Topicality: Beta-glucans are a group of biologically active polysaccharides of natural origin with a proven pleiotropic immunomodulatory effect. In vitro experiments as well as animal and human clinical studies proved the importance of beta-glucans in the treatment and prevention of allergic diseases. This opens up new prospects for the use of this widespread and popular group of natural substances. Aim: to investigate whether the addition of "Bewell®" can alleviate the symptoms of seasonal AR in children sensitized to ragweed during flowering and improve their quality of life. Materials and methods: The study involved 30 patients (12 girls, 18 boys) aged from 6 to 12 years with a verified diagnosis of seasonal allergic rhinitis. Clinical manifestations of rhinoconjunctivitis were evaluated according to T5SS scale, quality of life (QOL) – according to Paediatric Rhinoconjunctivitis Quality of Life Questionnaire (PRQLQ). Results: The study found that administration of "Bewell®" in the complex therapy of seasonal allergic rhinitis significantly reduced rhinoconjunctival manifestations (p<0.001). The surveyed patients reduced consumption of antihistamines. After a treatment course, a decrease in the PRQLQ score was detected for such parameters as: irritability, need to wake up at night due to nasal congestion, need to take medications to relieve AR symptoms such as antihistamines or inhaled corticosteroids, which indicated an increase in the quality of life of patients after the therapy. The use of a complex of parameters to evaluate the therapy effectiveness showed that the use of beta-1,3/1,6-glucan ("Bewell®") is promising for reducing AR symptoms in children, leading to their rapid regression, improving the quality of life and effective relief of AR symptoms. Administration of "Bewell®" reduces the need for concomitant therapy and helps to reduce the drug load.

Materials and methods: The results of surgical treatment in 30 patients with oral and oropharyngeal cancer were studied. Postoperative surgical complications were analyzed according to the Clavien-Dindo classification, the causes of postoperative complications after reconstructive operations. Results: Postoperative complications developed in 20.0% of cases and were represented by dehiscence of the wound edges, bleeding, and in the long-term period by the formation of a pharyngostomy, osteradionecrosis with a pathological fracture of the jaw. The resulting post-resection defects are inevitably accompanied by severe functional impairments: impaired opening of the mouth was noted in 36.7% of patients, impaired swallowing - in 3.3%, speech impairments - in all patients. The acoustic characteristics of speech were lower than those of a healthy person.

Introduction: Ronchopathy is a chronic progressive disease manifested by upper airway obstruction and chronic respiratory failure. The key process of pathological snoring is the obstructive breathing disorders. The obstructive sleep apnea syndrome (OSAS) develops on the basis of snoring. OSAS is accompanied by episodes of hypoxia and reoxygenation, which cause an increase of the level of reactive free radicals whith following development of the oxidative stress. The activation of peroxidative processes of proteins (POP) and lipids (POL) are initiated by free radicals which are noticeable components of endogenous intoxication (EI). The aim of the study was to investigate the intensity of POP and POL processes, the levels of OSAS components, and the indices EI in patients with ronchopathy and OSAS of varying severity in the dynamics of treatment. Materials and methods: 40 patients with ronchopathy and OSAS were examined at the State Institution “Institute of otolaryngology named after prof. O.S. Kolomiychenko of the National Academy of Medical Sciences of Ukraine”. All patients were divided according to the degree of snoring and hypoxia index (HI) into 4 groups of 10 persons each. Control group was formed by 10 healthy donors. The object of biochemical studies was blood serum. The intensity of POP was assessed by reaction with 2,4-dinitrophenyl-hydrazine by the Levin’s method in Reznick’s modification. POL intensity was determined by the interaction with 2-thiobarbituric acid (TBA) by Goncharenko. Catalase activity was determined by the method of Korolyuk and co-authors. The content of free thiol groups was determined by interaction with 5,5'-dithio-bis(2-nitrobenzoic acid). The content of medium weight molecules (MWM) and tyrosine-containing peptides (TCPs) were determined by spectroscopy at 254 and 280 nm, respectively. Statistical processing of the results was performed using the software package for biometric data WinPEPI. Results: Prior to the treatment in patients with ronchopathy and OSAS of varying severity, an increase in the content of MWM and TCPs were noted, that indicates the development of endogenous intoxication. In patients of all groups there was an intensification of POP, which was manifested by a significant increase of aDNFGn, aDNFGo, and kDNFGn levels. The content of TBA-positive products in patients with ronchopathy of both groups was at the level of control and increased significantly at progression of pathological process. In addition, in all groups of patients an increase of catalase activity was detected on the background of TCPs level decrease. It was found the efficacy of the offered treatment of patients with ronchopathy and OSAS of varying severity. The indices of EI, POP, POL, and antioxidant system’s were directed to improvement in contrast to the state before treatment, and some of them were improved almost to the level of control. Acute nasopharyngitis in children is an inflammation of the nasopharyngeal mucosa. Recently, the use of saline solutions in the pathology of the nose and nasopharynx has become a leader in cleaning the mucous membrane. The main direction of modern pharmacotherapy of pathology of the nose and nasopharynx today is antiinflammatory therapy. There are a sufficient number of studies proving the effectiveness of mometasone furoate nasal spray in the treatment of inflammatory diseases of the nasopharynx in children. Our study examined the clinical efficacy of Etacid in the treatment of acute nasopharyngitis in children. Among the clinical symptoms were considered nasal breathing disorders, nasalness, snoring, cough during sleep. Endoscopy studied stagnant signs in the nasal cavity, nasopharyngeal discharge, enlargement of the pharyngeal tonsil. Proven sufficient clinical efficacy of the drug "Etacid" in the treatment of acute nasopharyngitis.

Introduction: The causative agents of acute tonsillopharyngitis are viruses and bacterial flora. Treatment of acute pharyngitis is mainly local, which includes aerosols with antiseptics. Aim: Study of antimicrobial action of "Apicold oral spray" on clinical isolates of microorganisms Streptococcus pyogenes, Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis and Candida albicans isolated from patients' oropharynx in patients. Material and methods: The effect of Apicold Oral Spray (manufactured by APIPHARMA doo) on clinical isolates of Streptococcus pyogenes, Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis, and Candida albicans in Ukrainian patients was studied. Antimicrobial activity was assessed after 5, 10 and 20 minutes of action of one dose of "Apicold oral spray" on microorganisms. Bactericidal and fungicidal action (inhibition of cell growth of microorganisms) is expressed as a percentage according to the control. Results and discussion: Apicold Oral Spray has a 100% antimicrobial effect after 5 minutes on clinical isolates of Streptococcus pyogenes, Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis and Candida albicans (complete growth inhibition of all 5 clinical isolates after 20 minutes). Conclusion: "Apicold oral spray" has a high antimicrobial effect on clinical isolates of Streptococcus pyogenes, Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis and Candida albicans, which is confirmed by 100% inhibition of the growth of all bacteria after 5 minutes

Introduction: COVID-19 disease can lead to serious complications of the ENT organs. Among them are the developments of aseptic thrombosis of the arteries of the ENT organs and face, as well as auditory dysfunction. Purpose: to investigate changes in auditory function and blood supply to the ENT organs of patients which underwent coronavirus COVID-19. Materials and methods: examination and treatment of patients with complications of coronavirus infection: facial soft tissue thrombosis (2) and auditory dysfunction (38). The results obtained: Coronavirus infection in two patients resulted in thrombosis of the soft branches of the internal and external carotid arteries with the development of necrosis of the mucous membrane of the lateral wall of the nose, nasal sinuses, septum and palate, which required surgical treatment. Acute sensorineural hearing loss with an increase in thresholds of sound perception to 40 dB was observed in 23 (60.5%) patients, and most of them (19 people) suffered from severe coronavirus infection and received comprehensive treatment (hormones, anticoagulants, etc.). Hearing loss with an increase in the threshold of sound perception to 40-60 dB was determined in 15 (39.5%) patients, of whom only 5 underwent severe COVID-19. Conclusions: Patients who have undergone a coronavirus infection caused by the SARS-CoV-2 virus may develop otolaryngological complications in the form of auditory dysfunction or arterial thrombosis. More severe bilateral damage to the auditory analyzer occurs in patients who underwent mild COVID-19 and did not receive anti-inflammatory therapy. The use of glucocorticoids and anticoagulants for the treatment of coronavirus infection on time prevents the development of complications from the auditory analyzer and blood coagulation system.

Olfactory dysfunction may be the only early clinical manifestation in COVID-19 patients with no other significant signs. It is typical of the disease and can be significant for testing. The purpose of the review is to provide guidance to the otorhinolaryngologist in the problem of olfactory dysfunction in SARS-CoV-2 infection. Materials and Methods: The authors analyzed the available clinical data on the problem of olfactory dysfunction in SARS-CoV-2 infection. The data of statistics, clinical symptoms and pathogenesis were studied. Toexplain anosmia in COVID-19 patients, 4 possible mechanisms are considered: nasal congestion / nasal congestion and rhinorrhea; death of olfactory receptor neurons; infiltration of the brain and damage to the olfactorycenters; damage to the supporting cells of the olfactory epithelium. The analysis of clinical cases of patients with prolonged ansomia against the background of COVID-19 was carried out. Conclusions: Smell after COVID-19 in most cases is restored without specific treatment. There are no reports of studies in patients with long-term anosmia.

Introduction: Allergic rhinitis affects 10 to 40% of the population, and among children the incidence is 2-25%. Aim: Evaluation of the effectiveness and safety of "Apicold Mint" nasal spray in children and adults with allergic rhinitis. Material and methods: The effect of "Apicold Mint" Nasal Spray (manufactured by APIPHARMA doo) was studied in 45 patients with allergic rhinitis. The clinical study involved 21 (47%) children aged 4 to 18 years and 24 (53%) adults. Results and discussion: The use of the drug "Apicold Mint" nasal spray helps to reduce the symptoms of allergic rhinitis and eliminates the feeling of dryness, which often occurs with the use of topical corticosteroids and decongestants. In addition, the use of the drug "Apicold Mint" nasal spray significantly reduced the need for vasoconstrictor drops, which is also positive in the treatment of patients with allergic rhinitis. Conclusion: Appointment of the drug "Apicold Mint" nasal spray in the complex therapy of allergic rhinitis contributes to a faster reduction of symptoms and increases the effectiveness of local drugs.

Congenital anomalies of the ossicular chain are a rare condition in clinical practice. In literature described different variants of the ossicular chain anomaly from stapes footplate ankylosis to complete aplasia of the ossicular chain, oval, and round window. Possible congenital ossicular chain anomaly should be suspected in case of non-progressive conductive hearing loss with a normal ear drum during an otoscopic examination, without a history of previous ear infections or trauma. In this article, we described the clinical case of congenital ossicular chain anomaly presented as fixed incus. Stapes was presented as single, straight, broad crus that were attached to the central part of the footplate. Stapedius tendon was absent. The footplate was completely mobile. Exploratory tympanotomy was performed to confirm a diagnosis of congenital ossicular chain anomaly. The ossicular chain was reconstructed with PORP titanium prosthesis. Furthermore, a literature review was done, conclusions presented.

Introduction: Based of tht author's doctoral dissertation of tht problems of insurance medicine, relevant monographs and scientific articles, an attempt is presented to substantiate tht option of transforming our purely governmental national system of insurance medicine. Purpose: Using more than thirty of our innovative organizational and medico-economic standards, criteria and regulations, in tht process of their "activation" in the conditions of a full-scale experiment under the pilot project program in the Health Care institutions of various levels of medical district, to determine the level of their effectiveness to justify the specified transformation during the launch of our system of insurance medicine. Materials and methods: The object of the study was all residents (up to 2000) of the family physician's area of responsibility. On the basis of structural-logical analysis and the use of medical-economic standards, doctors' labor costs, distribution of otolaryngological diseases into clinical-statistical groups (CSG) and diagnosticrelated groups (DRG), as well as the degree of their complexity and the complexity of possible surgical interventions, thanks to the formula for calculating the labor costs of doctors and treatment process we obtained evidencebased information about real "budget" cost of treatment of diseases of a certain CSG. Results and discussion: At the first in trade part mental stage of insurance medicine, the main tool should be a system of medical and economic standards, which include the price of labor costs, in particular doctors, wage fund, rates of increasing coefficients for the treatment of complex diseases, the level of financial incentives for doctors and the like. The tables present a mechanism for calculating the evidence-based cost of treating diseases of varying degrees of complexity and physician salaries, as well as the possibilities of forming both an adequate amount of funds that "follow the patient" and forecasting the health improvement budget for the family physician's area of responsibility. Conclusions: Presented an algorithm for the implementation of our medical and economic standards, the feasibility of which has been proven in the "field" conditions of the field experiment in the process of professional management, can constitute, under conditions of its continuous improvement, the medical and economic basis for the start of our system of budgetary and insurance medicine.
