The role of organizational licensing in healthcare. 组织许可在医疗保健中的作用。

Much has been written about organizational accreditation in healthcare, but little attention has been paid to licensing providers as a means to control quality in the provision of health and social care. Licensing has a variety of different applications but frequently involves a mandatory assessment against standards. This paper reports on a comparison of the various ways in which the basic requirements for quality are framed in a range of licensing systems and concludes that there are considerable similarities in the approach to quality used across both health and social care licensing systems in many countries.

Paracetamol and fever management. 扑热息痛和发烧管理。

Paracetamol is one of the most commonly used legal drugs in the western world. Its availability is good, cost is low, and its uses include 'over-the-counter' (OTC) distribution, primary care prescribed therapy, secondary care 'post-operative' application and emergency treatment. Stated benefits of paracetamol include: the drug's analgesic effects, preference to aspirin in avoidance of Reye's syndrome, good patient tolerance, and iatrogenic complications are infrequent and minor. Stated cautions include hepatotoxic effect following minor doses and short duration use and users may incur compromised immune integrity. This paper is concerned with paracetamol's role in fever management. Public concern regarding, in particular, childhood fever and febrile convulsions is largely unwarranted. Despite paracetamol's reputation as a popular fever-reducing agent the drug is poorly effective in the control of febrility and febrile convulsions showing no important advantage compared with placebo. Paracetamol is probably grossly over-prescribed for fever management and its value more perceived than real. Greater efforts are needed to inform patients of the natural benefits of the biological strategy of fever and of the highly limited and in some cases contraindicated use of paracetamol in fever management.

Better design quality of public toilets for visually impaired persons: an all-round concept in design for the promotion of health. 提高视障人士公厕的设计质素:以促进健康为目的的全方位设计理念。

According to United Nations statistics, about one-thirtieth of the world's population is visually impaired. These visually impaired persons (VIPs) face a variety of difficulties in their daily lives. This is the case not only in countries with a shortage of resources or with relatively low living standards, but also in developed countries. Most of the time, such difficulties in daily life come from the misunderstanding of VIPs' wants and needs and in turn poor design quality. To enhance equal opportunities in society, promote public health and improve the design quality of the public environment and facilities, a project on how VIPs access public toilets has been under way since 2004. To maintain better design quality in public toilets, the FISH concept has been initiated. This design concept includes the design considerations of friendly, informative, safe, and hygienic. This paper reviews the wants and needs of VIPs that should be considered in using public toilets, and the help that they need to be given. Based on the findings of the project, this paper then discusses how better quality designs for public toilets to promote public health can be obtained by implementing FISH.
