Data Science Education and the Construction of Clinical Databases in the Field of Medicine and Dentistry 数据科学教育与医学与牙科临床数据库建设

ビッグデータに対応できるデータサイエンティスト 育成の必要性については,文部科学省,日本学術会議, 情報・システム研究機構からの報告書などにおいて強 く訴えられてきた。その後,AI戦略 2019の一環とし て,歯学部歯学科・口腔保健学科・歯科衛生学科等に おいても,2025年度までにすべての卒業生が数理・ データサイエンス・AIを理解し,具体的なデータに対 して利活用できる能力を持つことを目標として,取り 組みが推進されている。 日本では巨大な医療ビッグデータであるレセプト情 報が利活用されているが,貴重なデータを社会還元す る取組として,さらなる利活用と発展に大いに期待し たい。また,歯周治療目的で記録される診療データを 二次利用して行う後ろ向き研究も行われているが,各 研究者,各機関の努力のみに頼ることには限界がある。 今後は,日本歯周病学会として臨床データベースを構 築し,適切なルールの下で利活用できる体制が求めら れている。 本稿では,AI戦略 2019における歯学教育でのデー タサイエンス教育の取り組みと共に,日本歯周病学会 における臨床データベース委員会の非常にタイムリー な発足を紹介する。

A case of chronic periodontitis with drug-induced gingival hyperplasia who improved with non-surgical periodontal treatment 慢性牙周炎伴药物性牙龈增生1例,经非手术治疗后好转

: Gingival hyperplasia is one of the well-known side effects of calcium channel blockers, which are often used for the treatment of hypertension. Gingival hyperplasia may induce malalignment of the teeth and aesthetic problems. Although the mechanisms of gingival hyperplasia are not yet clearly un-derstood, inflammatory conditions of the periodontal tissue caused by dental plaques are known to be in-volved in the pathogenesis of gingival hyperplasia. In general, a change of the antihypertensive drug to one without the side effect of gingival hyperplasia by the patientʼs attending physician, and thorough in-itial treatment, including plaque control and scaling and root planing are effective for the treatment of calcium channel blockers-induced gingival hyperplasia. Periodontal surgery may be performed, if neces-sary. Herein, we report the case of a 41-year-old male patient, a hypertensive under treatment with the calcium channel blocker amlodipine besilate Ⓡ , who was diagnosed as having gingival hyperplasia and chronic periodontitis (Stage III, Grade C). He presented with marked gingival enlargement and bleeding, mobility of the anterior maxillary teeth, and disordered teeth. We attempted periodontal infection control, including instructions on oral hygiene, and professional care, including scaling and root planing and professional mechanical tooth cleaning. After re-evaluation, persistence of deep pockets with a pocket depth of more than 4 mm at multiple sites was observed, although the gingival hyperplasia had almost completely resolved. We planned a flap operation, but we could not do it, because the patient was very busy with his career and did not provide informed consent. So, we repeated scaling and root planing, and used a local drug delivery system in order to eliminate periodontopathic microorganisms. Furthermore, we motivated the patient by repeatedly explaining the importance of plaque control, professional care, as well as blood pressure control. As a result, the periodontal tissue and dentition improved with non-surgical therapy.

Expression of Cannabinoid Receptor GPR55 in Human Gingival Fibroblasts and Anti-inflammatory Effects of Cannabidiol via GPR55 大麻素受体GPR55在人牙龈成纤维细胞中的表达及大麻二酚的抗炎作用

: Cannabidiol (CBD) in cannabis has an anti-inflammatory effect, and its receptor, G protein-coupled receptor 55 (GPR55), has been suggested to be involved in the regulation of inflammation. Here, we investigated GPR55 expression in human gingival fibroblasts (HGFs) and anti-inflammatory effects of CBD via GPR55. GPR55 expression and localization were confirmed in control, LPS-stimulated (LPS), CBD-treated and LPS-stimulated (CBD + LPS), and CBD-treated (CBD) groups. We also compared interleukin (IL)-6 and IL-8 protein production in each group. GPR55 expression levels were significantly reduced in the LPS and CBD + LPS groups (p < 0.05) compared to control and CBD groups, and GPR55 protein was found diffusely in the cytoplasm of HGFs in all groups. In addition, IL-6 and IL-8 production was significantly reduced in non-knockdown HGFs in the CBD + LPS group compared to the LPS group (p < 0.05). By contrast, in GPR55-knockdown HGFs, there was a significant increase in IL-6 and IL-8 production in the LPS and CBD + LPS groups compared with the control groups (p < 0.05), but there was no significant difference in IL-6 and IL-8 production in GPR 55-knockdown HGFs between the LPS and CBD + LPS groups. In this study, we confirmed the homeostatic expression of GPR55 in HGFs and its tendency to decrease in expression during inflammation. It was also shown that part of an anti-inflammatory effect by CBD may be mediated by GPR55.

Here, we investigated GPR55 expression in human gingival fibroblasts (HGFs) and anti-inflammatory effects of CBD via GPR55. GPR55 expression and localization were confirmed in control, LPS-stimulated (LPS), CBD-treated and LPS-stimulated (CBD + LPS), and CBD-treated (CBD) groups. We also compared interleukin (IL)-6 and IL-8 protein production in each group. GPR55 expression levels were significantly reduced in the LPS and CBD + LPS groups (p < 0.05) compared to control and CBD groups, and GPR55 protein was found diffusely in the cytoplasm of HGFs in all groups. In addition, IL-6 and IL-8 production was significantly reduced in non-knockdown HGFs in the CBD + LPS group compared to the LPS group (p < 0.05). By contrast, in GPR55-knockdown HGFs, there was a significant increase in IL-6 and IL-8 production in the LPS and CBD + LPS groups compared with the control groups (p < 0.05), but there was no significant difference in IL-6 and IL-8 production in GPR 55-knockdown HGFs between the LPS and CBD + LPS groups. In this study, we confirmed the homeostatic expression of GPR55 in HGFs and its tendency to decrease in expression during inflammation.
