Hallux rigidus (HR) represents one of the most frequent forefoot disorders, together with hallux valgus and lateral toe deformities, being the most frequent degenerative process in the foot and ankle.This publication is the result of the collaboration of colleagues of the Spanish Society of Medicine and Foot and Ankle Surgery (Sociedad Española de Medicina y Cirugía del Pie y Tobillo, SEMCPT) and the Argentinean Society of Medicine and Foot and Leg Surgery (Sociedad Argentina de Medicina y Cirugía de Pie y Pierna, SAMCPP). It is an honor for us to have this work edited in the Journal of the Foot & Ankle, a journal with a marked Latin identity. We really thank Alexandre Leme Godoy-Santos, Chairman of the Editorial Board, for his help and contribution. This update topic has been coordinated by R. Viladot Pericé and A. Viladot Voegeli, together with M. Herrera and M. Núñez-Samper.Considering the interest and the extent of this topic and, according to the coordinators, we have divided the publication into two parts. The first one describes general aspects (definition, etiology, classification, treatment algorithm, etc.) and the conservative treatment of HR, whereas the second one addresses the different surgical techniques described to treat this disease.The aim of this work is to review our knowledge on HR and to perform an update on the innovations that have emerged during the last years.

Fracture of the posteromedial talar process (Cedell fracture) is a rare injury and is easily misdiagnosed as a simple ankle sprain. Suspicion should be heightened if specific mechanisms of injury are present, and, in these cases, a CT scan should be performed. Significant ankle pain and disability can result if these injuries are not identified and treated properly. Few cases have been described in the literature, and the efficacy of surgical techniques and approaches for fractures of the posterior talar process remains controversial. We report a 56-year-old man with an acute posteromedial talar fracture treated arthroscopically. This approach provides good access to the posterior ankle compartment, subtalar joint, and extraarticular structures, which may allow fracture reduction and fixation under arthroscopic visualization. Objective: To review indications for limb amputation or reconstruction using the SUPERankle procedure in patients diagnosed with Paley type 3 or 4 fibular hemimelia; To evaluate the correction of deformities needed to achieve a stable plantigrade foot, review variations of the original technique and their applicability, and describe challenges encountered in their execution. Methods: Qualitative, retrospective, descriptive study of 4 patients who underwent the SUPERankle procedure between 2019 and 2020 for treatment of foot and ankle deformities in fibular hemimelia. Pre- and postoperative clinical and radiographic evaluations were performed to identify objective evidence of the correction of foot and ankle deformities. The operative techniques employed are described. Results: The included patients met anatomical and psychosocial criteria for reconstruction, as established in the literature. Clinical correction of foot and ankle deformities was achieved, but on radiographic evaluation, there was no statistical difference; this was likely due to the small number of patients, given the rarity of fibular hemimelia. Conclusion: The procedure is reproducible, but requires a knowledgeable, well-trained surgical team. Correction of foot and ankle deformities can be achieved. The choice between amputation or reconstruction should be the result of an informed decision by Family members, supported by a multidisciplinary team. O método de Ponsetti é considerado, hoje, o padrão-ouro para o tratamento do pé torto congênito, inclusive em casos sindrômicos como a mielomeningocele. Sua indicação em casos negligenciados é bem estabelecida em crianças e adolescentes, mas para adultos, permanece incerto. Apresenta-se um caso clínico de uma paciente de 27 anos, portadora de mielomeningocele, com pés tortos congênitos negligenciados, que foi tratada nessa idade, pela primeira vez, com o método de Ponsetti, com obtenção de bons resultados. Em países em desenvolvimento, frequentemente encontram-se pacientes adultos portadores de pés tortos congênitos que não tiveram acesso a assistência a saúde. Apesar da idade limite para instituir o método de Ponsetti como opção terapêutica ainda ser obscura, é uma opção barata, segura, acessível, plausível de resultados satisfatórias mesmo em casos adultos, negligenciados e síndrômicos.

We report the case of a 60-year-old woman with a schwannoma of the medial plantar nerve. She presented with the complaint of pain for about 2 years and a painful lesion in the medial plantar region of the left forefoot and on the second toe. Physical examination showed well-defined, firm tumor masses painful to palpation on the medial and plantar surface of the left forefoot and second toe. We surgically explored the area and excised 3 lesions, resulting in complete pain relief and no aesthetic complaints. Schwannomas of the medial plantar nerve are rare tumors, with only a few reports of cases extending to the forefoot. The finding of multiple schwannomas may be a red flag for the possible existence of local, painful, nerve lesions despite a negative Tinel sign. Appropriate surgical planning contributed to our successful intervention, without aesthetic, painful, or functional sequelae for the patient. Level of Evidence V; Therapeutic Studies; Expert Opinion.

In 1997, the US National Library of Medicine (NLM) developed an Internet search system for publications in the medical field based on the MEDLINE database, PubMed.PubMed was established in 1966 and has more than 32 million citations in 5400 American journals from 80 countries around the world, offering users multiple ways to search for these publications. They can be searched by title, by field, by author, by diagnosis, or a multitude of other ways. This search tool is offered as a free service by NLM and has completely changed information systems in the medical field worldwide, making the electronic publication of scientific papers possible and desirable. The vast majority of these publications are originally from the MEDLINE database of NLM, but there are other publication sources. Belonging to PubMed makes the publication, the authors, the institution, and the research itself accessible without restrictions. There is currently a large number of requirements to include a journal in PubMed. Some are of a legal nature, such as the International Standard Serial Number (ISSN), others refer to journal publication frequency (at least 25 peer-reviewed articles published), and some refer to the quality and type of articles (minimum number of original studies, case reports, etc.). With the online publication of a large number of journals, PubMed is able to classify journals into more restricted categories – for example, accepting journals devoted exclusively to surgical techniques. The requirements are published and easily accessible, but Latin American journals have great difficulty in getting indexed. In the field of orthopedics in Latin America, the Brazilian Orthopaedic Journal (RBO), Acta Ortopédica Brasileira, and Revista Mexicana de Ortopedia y Traumatología are the only ones indexed.Assessments are conducted by third-party assessors who place great value on some points, about which we were able to learn through RBO’s indexing process. 1. Objective: To evaluate the postoperative results of patients who underwent surgical treatment of hallux valgus with a modified Reverdin-Isham (RI) technique and to compare the achieved correction with that reported by studies using the original technique. Methods: This is a retrospective study with patients with mild to moderate hallux valgus who underwent surgery from June 2010 to July 2019. All patients were operated using the modified RI technique. Data were collected through the American Orthopaedic Foot and Ankle Society (AOFAS) questionnaire, in addition to pre and postoperative radiographic studies of the intermetatarsal angle (IMA), the hallux valgus angle (HVA), and the distal metatarsal articular angle (DMAA). Results: The mean postoperative follow-up was 30.1 months, and the mean age of patients was 56.4 years. The median AOFAS score in the postoperative period was increased by 56 points. The mean HVA was reduced in 16.5°, the mean IMA was reduced in 4.3°, and the mean DMAA was reduced in 10°. There were no cases of displacement or deviation of the first metatarsal head during the postoperative follow-up. Conclusion: The modified RI technique provided considerable stability to the osteotomy, in addition to a significant correction of the measured angles and an improvement in AOFAS scores, demonstrating itself as an effective technique for correcting mild to moderate hallux valgus. Methods: This is a retrospective study with patients with mild to moderate hallux valgus who underwent surgery from June 2010 to July 2019. All patients were operated using the modified RI technique. Data were collected through the American Orthopaedic Foot and Ankle Society (AOFAS) questionnaire, in addition to pre and postoperative radiographic studies of the intermetatarsal angle (IMA), the hallux valgus angle (HVA), and the distal metatarsal articular angle (DMAA). Results: The mean postoperative follow-up was 30.1 months, and the mean age of patients was 56.4 years. The median AOFAS score in the postoperative period was increased by 56 points. The mean HVA was reduced in 16.5°, the mean IMA was reduced in 4.3°, and the mean DMAA was reduced in 10°. There were no cases of displacement or deviation of the first metatarsal head during the postoperative follow-up. Conclusion: The modified RI technique provided considerable stability to the osteotomy, in addition to a significant correction of the measured angles and an improvement in AOFAS scores, demonstrating itself as an effective technique for correcting mild to moderate hallux valgus. Objective:The aim of this study was to perform the translation and cultural adaptation of the Olerud-Molander Ankle Score (OMAS) in Brazilian Portuguese, in addition to determining its validity and reliability. Methods: This was a cross-sectional case-series study. The translation and adaptation processes were conducted in 5 stages: initial translation (2 bilingual Brazilians); synthesis of translations; back translation (2 bilingual Brazilians not involved in the first part of the study); consensus version and assessment (technical committee); and testing phase. The test-retest reliability and construct validity of the Brazilian Portuguese version of the OMAS (OMAS BrP) were evaluated in a sample of 40 participants. Construct validity was determined based on the correlations of the OMAS-BrP to the Foot Function Index (FFI) and Short-Form 12 (SF-12). Results: The OMAS-BrP had excellent test-retest reliability (ICC=0.99). The OMAS-BrP showed moderate and weak correlations with the SF 12 PCS and SF-12 MCS (r=0.68 and p<0.001; r=0.38 and p=0.014, respectively). The correlation coefficient between the OMAS-BrP and the FFI was graded as excellent (r=-0.846 and p<0.001). There was a strong correlation between the OMAS-BrP, the SF-12 PCS, and the FFI. Conclusion: The OMAS-BrP is a valid and reliable questionnaire, with psychometric parameters that are similar to those of its original version and other cross cultural adaptations. The OMAS-BrP is a useful patient-reported outcome (PRO) measure to evaluate Brazilian Portuguese-speaking patients with ankle fractures. Level of Evidence IV; Therapeutic Studies; Case Series.

To critically evaluate the current literature on the etiopathogenesis of Charcot neuroarthropathy, its diagnostic methods and therapeutic management. Methods: We searched for studies that related Charcot arthropathy with a location in the foot and ankle in the PUBMED and MEDLINE databases. Results: A total of 52 studies were used for this analysis. Conclusion: Charcot neuroarthropathy is a serious disease with significant potential to impact patient quality of life. Although its pathogenesis still raises much controversy, neuropathy seems to have a central role, leading to a trauma, injury, and inflammation cycle. Level of Evidence V; Therapeutic Studies; Expert Opinion.

We report an unusual case of extra-articular gouty tophus in the left medial malleolus. A 33-year-old man with a previous diagnosis of chronic gout presented with mild, non-disabling ankle pain associated with gout attacks. Imaging and histopathologic findings were inconclusive. The tumor was surgically resected, and the cavity was filled with methyl methacrylate. Histopathology confirmed the diagnosis in a sample collected intraoperatively. Giant cell tumor and bone cyst were ruled out. The patient had a good postoperative outcome. Level of Evidence V; Therapeutic Study; Expert Opinion.
