A case of removable partial denture restoration using implant supported surveyed crown in a maxillary edentulous patient

When planning oral rehabilitation for maxillary edentulous patients, fixed pros-thetic restoration using implants, complete denture restoration or overdentures using implants can be considered as treatment methods. In the case of complete denture restoration, it does not require additional surgery and is relatively eco-nomical. In the case of implant-supported fixed prostheses, the functional part is generally superior to that of complete denture restoration, but there are cases in which implant placement is clinically difficult. Recently in consideration of the patient’s needs and the condition of the remaining alveolar bone, after partial implant placement, a method of restoring with a removable partial denture using implant-supported surveyed crown is also being attempted. This case is a case of performing a removable partial denture restoration using implant-supported surveyed crown in the anterior maxilla, and showing satisfactory esthetic and functional results.

The functionally generated path (FGP) technique, first described by Meyer in 1933, is a method in the oral cavity to use the movement pathway formed by the opposing cusps within the border movement of the mandible. Using this method, an appropriate occlusal shape can be given to the patient. In this case, the FGP technique was selected to provide a bilateral balanced occlusion when restoring the edentulous maxilla that opposes the natural mandibular teeth with irregular arrangement with overdentures. In addition, in order to precisely form the occlusal surface of the posterior region with the FGP technique and to reduce the attrition of denture teeth, zirconia denture teeth, not conventional resin artificial teeth, were individually manufactured. After treatment with these materials and methods, satisfactory results were obtained for both the operator and the patient. (J Korean Acad Prosthodont 2022;60:55-62)

Purpose. The purpose of this pilot study is to evaluate the reliability and validity of the Korean version of the oral health impact profile (OHIP-EDENT K) for edentulous patients. Materials and methods. The study was conducted on 12 patients who fabricated overdenture in the Department of Prosthodontics, Korea University, Guro Hospital. All subjects completed the Korean version of Oral Health Impact Profile (OHIP K) questionnaire. Shorten version of the OHIP called OHIP-14 K and OHIP-EDENT K were derived from the datasets. Cronbach’s alpha was used to measure internal consistency of the summary scores for OHIP-EDENT K. The Spearman’s correlation coefficient between the summary scores for OHIP-EDENT K and OHIP K was calculated to evaluate concurrent validity. Results. The reliability of the summary scores for OHIP-EDENT K was acceptable (α=.736). The Spearman’s correlation coefficient of the summary scores for OHIP-EDENT K and OHIP K was 0.966, which was statistically significant (P<.001). OHIP-EDENT K exhibited less susceptibility to floor effects than OHIP-14 K and appeared to measure change as effectively as OHIP K. In order to prove the reliability, respon-siveness and validity of OHIP-EDENT K, further studies with more samples are needed. Conclusion. The OHIP-EDENT K, a questionnaire on oral health-related QOL comprising 19 items, has measurement properties comparable with the full 49-item version. This modified shortened version can be an alternative questionnaire to full version of OHIP K and OHIP-14 K in edentulous patients. (J Korean Acad Prosthodont 2021;59:305-13)

With the rapid development of aesthetic ceramic materials and resin adhesives, the transition from metal ceramic crown to all-ceramic restoration is being actively carried out. When making porcelain restorations, the advantages can be maximized only when appropriate forms and techniques are used. Recently, an esthetic partial-coverage restoration method with supra-gingival margins was introduced by applying a minimally invasive tooth removal method that preserves enamel as much as possible and enhances adhesion efficiency. Through this, various advantages such as minimization of tooth structure removal and ease of excess cement removal can be obtained. In addition, since this manufacturing method does not require cord packing, it is possible to save time for impression taking and reduce patient discomfort. Furthermore, the margins can be clearly obtained without the intervention of gingival fluid or blood, making it possible to manufacture more accurate restorations. In the following cases of patients with partial tooth defects, esthetic ceramic partial-coverage restorations with supra-gingival margin was applied using minimally invasive tooth preparation method according to the concept of adhesive dentistry. Therefore, unnecessary tooth abutment preparation was prevented and residual tooth structure could be preserved as much as possible. After delivery of the ceramic partial-coverage restorations, favorable outcome was obtained. (J Korean Acad Prosthodont 2021;59: 319-32)
