Adherence to a blended smoking cessation treatment 坚持混合戒烟治疗

Blended face-to-face and web-based treatment is a promising way to deliver smoking cessation therapy. Little is known about adherence to blended treatment and since adherence have been shown to be a measure for treatment’s acceptability and a determinant for treatment’s effectiveness, this study explores adherence to a blended smoking cessation treatment (BSCT). Patients’ (n=75) data were analyzed to trace adherence to BSCT delivered at an outpatient smoking cessation clinic. In total, 18 patient activities were selected to assess adherence. The minimum degree of adherence to these activities, for patients who reported abstinence at 6 months after treatment initiation, was used as threshold for adherence to BSCT. Finally, predictors for being adherent were studied out of 33 person-, smoking- and health-related baseline characteristics. Patients reporting abstinence, adhered to at least 61% of BSCT activities. Applying a 60% threshold, 18% of the patients were classified as adherent to BSCT. Marital status and social modelling were independent predictors for adherence. Patients having a partner had 11-times higher odds of being adherent (OR=11.3; CI: 1.33-98.99; p=0.03). While for social modeling – graded from 0 (partner and friends are not smoking) to 8 (partner and nearly all friends are smoking) – each unit increase was associated with 28% lower odds of being adherent (OR=0.72; CI: 0.55-0.94; p=0.02). The current study is the first study exploring adherence to a blended face-to-face and web-based smoking cessation treatment (BSCT) based on a substantial group of patients. It revealed a rather low adherence rate to BSCT. Furthermore, adherence was predicted by two baseline characteristics pertaining to patients’ social environment.

WHO-ERS project on smoking cessation in primary care in countries with high prevalence of tobacco use. First year experience 世卫组织- ers关于烟草使用高流行国家初级保健中戒烟的项目。第一年工作经验

Aim: In 2016, WHO and ERS, formed a novel collaboration -a successive 3y project to support WHO’s training activities for building capacity of WHO Member States to provide tobacco cessation interventions (TCIs) to all patients with respiratory diseases, both in hospital and primary care settings. 3 countries with high prevalence of tobacco use and low availability of cessation support were selected as target countries to implement the Year 1 project (Greece, Republic of Moldova, Bangladesh). Methods: 1) A national training network capable of further training HCPs was established to provide brief TCIs to respiratory patients, 2)an online 6-module training course on brief TCIs for health professionals was developed in English. The train-the-trainer workshops were jointly conducted by WHO and ERS experts as 30 HCPs were trained on brief TCIs in each country and subsequently endorsed to train others through structured cascading training programs (Fig1). Consequently, the national training teams conducted 4-6 training workshops for primary care providers(PCP) from 2-4 cities in each country. All trained PCPs were supported to deliver 5As/5Rs brief TCIs. Results: (Fig1) More than 10,000 smokers were enrolled and given brief advice on quitting as existing training centres for on-the-job training of PCPs were strengthened in each of the 3 countries to train PCPs on brief TCIs. Success quitting rates at 1month were rather high (37,1%,31,7% &48,4%) although from unexperienced HCPs. WHO-ERS collaboration could improve and sustain structural elements paramount to promoting tobacco cessation and significantly impacting in reducing tobacco use in the target countries.

Effect of a short tobacco cessation intervention among pregnant women: pilot results of the TOB-G project 短期戒烟干预对孕妇的影响:ob - g项目的试点结果

Introduction: Despite the magnitude of the risks associated with tobacco use during pregnancy, an estimated 6-19% of woman in Europe will continue to smoke during pregnancy and a large portion of woman who quit will return to smoking following pregnancy. The purpose of the pilot study was toexamine the effectiveness of the Tobacco Cessation Guidelines for High-risk Groups (TOB-G) for Pregnant and Post-Partum women on aiding cessation during pregnancy Methods: The intervention was based on the TOB-G project guidelines d in a sample of 67 pregnant women recruited from obstetrics outpatient visitsand was delivered based on an adaptation of the 5As models for smoking cessation. A research assistant contacted participants by phone at 1 and 6-months to document smoking status. Participants received three behavioural counselling sessions through a combination of face-to-face and telephone consultation. Results: A significant increase in self-reported smoking abstinence was documented following the intervention with 43.9% and 45.6% of participants abstinent at the 1 and 6-month follow-up respectively. In a conservative analysis if we assume missing data for participants we were unable to reach for follow-up as active smokers the quit rates are 26.9% and 38.8% at the 1 and 6-month follow-up respectively. Conclusion: The counselling intervention based on the TOB-G guidelines was associated with significant increase in smoking abstinence among pregnant women. Further research with larger sample sizes is needed.

Background/Aims: Allen Carr’s Easyway to Stop Smoking (AC) is widely used. There are no published randomised clinical trials (RCT) to support its use. Pharmacotherapy and counselling are effective smoking cessation treatments but pharmacotherapy is not used in AC. AC is a behavioural intervention which seems unique. Primary aim: to determine if AC had higher quit rates than Quit.ie service (Quit.ie is an online portal for smoking cessation) at 3months(mos), secondary aims: quit rates at 1,6, and 12mos. and determining associated factors including weight change. Methods: Open, single centre, RCT, 300 smokers recruited, 18yrs plus, a minimum 5 cigarettes daily, matched for age, sex and education, were randomly assigned to AC and Quit.ie. Participants with acute cardiac, respiratory, serious psychiatric illnesses, on-going treatment for alcohol/ illicit drugs were excluded. AC consisted of a 5 hour seminar, in a group setting. Self-reported and CO validated quit rates were analysed at 1,3,6, and 12mos. Results: AC, 151 and Quit.ie, 149 mean age 44 years, outcomes for all 300 were analysed on an Intention to Treat basis.(West et al., 2005) AC had higher quit rates at 1,3,6, and 12mos, AC: 38%(n=57), 27%(n=40), 23%(n=35), 22 %(n=33) vs Quit.ie: 20%(n=30), 15%(n=22), 15%(n=23), 11%(n=17), respectively (all pvalues Conclusions: Allen Carr quit rates were superior to Quit.ie, comparable to established interventions, and deserves further evaluation.

Documentation of electronic nicotine delivery systems (ENDS) in medically admitted patients in Connolly Hospital Dublin. Background: Electronic nicotine delivery systems (ENDS) is commercially available since 2004.the health hazards posed by ENDS is not yet known. Documentation of ENDS commonly known as e cigarettes is important from surveillance and research point of view.the use of e cigarettes is reported to be 10% in Ireland. Methods: we performed retrospective analysis of 50 medical admission notes in admitted patients in Connolly hospital, Dublin.patients were selected randomly and approval from the ethics committee was obtained. Patients were 60% female, age [mean±SD] 75±13years,34% patients had known diagnosis of COPD. Results: shortness of breath was the commonest presenting complaint seen in 40% of patients,18% presented with cerebrovascular accident,12% presented with fall at home, 30% had other complaints. Smoking status was documented in 66% of the patients.38% of the patients had their smoking pack years documented in medical notes.36% of the patients had never smoked in their life.e cigarettes status was documented in 1 patient(0.03%). Alcohol consumption history was documented in 44% of patients. Conclusion: we hypothesize that patients don’t consider e cigarettes use as smoking. It’s important to have standardized documentation of e cigarettes to facilitate smoking cessation advice and discussions regarding potential health hazards and nicotine dependence. Good documentation in medical notes may help in surveillance and research into potential health hazards caused by electronic nicotine delivery systems (ENDS)

Electronic nicotine delivery systems - new challenge for tobacco control policy: results from Russian tobacco control policy evaluation study 电子尼古丁输送系统——控烟政策的新挑战:俄罗斯控烟政策评价研究结果

Popularity of electronic nicotine delivery systems (ENDS) is increasing in Russia. Still little is known about the situation with ENDS in the country. Aim: To assess the exposure of population to advertisement, promotion of ENDS in Russia, and attitudes and beliefs of the population regarding these products. Methods: Data from Russian Tobacco Control Law Evaluation Survey (June – August 2017) were analysed. 11699 randomly selected respondents from 4320 households in 5 Subjects of Russian Federation were interviewed and a stratified sample (n=5289 aged≥18 years) of smokers (n=3228), former smokers (n=1012) and never smokers (n=1049) participated in the survey. Results: In past 6 month 19.6% of the respondents were exposed to promotion of ENDS via internet, 18.4% - at the points of sale, 18.1% - via social networks. The other sources of advertisement had lesser contribution in marketing and promotion of ENDS. Exposure of the youngest age group (18-24) to e-cigarettes promotion was 2-3 times higher compared to the other age groups – via all sources (p 61% of respondents believe that the second hand vape of e-cigarettes is harmful for health of people. Majority agreed that is necessary to regulate allowed amounts of the nicotine in e-liquids (65.4%), ban the aromatic supplements (56,5%), ban vaping in public places where tobacco smoking is banned (70.6%), ban the sales of these products to minors (82%), and the use of e-cigarettes by those(83,7%). 77,6% support the need for ban of all forms of advertisement, marketing and promotion of ENDS. The Findings: will be used to support new strong legislation to regulate ENDS in Russia.
