In vivo Evaluation of the Hemostatic and Cicatrising Activities of a Gel Based on Vernonia Conferta Benth. On Post-extraction Wounds in Wistar Rats 水龙花凝胶止血和瘢痕活性的体内评价。Wistar大鼠拔牙后伤口的研究

Introduction: Despite the progress in oral surgical techniques, the occurrence of post operative haemorrhage can lead to delayed healing. It is with this in mind that we proposed to explore other therapeutic avenues to try to provide other solutions through traditional African medicine. The Aim of the Study: To evaluate the haemostatic and healing activity of a Vernonia conferta-based gel on postextractional wounds induced in Wistar rats. Methodology: An experimental study was conducted over a period of eight months during 2021 at the multidisciplinary laboratory of Galenic Pharmacy and Pharmaceutical Legislation of the Faculty of Medicine and Biomedical Sciences of the University of Yaoundé 1. A gel formulation test carried out in standard proportions for a 2% concentration such as the Elugel reference gel used as a positive control. To evaluate the haemostatic and healing activity, we made up three batches of five female wistar rats with a body mass of between 100 and 130 grams, acclimatised for three days before the experiment. This test consisted of performing an avulsion of the central incisor in the rat and then applying our active principle for 5 successive days on one of the batches and our reference gel on the other batch, while the last batch will be under observation receiving no treatment and considered as a negative control. The observation was carried out during 14 days while respecting the feeding conditions of the animals. The haemostatic activity was evaluated using a stopwatch or we took the bleeding time after tooth extraction in the constituted batches. The healing activity was evaluated by taking a tissue sample from the extraction site on days 3, 7 and 14 for histological analysis. Results: The aqueous extract-based gel, made in standard proportions at a 2% concentration like the reference gel used as a positive control, stopped bleeding almost instantly, and clot formation occurred within 1-3 minutes after application. As for the healing activity, the batch of rats that received our test gel showed a rapid re-epithelialization observed on day 14ème, testifying to the restitution of the tissue injured during avulsion. Conclusion: The adhesion of Vernonia conferta aqueous leaf gel to the post-extraction alveolar mucosa in vivo in wistar rats has a favourable profile, as it allows faster haemostasis and accelerated healing.

Effectiveness of Analgesics in the Treatment of Temporomandibular Dysfunction: A Systematic Literature Review 镇痛药治疗颞下颌功能障碍的有效性:系统文献综述

Introduction: Temporo-mandibular dysfunctions correspond to a defect adaptation of the manducatory apparatus. Different therapeutic methods are available for the treatment of these pathologies: Splints, physical therapies, occlusal therapies, surgery or pharmacological treatments. The objective of our work is to analyze the clinical effectiveness of analgesics in the treatment of disorders of the masticatory apparatus by applying the Evidence approach Based Dentistry. Methods: The collection of data from the literature scientific was carried out among the production that appeared during the period from 2011 to 2021. The documentary research was carried out using accessible databases via the Internet namely MEDLINE, ELSEVIER and Cochrane Database of Systematic through their PubMed, Science Direct and Cochrane library search engines, as well as the website Google Scholar. A bottom-up manual search covering the same study period was carried out in order to enrich our bibliography. The writing of this systematic review followed the guidelines of the PRISMA Statement (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Results: 9 relevant randomized clinical trials were selected after critical reading guided by the CASP type reading grids. The items have been listed in 4 categories according to the route of administration of the analgesics (topical route, oral, intramuscular route and intra-articular route) and classified in tables according to the PICO criteria (Participants/Interventions/Comparison/Results “Outcomes”). The results of our study show a significant reduction in the intensity of the pain and the rates of cytokines as well as a significant increase in maximum mouth opening. Conclusion: The use of analgesics increases the efficacy of treatment by decreasing pain intensity and cytokine levels, through both their analgesic and anti-inflammatory actions. This efficacy is similar for both levels II and III. However, additional long-term clinical studies with a larger sample size are needed to confirm the data collected and to further explore the effect of analgesics in the treatment of TMDs.

The Dentists of the Family Health Strategy: Study on the Taste Related to their Practices 牙医的家庭健康策略:与牙医实践有关的口味研究

This study aimed to characterize origin and social trajectory as well as the taste relative to the professional practices of dentists working in the ESF of the municipal network of Salvador-BA. This is a cross-sectional, quantitative and exploratory study involving dentists who worked at the ESF in Salvador-BA. Data collection was carried out through structured questionnaires. Simple, relative frequencies were obtained and the Chi-square test was performed. 72 dentists who worked in the FHS participated in the study. There was a predominance of female dentists (80.28%) and with an average age of 39.2 years. As for social origin, 13.46% had a popular social origin, and 76.79% had an upward trend. About what they most like to do in the clinic, 58.82% answered procedures involving the restorative clinic. The taste for surgical clinic seems to have a correlation with popular social origin (28.57%). The study confirmed a phenomenon already described in the literature as feminization. Most dentists have an upward trend, which points to a rise in accumulated school capital. The taste was preferably for the restorative clinic. Therefore, the taste of professionals of popular origin for the surgical clinic can be the expression of a class habitus. This study points to a relationship between the taste for surgical clinic among professionals of popular origin.

Dental Enamel Alterations in Preschool Children from Salvador-Ba: Occurrence and Associated Factors 萨尔瓦多-巴学龄前儿童牙釉质改变:发生及相关因素

Objective: To describe the prevalence and distribution of Developmental Defects of Enamel (DDE) in children aged 3 to 5 years old in the city of Salvador-BA-Brazil, and to identify potential associated factors, considering socioeconomic, pregnancy, care characteristics childbirth and child behavior. Methodology: This cross-sectional study investigated the DDE in children aged 3 to 5 years, who attended municipal public daycare centers and Family Health Units, in 2018. The oral examination was carried out and it was applied a questionnaire to the respondent about characteristics of children and families. Descriptive and multivariate regression analyzes were performed, assuming a confidence level of 95%. Results: 1591 preschoolers were examined, with a mean age of 54 months. Most children were male (50.41%) and had brown or black skin color (93.34%). The prevalence of DDE was 9.24%. In the multivariate analysis, it was found that the male sex had a negative association with the DDE, while the age greater than or equal to 54 months and the presence of caries in the primary dentition showed a positive association with the DDE. Conclusion: DDEs are shown to be an important oral disease among preschoolage children to be monitored in the city of Salvador-BA, and strongly associated with dental caries.

Background: Bibliometric is the quantitative analysis of the characteristics of documents published by researchers. Aim: The aim of this study is to make a bibliometric analysis of publications of Arab countries’ universities in dentistry between January 2010 and December 2020 based on publications appeared in the indexed journals on PubMed. Materials and methods: Our work is an exhaustive retrospective descriptive study. Results: Out of 22 Arab countries, 17 have at least one university of dentistry. Our study showed the following results: - 1337 articles were published. - 59 universities from 15 countries published, which represents 32.2%. - 433 professors have published at least one article, which represents 15.58%. - Saudi Arabia is in the lead with 524 articles, which represents 39.19%. - The Saudi Pr. Sadeq Ali Al Maweri occupies the 1st place with 27 articles. - The Moroccan Bourzgui Farid and Ousehal Lahcen from Hassan II University occupy the 11th place with 8 articles. - The contribution of Moroccan authors is 4.11%, occupying the 8th position among Arab countries. - Descriptive cross-sectional studies are at the top of the ranking with 30.96% of articles. - Oral surgery is at the top of the ranking with 30.66% of articles. Conclusion: The results obtained showed that the rate of scientific production in dentistry at Arab dental universities has increased over the years; however, it is still far from that of European and American universities. The infrastructure of research in the Arab world requires efforts from all those responsible.

The Effect of Volatile Oils on Debonding of Polycarbonate Brackets Reinforced with Ceramic Filler: A Qualitative Study 挥发油对陶瓷填料增强聚碳酸酯支架脱粘效果的定性研究

Introduction: An effective debonding agent should be capable of reducing not only the debonding force, but also the amount of adhesive remaining with no enamel or bracket fracture. Aim: The aim of the present in vitro study was to evaluate the amount of the adhesive remnants, enamel fracture as well as bracket fracture after application of different volatile oils at different time of application using ARI system. Material and Method: Polycarbonate brackets reinforced with ceramic filler were bonded to one hundred and eighty (180) sound extracted human premolars using Transbond XT light curing adhesive. The teeth were then distributed into five groups. Teeth were then de-bonded using the Instron Universal Testing Machine. The enamel surface of debonded teeth and their bracket bases were examined under the stereomicroscope SEM for adhesive remnants (ARI), enamel fracture and bracket fracture. Result: At 5 minutes of application, no significant difference was noticed among volatile oils in all the qualitative results including; adhesive remnants, enamel fracture and bracket fracture. At 5 minutes of application, the peppermint volatile oil exhibited the highest percentage (55%) of score 1 whereas the clove oil revealed the least percentage (15%). At 30 minutes of application, the black seed exhibited the least percentage of score 1 and the highest percentage of score 2 (75%). At 5 minutes, the peppermint oil showed the least brackets fracture (20%) whereas the clove oil was in the second rank (25%) and the mixture of peppermint oil with black seed oil revealed the highest (55%). At 30 minutes, the clove oil showed the least bracket fracture (5%) whereas peppermint oil was in the second rank (20%) and the black seed oil depicted the highest percentage (45%). Conclusion: The present study supported that the 5- minutes of peppermint as well as the black seed volatile oil applications can be considered the best debonding agents whereas the clove oil can be used as a medication or mouthwash before debonding. The latter approach worth further investigations.

Clinical Significance of Oral Health Related Quality of Life for Oral Cancer Patients: A Systematic Review 口腔癌患者口腔健康相关生活质量的临床意义:系统综述

Background: Oral cancer is relevant not only to medical practitioners but also to dentists because they have a responsibility in the early detection and prevention in the early stages of disease. Dentists are also responsible for supportive care and prosthetic rehabilitation of oral cancer patients undergoing treatment. However, more work is required on the clinical significance of quality of life for oral cancer patients. Few reviews have been conducted on the clinical significance of oral health related quality of life in oral cancer patients and hence there is minimal guidance on the support these patients need. Methods: An electronic search was conducted on Google Scholar, PubMed, Biomedical central, JSTOR and Oxford Academic databases between October 2020 and November 2021 using the keywords “oral cancer”, “oral health related quality of life” and “clinical significance”. The search was limited to dental journals and MEDLINE and the information was extracted using the PRISMA guidelines. Results: Nine articles out of eighty-eight were found to be most relevant and were included in the analysis. Surgery has had significantly (p<0.05) positive effects on the oral health related quality of life of oral cancer patients than patients who received radiotherapy. However, microvascular surgery seemed to negatively affect the quality of life than local reconstruction. Xerostomia was worst in patients undergoing head and neck radiotherapy, then those patients who only had oral surgery. Conclusion: Evidence suggests that oral cancer patients’ experience a negative impact to health-related quality of life especially in the first three months but the quality of life improves thereafter almost five years after oncological treatment.

Minimally Invasive Aesthetic Crown Lengthening: A Piezoelectric Assisted Case Series with 4-Year Follow-Up 微创美观冠延长:压电辅助病例系列与4年随访

Aim: This retrospective study aims to reveal the clinical outcome at 48 months of a minimally invasive crown lengthening technique performing an intrasurcular flapless osteotomy, using a piezoelectric tip designed for this purpose. Material and Methods: All subjects in the current case series required esthetic crown lengthening procedures in the anterior esthetic zone of the maxilla. Prosthetic planning was carried out, which included a digital waxup of the ideal tooth morphology, which allowed constructing a surgical guide to transfer the proposed gingival margins to the patient. Gingivectomy was performed by using an electrosurgery system Perfect TCS IITM (Coltene Whaledent) and osteotomy by using a piezoelectric insert PiezoSmileTM (BROK) until achieving a bone crestgingival margin distance similar to the presurgical. Results: 57 patients were followed postoperatively for 48 months. During the post-operative period, healing, complications, periodontal and aesthetic health parameters including the position of the gingival margin were observed. WES, PES and RES indexes showed an optimal integration with the restorative materials and a good aesthetic result. Conclusion: Minimally invasive crown lengthening technique with electrosurgery and a piezoelectric insert is effective in repositioning the gingival margin in a stable and predictable manner within a 48 months follow-up.

Malocclusion consists of growth and development abnormalities in the relationship between dental arches and / or jaw bones. It usually has a high prevalence in children of preschool age, so it is considered a public health problem. The present study aims to describe the occurrence of malocclusion among preschoolers in Salvador-BA, as well as, to exploratively identify potential factors associated with this problem. This is an epidemiological cross- sectional study involving a random sample of 1577 children in the pre-school age group (36 to 71 months) who attended municipal public daycare centers in Salvador-BA. The study population had a mean age of 54 months and was predominantly composed of blacks / browns (92.02%) and male children (50.29%). The prevalence of malocclusion was 40.46% and the most prevalent condition was open bite (approximately 14%). Digital sucking was performed by 6.73% of the population and 10.39% used pacifiers. Most children (69.43%) do not have the habit of biting objects, and 26.14% gnash their teeth. The protective factors were breastfeeding age ≥ 12 months (PR = 0.63; CI = 0.48- 0.82) and the habit of grinding teeth (PR = 0.7; CI = 0.52- 0.95); and risk factors: digital suction (PR = 5.24; CI = 3.26- 8.41), pacifier (PR = 2.13; CI = 1.27- 3.57), and low insertion of labial frenum (PR = 1.65; CI = 1.15-2.37) Educational measures with family and guardians are essential to reduce prevalence of the disease.
