Background: This review specifically focuses on the use of video consultation in diabetes management, in comparison to standard care. The population of chronic condition sufferers in the UK is considerable and teleconsultation use has, in recent years, been explored. COVID‐19 has created an additional pressure on health services to use teleconsultation. Diabetes mellitus affects approximately four million people in the UK. If clinical outcomes are uncompromised, the benefits of using a remote service could encourage the use of video consultation for diabetes management in normal practice.

A 52‐year‐old man with type 2 diabetes presented with peripherally spreading erythematous annular eruptions around the insulin injection sites on the anterior thighs. These lesions appeared shortly after initiation of subcutaneous premix human insulin injection. On examination, two well‐defined annular erythematous plaques of size around 10cm x 5cm with a rim of trailing scale over the margin were seen over the anterior aspect of bilateral thighs. He underwent assessment for diabetic complications screening and other relevant investigations. Histopathological examination of the lesion revealed uninvolved epidermis along with classical coat sleeve appearance of perivascular lymphocytic infiltration and dermal oedema in the upper dermis, which was suggestive of a diagnosis of erythema annulare centrifugum (EAC). The lesions resolved completely in two weeks after stoppage of insulin.

People with diabetes manage their condition 24 hours a day, seven days a week and only about 2% of decisions are made in consultation with a health care professional. Many factors impact on a person's ability to manage their diabetes, and consequently on their mental health and wellbeing. These include everyday life pressures: work, celebrations, holidays and major life events, such as loss of a loved one or a job. Diabetes‐specific stresses include diabetes distress and burnout, fear of hypos and complications, eating disorders and denial, with an increased risk of anxiety and depression. Some people have pre‐existing mental health conditions which lead directly to development of diabetes and add to the challenge of glycaemic management.

In this fourth article in our ‘Test Tips’ series, Jasmine Ramsden and Dr Ruth Poole compare the measurement of urine protein by three tests: urine dipstick testing, albumin:creatinine ratio and protein:creatinine ratio – providing guidance on key practical points to consider.

Purpose: Cognitive impairment is associated with worsening of diabetic retinopathy (DR). While cognitive impairment has been observed in the elderly with DR, no report on cognition is available in middle‐aged patients with DR. We explored the association between midlife cognitive impairment and DR in an Indian population cohort.

Purpose: The purpose of this study is to characterise the decision‐making and self‐management strategies of individuals with type 1 diabetes (T1D) participating in a 150‐mile ultra‐endurance cycling event and inform health care professionals working in T1D. Participation in ultra‐endurance exercise is becoming increasingly popular in the T1D community and self‐management in this context needs further investigation.
