Evaluation of Impacted Maxillary Canine Position Using Panoramic Radiographs and Cone-beam Computed Tomography 利用全景x线片和锥束计算机断层评估上颌阻生犬位

The purpose of this study was to compare the mesio-distal and vertical position of impacted maxillary canines on panoramic radiographs with the labio-palatal position of impacted canines and root resorption of adjacent teeth on CBCT (ConeBeam Computed Tomography). A total of 54 patients with 69 impacted maxillary canines were included in this study. On panoramic radiographs, the mesio-distal (Sector I - V), and vertical position, and angulation of impacted maxillary canines were evaluated. Labio-palatal position and root resorption of adjacent teeth were evaluated on CBCT. Labial impaction of canines was most frequent in panoramic sector I. Impaction within the arch was most frequent in sector I and II. Palatal impaction was most frequent in sector III, IV, and V. Mesially positioned canines on panoramic radiographs tended to be palatally impacted. Most of the root resorption of adjacent teeth occurred in sector III, IV, and V (p < 0.05). Canines with low angulation on panoramic radiographs tended to be palatally impacted and resorb the roots of adjacent teeth. The vertical position of impacted canines on panoramic radiographs was not statistically significant with respect to labio-palatal position and root resorption of adjacent teeth on CBCT. Analysis of the mesio-distal position and angulation of impacted maxillary canines on panoramic radiographs can be used to predict the labio-palatal position of impacted canines and root resorption of adjacent teeth.

Factors Influencing the Duration of Forced Eruption in Impacted Maxillary Canines 影响上颌阻生牙强制出牙持续时间的因素

The aim of this study was to identify factors that affect the treatment duration and to predict the duration of forced eruption in impacted maxillary canines using panoramic radiographs and cone-beam computed tomography. This retrospective study was performed by reviewing medical records and radiographs of 73 patients (93 impacted maxillary canines) from the age of 8 to 18 years who were treated with surgical and orthodontic interventions on impacted maxillary canines from January 2012 to December 2020 in Ajou University Dental Hospital. Stepwise multiple regression analysis showed that the distance between the canine cusp tip and the occlusal plane, mesio-distal location, bucco-palatal location, patient’s age, and use of rapid palatal expansion are significant factors with regard to the duration of forced eruption. There was a statistically significant correlation of the treatment duration with the angulation between the axis of the canine and the occlusal plane and unilateral or bilateral impaction. It can be concluded that the duration of forced eruption in impacted maxillary canines could be shorter when the impacted canine is closer to the occlusal plane and located in the lateral incisor or canine area, buccal or middle, the patient is younger and uses rapid palatal expansion.

Minor Orthodontic Treatment Using NiTi Wire Exerting Light Force: Case Reports NiTi丝施加光力治疗轻微正畸1例

Minor orthodontic treatment using a thin wire with high elasticity can shorten the treatment period and reduce the load on the anchorage by the application of light force. Since it can be applied immediately without a dental laboratory procedure and does not require the patient's cooperation, it can be clinically useful. The cases reported here have led to positive results in short periods of treatment, using only a segmented straight 0.012 inch NiTi wire and flowable resin to address various locations within the oral cavity, such as the anterior teeth, premolars, and molars.

pH, Ion Release Capability, and Solubility Value of Premixed Mineral Trioxide Aggregates 三氧化二矿预混料的pH、离子释放能力和溶解度

The current study aimed to compare the pH, solubility value, and ion release capability of premixed mineral trioxide aggregates (MTAs) versus conventional pulp capping materials before and after setting. The following materials were used: resin-modified calcium silicate cement (TheraCal LC®, TLC), resin-modified calcium hydroxide cement (Ultra-BlendTM plus, UBP), and 2 kinds of premixed MTA (Endocem MTA® premixed regular 〔EMPR〕 and Well-RootTM PT 〔WRP〕). The specimens of each material were prepared before and after setting and were immersed in distilled water. The materials’ pH and solubility value were assessed. Next, three kinds of ion (calcium, sulfide, and strontium) released by pulp capping materials were evaluated via inductively coupled plasma atomic emission spectrometry. In the after-setting group, the pH of TLC and UBP decreased. However, the pH of the premixed MTAs increased with time. TLC released a higher concentration of strontium ion compared with the other materials. Meanwhile, EMPR released a significantly high concentration of sulfide ion (p < 0.05). In the after-setting group, the 2 kinds of premixed MTAs released a significantly higher concentration of calcium ion compared with the other materials (p < 0.05). In the after-setting group, EMPR had a significantly low solubility value (p < 0.05). The Kruskal-Wallis test, followed by the Mann-Whitney U test with Bonferroni correction, was used in statistical analysis. In conclusion, resin-modified calcium silicate cement, modified calcium hydroxide cement, and the 2 kinds of premixed MTAs had an alkaline pH and low solubility value and they released various concentrations of ions after setting.

Sleep Disordered Breathing in Children 儿童睡眠呼吸障碍

Sleep disordered breathing (SDB) is a disease characterized by repeated hypopnea and apnea during sleep due to complete or partial obstruction of upper airway. The prevalence of pediatric SDB is approximately 12 - 15%, and the most common age group is preschool children aged 3 - 5 years. Children show more varied presentations, from snoring and frequent arousals to enuresis and hyperactivity. The main cause of pediatric SDB is obstruction of the upper airway related to enlarged tonsils and adenoids. If SDB is left untreated, it can cause complications such as learning difficulties, cognitive impairment, behavioral problems, cardiovascular disease, metabolic syndrome, and poor growth. Pediatric dentists are in a special position to identify children at risk for SDB. Pediatric dentists recognize clinical features related to SDB, and they should screen for SDB by using the pediatric sleep questionnaire (PSQ), lateral cephalometry radiograph, and portable sleep monitoring test and refer to sleep specialists. As a therapeutic approach, maxillary arch expansion treatment, mandible advancement device, and lingual frenectomy can be performed. Pediatric dentists should recognize that prolonged mouth breathing, lower tongue posture, and ankyloglossia can cause abnormal facial skeletal growth patterns and sleep problems. Pediatric dentists should be able to prevent these problems through early intervention.

Color Distribution of Maxillary Permanent Incisors in Korean Pediatric Patients Using a Spectrophotometer 用分光光度计观察韩国儿童上颌恒切牙的颜色分布

This study aims to analyze the color distribution of maxillary permanent incisors in Korean pediatric patients and determine the effects of age and root developmental stage on tooth color. The L*a*b* values of 404 sound and fully erupted maxillary incisors without dental caries, restorations, trauma history or discoloration from 101 Korean patients between ages 7 and 15, with a mean age 10.0 ± 1.5, were analyzed with a spectrophotometer. CIE L*a*b* values were 84.01, 0.17, and 24.07 in maxillary central incisors, and 82.33, 0.31, and 25.99 in maxillary lateral incisors. L* values of maxillary central incisors were higher, and b* values of maxillary central incisors were lower than those of maxillary lateral incisors (p < 0.001). The color differences among the subregions exceeded the clinical perceptibility threshold in both of the maxillary central and lateral incisors. L* value for children at age 10 and younger was 84.13 in maxillary central incisors and 84.04 in maxillary lateral incisors, and those of older patients were 80.62 and 80.56, respectively. L* value of maxillary incisors of children at age 10 and younger was significantly higher than that of older patients. The root developmental stage did not affect tooth color. This study suggests that the color differences between maxillary central and lateral incisors and among the subregions of a tooth and the effects of age should be considered for aesthetic restorations of permanent incisors in pediatric patients.

L* value for children at age 10 and younger was 84.13 in maxillary central incisors and 84.04 in maxillary lateral incisors, and those of older patients were 80.62 and 80.56, respectively. L* value of maxillary incisors of children at age 10 and younger was significantly higher than that of older patients. The root developmental stage did not affect tooth color. Investigation of Stress-Inducing Factors and Occupational Stress Levels in General and Pediatric Dentists 普通牙医和儿科牙医压力诱发因素及职业压力水平调查

The purpose of this study was to compare factors causing stress in pediatric and general dentists when treating pediatric patients and to evaluate their overall occupational stress level. A total of 191 dentists participated in the online survey, consisting of 66 pediatric dentists and 125 general dentists. The questionnaire was conducted using Google Form. Both groups were stressed due to poor cooperation of patient and caregiver, uncertain prognosis of treatment, and low cost of pediatric dental treatment. The pediatric dentists felt relatively high stress due to poor cooperation from caregivers and an uncertain prognosis of treatment (p < 0.05). Overall occupational stress was high in both groups in the order of patient, time, job environment, and income-related stress, and the degrees of pediatric dentists were lower than general dentists. Among the 3 sub-factors of occupational burnout, more than 98% of both groups showed burnout in “depersonalization”, and more than 69% of both groups showed burnout in “emotional exhaustion”. Both groups showed a low burnout rate in “reduced sense of accomplishment” and pediatric dentists showed a higher sense of accomplishment than general dentists (p < 0.05). This study showed that both groups were under high occupational stress due to various factors, and efforts were required to relieve stress.

Management of Displaced Maxillary Canines by Extraction of the Primary Canine: Factors Affecting Treatment Outcome 拔牙治疗上颌牙移位:影响治疗结果的因素

The purpose of this study was to evaluate the effect of interceptive primary canine extraction in palatally and buccally displaced maxillary permanent canines, and to analyze the clinical and radiographic factors affecting the treatment outcome. 97 maxillary permanent canines from 86 patients whose maxillary permanent canine were in the mesio-occlusal directions and overlapped with the roots of the adjacent teeth were analyzed. In 64 of 97 (66.0%) maxillary permanent canines, the displaced crown was completely deviated from the adjacent lateral incisor root only by extraction of the primary canine. Not only the characteristics of maxillary permanent canines such as bucco-palatal displacement direction, horizontal and vertical position of the crown tip, and presence of apical closure, but also periapical rarefaction on the primary canine and peg-shaped adjacent lateral incisor significantly affected the treatment outcome.

The Distribution of Pediatric Patients According to Accessibility to Dental Facilities in Pusan National University Dental Hospital 釜山大学牙科医院的儿童病患分布情况

The purpose of this study was to analyze the accessibility to dental facilities and the differences in chief complaints of new patients who visited Pusan National University Dental Hospital. We collected information from electronic medical records for 1,820 new patients. The accessibility was analyzed by measuring the distance from the patient’s house to the dental facilities. The distance was categorized into 10 km sections, and the presence or absence of a local pediatric dental clinic within 10 km of the patient’s house was also categorized. As the distance between the house and Pusan National University Dental Hospital increased, the proportion of patients who visited for dental caries decreased, and orthodontic treatment increased. Dental caries accounts for 27.9% of less than 10 km and 20.5% over 30 km. Orthodontic treatment accounts for 25.4% within 10 km and 27.3% more than 30 km away. The presence or absence of a local pediatric dental clinic within 10 km did not significantly affect the distribution of chief complaints. This study can be used as basic research data to establish effective treatment measures that can improve the physical and geographical accessibility of patients visiting pediatric dentistry.

Assessing Spatial Accessibility to Pediatric Dental Clinics Using the Geographic Information System 利用地理信息系统评估儿童牙科诊所的空间可达性

The purpose of this study was to evaluate the accessibility of pediatric dental clinics and quantitatively compare the regions with the best and worst accessibility to pediatric dental clinics in South Korea using the geographic information system. Seoul city showed the highest density of pediatric dental clinics, while Gyeongbuk and Jeonbuk provinces showed the lowest density. Seoul city had a service area ratio of 100% in all districts on a 10 km basis. In Gyeongbuk and Jeonbuk provinces, more than half of the districts had a service area ratio of 0%. In Seoul, the average distance from elementary school to the nearest pediatric dental clinic was 3.45 km. The average distance values were 43.66 km and 40.57 km in Gyeongbuk and Jeonbuk provinces, respectively, which were 12.66 times and 11.76 times longer than the average distance in Seoul city.
