Metastatic tumors to the pancreas in pediatric patients are exceedingly rare, and even more so when originating from primary bone tumors like osteosarcoma. Here, we present the case of a 12-year-old girl diagnosed with pancreatic metastasis as the initial presentation of osteosarcoma, emphasizing the diagnostic challenges and therapeutic considerations in such cases.

A giant hepatic hemangioma is a large benign liver tumor, typically over 10 cm in size. While often asymptomatic, these tumors can cause abdominal discomfort or pain. Diagnosis relies on imaging like ultrasound and CT/MRI scans showing characteristic features. Treatment options include surgery for symptomatic or enlarging tumors, with alternative approaches like embolization or ablation considered in certain cases. Prognosis is generally good with appropriate management, including rare instances of spontaneous regression, especially in postmenopausal women. Further research is needed to refine treatment strategies for giant hepatic hemangiomas.

Neonatal tetanus is a toxi-infection caused by Clostridium tetani, with a neurological tropism, causing significant morbidity and mortality in newborns in developing countries. The infection almost always starts in the umbilical cord. Diagnosis is clinical. Treatment is two-pronged: etiological (specific immunoglobulins, antibiotic therapy, anti-tetanus vaccination and treatment of the portal of entry) and symptomatic (sedatives and muscle relaxants, curarization). Prevention is the only effective option, and relies on vaccination of mothers during pregnancy and health education of the population. We report the case of a newborn hospitalized for neonatal tetanus and treated with antibiotic therapy combined with intrathecal serotherapy, with a good evolution.

The removal of metallic foreign bodies in the lower airways, a source of recurrent pneumonia, is not currently performed in the Congolese population using video-assisted thoracoscopy surgery (VATS) under unipulmonary ventilation. Multiple failures of rigid and/or flexible bronchoscopy are likely to be a cause of morbidity and mortality. To the best of our knowledge, we report a first experience in DRC, of right middle lobectomy on metallic foreign body by VATS in a 14-year-old girl after two years of accidental inhalation of a metallic key, in the face of two bronchoscopy failures. A partial transverse bronchotomy successfully performed under VATS allowed extraction of the foreign body followed by bronchial reconstruction with resection of the right middle lung lobe. The aim of the multidisciplinary team was to determine whether lobectomy by VATS would result in shorter hospital stays, fewer complications and faster recovery than lobectomy by thoracotomy, to review the key stages of case management using an unusual approach in the region, and to draw a conclusion based on this first experience.

Trigeminal neuromas are rare tumors arising from the trigeminal nerve, often presenting diagnostic challenges due to their infrequent occurrence and varied clinical manifestations. We present a case report of 52 year-old female patient presenting with right diplopia and progressive facial pain resistant to standard treatment. Early recognition and appropriate management are crucial to prevent complications and improve patient outcomes. This report underscores the importance of considering trigeminal neuromas in the differential diagnosis of facial pain and the pivotal role of imaging modalities in guiding clinical management decisions.

Abdominal wall endometriosis is a rare condition characterized by the presence of endometrial tissue outside the uterus, typically within the abdominal wall muscles. This condition often presents with cyclic abdominal pain, swelling, and a palpable mass, mimicking other abdominal pathologies. Diagnosis is challenging and requires a high index of suspicion, especially in women of reproductive age with a history of pelvic endometriosis. Imaging studies such as ultrasound and MRI can aid in diagnosis, but definitive diagnosis is made through histopathological examination of excised tissue. Treatment options include surgical excision of the lesion with clear margins to alleviate symptoms and prevent recurrence. Multidisciplinary collaboration between gynecologists and general surgeons is essential for optimal management of abdominal wall endometriosis cases. We present the case of a 38-year-old female patient diagnosed with endometriosis involving the right abdominal muscle.

This summary discusses the rare occurrence of non-traumatic bilateral rupture of the knee extensor apparatus. This pathology involves spontaneous rupture of the quadriceps et patellar tendons in both knees without a history of recent trauma. Non-traumatic tendon ruptures, particularly bilateral cases, are uncommon and may be associated with underlying systemic or metabolic conditions such as diabetes or renal failure, which can contribute to tendon fragility. Management typically involves surgical repair of the ruptured tendons followed by intensive rehabilitation to restore knee function and strength. Close monitoring is essential to identify potential complications and optimize long-term outcomes.

Introduction: Because of the important anatomical structures situated superficially, reconstruction of wrist soft tissue defects is challenging for the plastic surgeon. Depending on the characteristics of the defect, various reconstruction methods are commonly performed. A pedicled perforator flap stands as a valuable option providing reliable coverage of the defect while preserving the vascularity. Case report: A 35-year-old male presented with a post traumatic wrist soft tissue defect with exposure of underlying structures. Surgical debridement was performed, and the resulting defect was reconstructed with a distally-based radial artery pedicled perforator flap. Postoperative recovery was uncomplicated. Conclusion: Radial artery perforator flap proved to be a workhorse flap for soft tissue reconstruction of the wrist. It has many of the benefits of the radial forearm flap but minimizes the disadvantages.

Umbilical hernia is common and can lead to life-threatening complications such as incarceration or strangulation. The presence of malignant tumours in hernial sacs remains very rare. We report here a case of epiploic carcinosis discovered during cure of an incarcerated umbilical hernia in a 75-year-old woman. She had macroscopic signs of malignancy during the operation.

Orbital cellulitis is extremely rare in newborns, constituting an emergency that can jeopardize functional or vital prognosis. Early identification of symptoms and implementation of appropriate treatment are essential to ensure a good prognosis and reduce the risk of long-term complications. We report the case of a 16-day-old male newborn hospitalized for Left orbital cellulitis stage 3 according to the Chandler classification, treated with antibiotics and an evacuation puncture with a good outcome.
