La pandemia ha permitido visualizar mas claramente las prioridades de la humanidad, nunca tan globalizada como ahora, para las proximas decadas. Todo lo vinculado a la salud, que quiza hace unos meses no se percibia como prioridad, estara junto a los temas ambientales en los primeros lugares. Como hemos sido testigos, el coronavirus ha desnudado realidades en todo el mundo y tambien ha permitido visualizar oportunidades. En esta serie de articulos, que ambiciosamente hemos titulado Efectos colaterales positivos, intentamos jerarquizar algunas de esas oportunidades en el ambito medico en general y en la cardiologia en particular.

Arritmias ventriculares en el posoperatorio de cirugía cardíaca 心脏手术后室性心律失常

Arrhythmias represent one of the most frequent complications during the postoperative period of cardiac surgery. They may worsen patient evolution, increase the in-hospital length of stay, modify therapeutics and increase health costs. Its proper diagnosis and management improves the long-term prognosis of patients. Atrial fibrillation is the most common arrhythmia in the postoperative period of cardiac surgery, with other supraventricular arrhythmias being less common. Ventricular arrhythmias are cause of increases in morbidity and mortality. Their main characteristics will be discussed in this review.
心律失常是心脏手术后最常见的并发症之一。它们可能使患者病情恶化,延长住院时间,改变治疗方法并增加医疗费用。正确的诊断和处理可以改善患者的长期预后。心房颤动是心脏手术术后最常见的心律失常,其他室上性心律失常较少见。室性心律失常是发病率和死亡率增加的原因。本文将讨论它们的主要特征。

Prevención y manejo del sangrado en cirugía cardíaca 心脏手术出血的预防与管理

emphasis will be placed on its impact on hemostasis. Bleeding is one of the most frequent complications and with the highest morbidity and mortality in these patients. Although it is caused by a number of factors, the surgical technique, the cardiopulmonary bypass time and the pre-existing diseases acquire special relevance. The appearance of new procoagulant drugs and systems for monitoring hemostasis, the use of management protocols, with good evidence but low adherence, have so far not been able to reduce the use of blood components. In this review we analyse the current recommendations for the management of bleeding in cardiac surgery, its prevention through the preoperative optimization of the patient, stratify their risk, maintain intraoperative hemostasis and treat postoperative bleeding complications in those patients who suffer them.

Lipoma pericárdico: tumor benigno como cauda de taponamiento cardíaco a baja presión 心包脂肪瘤:低血压下心脏阻塞尾部的良性肿瘤

Cardiac tumors are rare and cardiac lipomas represent a small fraction of them. Most of these tumors differ in clinical manifestations, diagnosis, morphology and size, making them difficult to diagnose. We report the case of an 80 years old female patient, with progressive dyspnea and decompensated heart failure, who developed pericardial effusion and subsequent cardiac tamponade. After transthoracic echocardiography and thoracic computed tomography, pericardial lipoma was evidenced as the cause of low pressure cardiac tamponade.
