Objectives: To identify the correlation of early cognitive impairment following intracerebral haemorrhage. Methods: A total of 30 adult patients (>15 years) with intracerebral hemorrhage were enrolled in the study. Demographic profile, clinical and radiological profile of the patients was noted. Cognitive status at discharge was assessed using Montreal Cognitive Assessment (MoCA). Results: Mean age was 63.53±12.11 years. Majority were males (56.7%). All the patients had cognitive impairment - majority (76.7%) had moderate cognitive impairment followed by severe impairment (16.7%) and mild impairment (6.7%) respectively at time of discharge. History of tobacco use showed a significant association with severe cognitive impairment. Conclusions: Mild to moderate cognitive impairment is quite frequent among intracerebral hemorrhage patients at time of discharge irrespective of the clinical, demographic and radiological profile.

Introduction: COVID-19 vaccines are essential in reducing symptoms and severity of the disease as well as preventing infection with COVID-19. Vaccination of pregnant women against COVID-19 has been recommended globally. In Zambia, there is a paucity of information on the knowledge, attitude, acceptance, and perceived risks of COVID-19 vaccines among pregnant women. Therefore, this study assessed the knowledge, attitude, acceptance, and perceived risks of COVID-19 vaccination among pregnant women attending antenatal care at Women and Newborn Hospital (WNH) of the University Teaching Hospitals (UTHs) in Lusaka, Zambia. Methods: This was a descriptive cross-sectional study conducted from August 2023 to October 2023 among 300 pregnant women attending antenatal care at Women and Newborn Hospital in Lusaka, Zambia. Data were collected using a structured questionnaire and analysed using Statistical Package for Social Science (SPSS) version 23.0. The Chi-square test was used to test the relationship between the dependent and independent variables. The statistical significance was at a 95% confidence level. Results: Of the 300 participants, 186 (62.0%) were aged between 24 and 34 years and 185 (61.5%) were aware that the COVID-19 vaccine was recommended in pregnancy. Overall, 284 (94.7%) had good knowledge, 258 (86.0%) had a positive attitude, and 186 (62.0%) were vaccinated. Additionally, 20 (17.5%) who were not vaccinated were willing to be vaccinated. Furthermore, most of the participants felt that COVID-19 vaccines were safe to use in pregnancy. The results show a lower vaccine acceptance compared to the knowledge and attitude levels. Having good knowledge and a positive attitude was associated with education status (X^2=33.011, p=0.001 for knowledge level) and (X^2=9.132, p=0.029 for attitude level). Conclusion: This study found that pregnant women attending antenatal care at WNH had good knowledge, positive attitudes, good perceptions, and high acceptance of ..........

Introduction: Supraventricular tachycardias represent a fairly common condition, often encountered in emergencies but which remains underestimated due to the rarity of seizures or their atypical manifestations. Methods: We report the results of a retrospective and descriptive single-center study of 32 cases of supraventricular tachycardia carried out in the cardiology department of the Cheikh Khalifa University Hospital over a period of 2 years: from January 2020 to January 2022.We studied in this work the epidemiological, clinical, paraclinical and therapeutic characteristics of these tachycardias. The results of our series are compared with those of the literature. Results: The age of the patients varies between 27 and 80 years with an average age of 58.1 years with a sex ratio M/F=1.6. All patients were symptomatic with the presence of palpitations in 87.5% of cases accompanied by dyspnea in 40.62% of cases, Chest pain and lipothymia were present in 15.62% and Syncope was found in 2 patients 6.25% of cases. Endocavitary electrophysiological exploration has proven to be important in defining the mechanism of these tachycardias, in this case tachycardias with one-to-one conduction; In our series, it confirmed the presence of a typical atrial flutter in 53% of cases, and objectified tachycardia by intranodal reentry in 28.12% of cases and less frequently reentry by accessory route, identified in 18.75% of cases. In this series, radiofrequency represents the energy most used in the management of these supraventricular tachycardias. Conclusion: Conventional fluoroscopy was the most widely used given the typical nature of these SVTs (84.6% of cases) while three-dimensional mapping was only used in 12.5% of patients who were admitted for recurrences. The low risk of complications and the high success rates revealed in our series make ablation a reference treatment, which has made it possible to change the prognosis of supraventricular tachycardias.

The rupture of the rotator cuff is a common condition in Orthopedics, with massive and irreparable tears posing a challenge for Orthopedic surgeons. There are multiple techniques described for its treatment, with one of the most recent being the implantation of a subacromial balloon. This technically simple technique has shown good biomechanical results in the literature but controversial clinical outcomes. Most of the existing literature is of low scientific evidence. Studies with higher evidence are starting to emerge, but there is still no clear definition of the role of subacromial balloons.

Hypokalemic nephropathy is a rare tubulointerstitial disorder due to chronic hypokalemia that can lead to serious morphological or functional renal alterations. We report a case of a 31-year-old patient from North Africa, admitted to the Department of Nephrology for exploration of proteinuria at 1.2g/24h associated with renal failure at 21mg/l of plasma creatinine in a context of chronic hypokalemia around 2.5mEq/l. The nephropathy check up didn’t show anything particular. The renal biopsy performed revealed renal tubes with vacuolation of the epithelium, with the presence of a few hyaline casts. The interstitial tissue showed minimal fibrosis and a moderate inflammatory infiltrate composed of lymphocytes and histiocytes, all easing the diagnosis of hypokalemic nephropathy. The patient was treated with potassium supplementation in combination with an angiotensin converting enzyme inhibitor (ACE inhibitor). The evolution was marked by a progressive improvement of renal function going from an glomerular filtration rate (GFR) of 28 ml/min (at admission) to 55 ml/min after 6 months, with negativation of proteinuria. Although the hypokalemic nephropathy seems to have disappeared, it must be considered in the face of any alteration of renal function in a context of chronic hypokalemia. Histology is essential to confirm the diagnosis.

Background: Coronary artery disease (CAD) is a leading cause of morbidity and mortality worldwide. Analyzing the profile of CAD patients involves exploring various demographic and clinical factors that may influence the occurrence, progression, and management of the disease. This study aimed to analyze the profile of patients with coronary artery disease, focusing on demographic and clinical characteristics. Methods: This prospective observational study was conducted in the Department of Cardiology, 250 Bed General Hospital in Meherpur, Bangladesh, from June 2021 to July 2022. A total of 87 coronary artery disease patients were enrolled in this study as the study subjects. MS Office tools were applied for data analysis. Results: The mean age of the participants was 46.83 ±14.31 years; males were 70%. Approximately one-third of them had a BMI (Kg/m2) of ≥30. The most common symptom was chest pain, present in 70% and the most common complication was mitral regurgitation, affecting 26% of individuals. The predominant risk factors were diabetes mellitus (21%) and hypertension (18%). Angiographic findings revealed normal findings in 37% of patients. Single-vessel disease (SVD) was observed in 28% of cases, while double-vessel disease (DVD) and triple-vessel disease (TVD) were found in 20% and 14% of patients, respectively. Conclusion: Middle-aged male individuals are mainly prone to coronary artery diseases (CAD). For CAD patients, chest pain is the most common symptom, diabetes mellitus and hypertension are predominant risk factors and mitral regurgitation is the most prevalent complication.

Background: Thrombocytopenia is a common biological sign, and in severe situations, bleeding can ensue. Thrombocytopenia caused mostly by platelet immunological destruction, decreased platelet synthesis in bone marrow, and a delay in megakaryocyte maturation. Dengue fever haemostasis disruption is exacerbated by thrombocytopenia and platelet dysfunction. Objective: To determine the relative prevalence of various diseases in fever with thrombocytopenia. Materials and Methods: This observational, cross-sectional study was carried out by the Department of Medicine at the 100-bed district hospital in Narsingdi between July 2021 and June 2022. Total 100 adult patients (over the age of 18) who had thrombocytopenia and presented with fever were included in this study. We excluded patients who had a fever but no thrombocytopenia. Additionally, patients with thrombocytopenia but no fever were excluded from the research. Results: Malaria was the most prevalent cause of fever (44.0%), followed by dengue (27.0%), viral fever (18.0%), enteric fever (5.0%), dengue + malaria (3.0%), septicemia (2.0%), and hematologic malignancy (1.0%). Conclusion: Dengue fever and malaria are common causes of fever with thrombocytopenia because of seasonal and regional variations.

Thrombocytopenia caused mostly by platelet immunological destruction, decreased platelet synthesis in bone marrow, and a delay in megakaryocyte maturation. Dengue fever haemostasis disruption is exacerbated by thrombocytopenia and platelet dysfunction. Objective: To determine the relative prevalence of various diseases in fever with thrombocytopenia. Materials and Methods: This observational, cross-sectional study was carried out by the Department of Medicine at the 100-bed district hospital in Narsingdi between July 2021 and June 2022. Total 100 adult patients (over the age of 18) who had thrombocytopenia and presented with fever were included in this study. We excluded patients who had a fever but no thrombocytopenia. Additionally, patients with thrombocytopenia but no fever were excluded from the research. Results: Malaria was the most prevalent cause of fever (44.0%), followed by dengue (27.0%), viral fever (18.0%), enteric fever (5.0%), dengue + malaria (3.0%), septicemia (2.0%), and hematologic malignancy (1.0%). Conclusion: Dengue fever and malaria are common causes of fever with thrombocytopenia because of seasonal and regional variations.","PeriodicalId":193141,"journal":{"name":"SAS Journal of Medicine","volume":"12 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-11-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139213481","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
