Analysis of Adult Users of Emergency Departments Visits for Primary Dental Related Complaints in the United States in 2015AW 2015年美国因初级牙科相关投诉到急诊室就诊的成年用户分析

Objectives: Using nationally representative data, the researchers examined the differences in dental care utilization in Hospital-Based Ambulatory Care Services working age adults associated by lack of insurance, and other co-variates. Methods: The researcher used data from the 2015 National Hospital Ambulatory Medical Care Survey (19-65 years age; N= 12,956 unweighted observations). Multiple logistic regression analysis was performed to determine the association with dental insurance coverage and other covariates. Results: In 2015, There were an estimated 83 million ED visits among working adults ages 19-64 in the United States. Dental related complaints accounted for between 0.1 and 0.5 percent of all visits, depending on patient characteristics. Dental visits were significantly more likely among patients with self-paying or with government insurance relative to the privately insured. Conclusion: Medicaid and Self pay adults had significant higher odds of making dental emergency visits. The limited scope of dental treatment in the ED, coupled with poor availability of safety-net dental resources, may result in dental exacerbations. The engagement of safety-net dental service accessibility is crucial to reducing dental ED visits and improving dental health, particularly among low-income, self-pay populations.

The Influence of Combined Surface Treatmentsusing Plasma of Argon, Er:Yag Laser and Sandblasting on the Shear Bond Strength of Zirconia Core to the Veneering Ceramic – In Vitro Study 氩等离子体、Er:Yag激光和喷砂联合表面处理对氧化锆芯与贴面陶瓷剪切结合强度的影响

Background: Success of zirconia-based prosthesis is greatly affected by ceramic chipping and delamination due to poor bonding between zirconia framework and veneering ceramic. This study uses combination of surface treatment with Non-thermal Argon Plasma to evaluate its effect on Shear Bond Strength (SBS). Objective: The study aimed to evaluate the effect of different surface treatment using sandblasting, argon plasma, Er:YAG laser and its combination on SBS of zirconia to veneering ceramic. Material and Method: 150 milled and sintered zirconia blocks (10mm x 18mm) were subjected to various surface treatments and were randomly divided into five groups according to the surface treatment methods namely. Group 1: Argon plasma(220 V,100W and constant gas pressure 1 kg/cm2 and gas flow 8.5L/min) + Liner (E max ceram zirliner), Group 2: Atmospheric pressure argon plasma, Group 3: Sandblasting, Group 4: Sandblasting + Laser ablation ( Er:YAG ; 300mJ/pulse energy, frequency-10Hz ) + Argon plasma and Group 5: Sandblasting + Argon plasma. Specimens were subjected to shear bond strength test by a universal testing machine with a crosshead speed of 1 mm/min. The data were analysed with one-way ANOVA and Bonferoni test for multiple comparisons tests (α=0.05). Result: Highest shear bond strength value was obtained for Group 4 followed by Group 5, Group 3, Group 2 with least value obtained for Group 1. Conclusion: This study shows that sandblasting + Er:YAG + Argon plasma showed higher shear bond strength when compared to other groups.

New Jersey FQHC Process Improvement Increases Oral Health Access for HIV Patients 新泽西州FQHC流程改进增加了艾滋病毒患者的口腔健康

Introduction: Patients living with HIV/AIDS (PLWHA) have more unmet oral health care needs than the general population, outpacing unmet medical needs [1]. Poor oral health can impact a person’s confidence and ability to speak, eat, work, sleep, and socialize [2]. Lack of access to dental care is a national issue for HIV patients; providing access is a challenge faced by many health centers and practices, including Zufall Health in New Jersey, a federally qualified health center (FQHC). In collaboration with Northeast/Caribbean AIDS Education and Training Center (NECA AETC), Zufall embarked on an initiative to improve access to quality dental care for PLWHA. AETC is the training component of the Ryan White Program, a federally funded program that provides medical care, support services, and medications for PLWHA who are low income, uninsured, or underserved [3-5]. Materials and Methods: In 2019, Zufall Health launched a quality improvement project to increase oral health access and services for Ryan White patients by integrating all partners involved in providing health care: medical, dental, psychological, behavioral, and HIV/AIDS providers and case managers. The project goal was to increase the number of patients receiving dental care to improve oral and overall health. Results/Observations: As a result of the project, there were significant quantitative and qualitative improvements in the oral health and quality of life of PLWHA: more dental encounters, a higher percentage of patients with a dental home, and the surpassing of the project goal of a 10% increase in dental referrals.

Clinical Manifestation of Oral Tuberculosis in HIV Patient: A Review Article HIV患者口腔结核的临床表现综述

Introduction: Tuberculosis (TB) is a disease that has become a global problem. the incidence of TB cases decreased by 9% from 2015 to 2019. In general, TB disease is divided into 2, namely latent TB and active TB. Latent TB indicates the presence of Mycobacterium Tuberculosis bacteria but is in an inactive/ inactive phase, but in this type of TB there is a 10% chance that these bacteria will be active. Meanwhile, active TB shows active bacterial activity so that it can infect other hosts. Objective: This literature study is to determine the clinical manifestations of tuberculosis in the oral cavity in HIV patients. Discussion: Mycobacterium tuberculosis is slowly replicating intracellular pathogen in macrophages that elicits a T cell immune response mediated by antigen-specific CD4+ and CD8+ T cells. This immune response may eliminate the Mycobacterium tuberculosis, but more frequently Mycobacterium tuberculosis persists in a latent form, constituting a reservoir of inactive Mycobacterium tuberculosis that under certain circumstances may become active. TB may occur at any stage of HIV disease and, as in the case presented here, may be the first indicator of HIV infection. HIV-seropositive subjects and particularly those HIV-seropositive subjects with low CD4+ T cell counts more frequently have extrapulmonary TB than do HIV-seropositive subjects or HIV-seropositive subjects with high CD4+ T cell counts. Conclusion: Patients who suffer from HIV may be associated with the tuberculosis infection. The TB infection may be known from the oral manifestation in HIV patients.

Management of Impacted Teeth in Orthodontics 正畸中埋伏牙的处理

Introduction and Purpose: Treatment of impacted teeth in orthodontics is crucially important, as clinician is supposed to manage such cases in his/her daily practice. However, impaction cases can be so complicated especially when other factors interfere within their treatments such as anatomical locations of the impacted teeth, surgical considerations and Orthodontic Traction difficulties. The management of impacted teeth in orthodontic practice varies widely from extraction of the impacted tooth to forced orthodontic eruption. Orthodontic eruption varies between closed or open techniques that must be determined for each case, per se. Materials and Methods: One hundred fifty one impacted teeth cases were followed up, classified by their techniques as to register the results statistically. The approaches for handling the impacted teeth were open forced eruption “as window or APF Apically Positioned Flap”, closed forced eruption, negligence and extraction when indicated. Bonding with Light Cure orthodontic composite was used for all cases of forced eruption. The used Device had been Mini LED, Acteron, Li-ION 3.7V, 2500mAh. Time of Exposure 20 Second. The traction attachments had been either Titanium Button and Chain (Watted) Dentaurum, or conventional Buttons (with ligature wires or elastics). This research discusses the prognosis of impacted teeth, the approaches of the orthodontic forced eruption and when to expose, extract or even “neglect”! The article does not recommend one treatment plan for all cases, in contrary; it urges the reader to search more for innovative solutions whenever such cases come across. Results and Conclusion: It is recommended that the decision regarding orthodontic forced eruption (closed or open), or extraction be based on evaluation of each independent case. In spite of the result of this research that 126 cases of the 151 screened cases had been treated successfully by orthodontic forced eruption, but complications such ankylosis, resorption, eruption failure and periodontal pockets need to be taken into account. Another complication can often be encountered that is bonding failures, especially in closed force eruption. However, the new bonding materials and the improved techniques have helped to overcome such a problem. Finally, the more precise the location and position of the impacted tooth is known, the easier the procedure becomes.

However, impaction cases can be so complicated especially when other factors interfere within their treatments such as anatomical locations of the impacted teeth, surgical considerations and Orthodontic Traction difficulties. The management of impacted teeth in orthodontic practice varies widely from extraction of the impacted tooth to forced orthodontic eruption. Orthodontic eruption varies between closed or open techniques that must be determined for each case, per se. Materials and Methods: One hundred fifty one impacted teeth cases were followed up, classified by their techniques as to register the results statistically. The approaches for handling the impacted teeth were open forced eruption “as window or APF Apically Positioned Flap”, closed forced eruption, negligence and extraction when indicated. Bonding with Light Cure orthodontic composite was used for all cases of forced eruption. The used Device had been Mini LED, Acteron, Li-ION 3.7V, 2500mAh. Time of Exposure 20 Second. The traction attachments had been either Titanium Button and Chain (Watted) Dentaurum, or conventional Buttons (with ligature wires or elastics). This research discusses the prognosis of impacted teeth, the approaches of the orthodontic forced eruption and when to expose, extract or even “neglect”! The article does not recommend one treatment plan for all cases, in contrary; it urges the reader to search more for innovative solutions whenever such cases come across. Results and Conclusion: It is recommended that the decision regarding orthodontic forced eruption (closed or open), or extraction be based on evaluation of each independent case. In spite of the result of this research that 126 cases of the 151 screened cases had been treated successfully by orthodontic forced eruption, but complications such ankylosis, resorption, eruption failure and periodontal pockets need to be taken into account. Another complication can often be encountered that is bonding failures, especially in closed force eruption. However, the new bonding materials and the improved techniques have helped to overcome such a problem. Implant placement in patient with history of bisphosphonate therapy is still controversial due to the risks of osteointegration failure and Biphosphonate related osteonecrosis of the jaws. Very few cases are reported. This paper aimed to report the success at 3 years follow up of 4 implants placed in a female patient with history of breast cancer and zoledronate infu-sions ( twice yearly for 2 years).

Frequencies of Failure In The Case of Ceramic Veneer Treatment and the Presence Of Composite Fillings: About A Clinical Trial 陶瓷贴面治疗的失败率和复合填充物的存在:关于一项临床试验

Introduction: The restoration of teeth with composite ceramic veneers has always been a subject of controversy: for some authors the presence of such restorations is a source of failure while for others not necessarily. Material and Method: Our study; is a randomized clinical trial comparing three types of crown preparations for bonded ceramic veneers. Our clinical trial which evaluated 264 bonded veneers in 73 patients included 235 living teeth and 29 mortified teeth that underwent endodontic treatment. Among the secondary objectives of this study: the evaluation of the durability of ceramic veneers bonded to teeth with composite restorations. Results: The Chi-square test of the occurrence of events according to the unblocked / blocked character gives a p = 0.05 (p ≤ 0.05) so the difference is significant. Conclusion: The presence of composite fillings affected the survival rate of veneers in our study

Surgical and Orthodontic Treatment of Impaction Maxillary Central Incisor Associated With Supernumerary 上颌中切牙并发多牙嵌塞的外科及正畸治疗

Impaction of maxillary central incisors is frequent in dental practice. However its management is challenging because of its importance in aesthetics. Supernumerary teeth are the most common dental anomaly that can cause impaction of adjacent teeth, crowding, diastema, rotation and displacement of teeth. We report a case of 13 year old male with an impacted supernumerary tooth in the maxillary anterior region, which was interfering with the eruption of the permanent, left central incisor. The impacted supernumerary tooth was surgically removed. With the application of an orthodontic traction, impacted left maxillary central incisor was brought down to its proper position in the dental arch.

Efficacy of Surgical Laser Diode Therapy in the Management of Labial and Lingual Frenectomy 外科激光二极管治疗唇舌系带切除术的疗效

A frenum, a triangle-shaped fold of tissue connects the lip, tongue and buccal musculature to the alveolar bone in the oral cavity. Any abnormalities in the size and location of the frenum can cause functional and esthetic problems which require surgical excision. Frenectomy can be accomplished either by the routine scalpel technique, or by using lasers such diode laser. The 1st case is an 8-year-old healthy boy with the chief complaint of spacing between his upper front teeth and a palatoversion of the left central incisor. Intraoral clinical examination revealed the presence of midline diastema (1 mm) with high, fibrous labial frenal attachment. The 2nd case is a 6-year-old boy consulted with the complaint of difficulty in speech associated to the limitations of the lingual movements. Intraoral examination revealed partial ankyloglossia. The 2 cases, the frenectomy was then made with 980 nm diode laser.
