which represents a severe threat to public health. During the Covid-19 pandemic, Covid vaccines have been a prime target for traffickers, but all types of vaccines are falsified by profit-hungry criminals.The consequences of falsification on global health are serious: decline in vaccination coverage, loss of control of epidemics claiming yet more victims, resurgence of diseases that been under control. Fake vaccines also fuel the mistrust of populations towards science and towards the authorities.In order to tackle this scourge, a general and coordinated mobilization of all actors concerned is urgently needed: health professionals, political decision-makers, police and customs forces, judges and prosecutors, without forgetting the crucial awareness-raising of public opinion.

Background/rationale: Tuberculosis is a public health issue. Extra-pulmonary presentations are rare and sometimes hard to diagnose and manage. Here, we report a case of tuberculosis of the chest wall in a 28-year-old man in order to discuss the mechanism, diagnostic and therapeutic problems posed by this rare localization.

Description: The patient presented with wall swelling discovered after physical effort. Imaging was in favor of an abscess or infected hematoma. The excisional surgery established the tuberculous origin which was confirmed by histology. Anti-tuberculosis treatment during 9 months completed the surgical treatment and led to the patient's recovery.

Discussion/conclusion: The rarity of this localization of tuberculosis should be investigated. Etiological treatment must be instituted and regular monitoring scheduled to avoid recurrence.

Conclusion: Direct-acting antivirals are effective and characterized by good tolerance in these Malagasy hepatitis C patients.
