A Comparative Study of Two Resonance Frequency Analysis Instruments 两种共振频率分析仪器的比较研究

Primary implant stability (PIS) depends on the surgical technique, implant design, and the characteristics of the recipient bone, among other factors. This paper aims to study different parameters (recipient bone characteristics like bone density (BD) and mean alveolar crest width (MACW)) that may be related to implant primary stability outcomes. With that, it can be determined if they are reliable PIS methods and guide treatment decisions for successful implant outcomes. Thirty-five dental implants (25 BTI Interna® CORE and 10 Bioner Top DM) were placed in 10 patients in 2022 and 2023. All of them underwent Cone Beam Computed Tomography (CBCT) scans to obtain values for MACW and BD. PIS measurements used were implant stability quotient (ISQ) obtained through different resonance frequency analysis (RFA) methods (Osstell® ISQ and Penguin RFA®) and insertion torque (IT). The results for the following variables showed significance at p<0.001 and p<0.05. Highly significant relationships were observed between Osstell and Penguin RFA values (p<0.001). Regarding IT, a direct proportional relationship with ISQ was observed (p<0.05). PIS, as measured by ISQ and IT, did not show a significant correlation with the mean alveolar crest width of the edentulous ridge measured by CBCT, suggesting that wide or narrow alveolar crests did not have an effect on rotational or lateral PIS in both implant groups. The most significant predictor of lateral and rotational PIS in our patients was the Hounsfield Units (HU) value 0.5 mm away from the implant placement area (p<0.001) and (p<0.05), respectively. In conclusion, non-invasive PIS methods could be considered as a guide for clinicians in treatment decision-making and implant success. However, further research is needed to predict PIS.

Cephalometric Determination of the Vertical Dimension of Occlusion: Clinical Study 咬合垂直尺寸的头颅测量测定:临床研究

The following is a retrospective descriptive cross-sectional stud aimed at determining the vertical cephalometric standards of a Moroccan population. The values found will be used to orient the choice of the VDO during complex prosthetic rehabilitations where all dental references are absent. The study was conducted on 80 teleradiographies of subjects consulting the ODF department of the CCTD Casablanca with a skeletal class 1 dental occlusion in the absence of any previous or current orthodontic treatment. After performing the cephalometric tracings, we correlated the VDO with 7 angular measurements: ENA-Xi-Pm, GoGn/SN, Occ/SN, Occ/Fr, SGn/Fr, FMA, GoGn/GoENA. Our study led us to the following results: • For the ENA-Xi-Pm angle, we found a mean value of 47.9. • The GoGn/SN angle averaged 38.3°, it varied in 37.29.° • For the Occ/SN angle, it was found that the average value was 21.2° for the whole sample. • The Occ/Fr angle was on average 9.9.° • The SGn/Fr angle averaged 60° in the Moroccan population. • The measurement of the FMA angle showed an average of 27. • Finally, for the GoGn/GoENA angle, we found a mean value of 50. The comparison of our results with those of studies conducted on different populations showed significant differences. This can be explained by the tendency to hyperdivergence in the Moroccan population, hence the interest in using the mean values specific to each population. The comparison of the global means with the means of each group showed that the angular values in hyperdivergent cases are significantly higher than the global values and the angular values in hypodivergent cases are significantly lower than the global values, and for the normodivergent cases, we found a significant difference only for the values of the FMA angle which is lower than the global value of the sample. And thus, the determination of the VDO using cephalometric analysis represents one of the best methods to correctly restore the occlusal environment in a complex prosthetic project, given that the restauration is based on cephalometric standards of the corresponding population.

Bjork-Jarabak Analysis in Different Anteroposterior Skeletal Relationship: A Comparative Study 不同前后骨骼关系中的 Bjork-Jarabak 分析:比较研究

Aims: This study aimed to evaluate cranial base measurements in individuals with different skeletal relationships (class I, II, and III) using Bjork-Jarabak analysis and compare them with existing research. Material and Method: Lateral cephalograms of 103 Sudanese participants (aged 18-22) were assessed, grouped by ANB angle. Results: Showed no significant differences between class I and II, or class I and III skeletal relationships. However, class II and III relationships exhibited significant differences in Articulare angle, Gonial angle, ramal height, and mandibular length (P<0.0001). Discrepancies in craniofacial morphology were observed between Sudanese and Bjork-Jarabak norms for Caucasian. Conclusion: Saddle angle, Bjork sum of angles, and cranial case showed no significant differences among skeletal classes, but significant differences in several measurements existed between class II and III relationships.

Management of Edentulous Patients with Maxillary Substance Loss: A Case Report 对上颌骨实质缺失的无牙颌患者的管理:病例报告

Depending on the location and extent of the loss of substance following maxillary resection surgery, all functions of the masticatory apparatus may be adversely affected. Eating and communication are impaired, which has a negative impact on the patient's psychological state [1]. When reconstructive maxillofacial surgery is not possible, rehabilitation with a maxillofacial prosthesis is essential. It must restore functions (phonation, eating, breathing, etc.) and reintegrate the patient into his psychosocial environment by masking his disability [2]. To achieve these objectives, this prosthesis, like a conventional removable prosthesis, must meet the Housset triad (support - retention - stabilisation), but also the objective of tightness. We therefore speak of a "prosthetic tetrad in PMF" (Housset's triad completed by the objective of tightness) [3]. In the case of a completely edentulous arch with substance loss, the treatment becomes more complex due to the reduced support surface. The maxillofacial prosthodontist must therefore be extremely vigilant to ensure the success of this appliance. A wise choice of technique and impression material, as well as respect for the occlusal parameters (occlusal plane, vertical dimension, centric relation, etc.) will ensure prosthetic stability and integration [4]. In this article, we will focus on the peculiarities of the management and the different stages in the fabrication of a maxillofacial prosthesis for a patient with total edentulism and loss of maxillary substance.

Correction of Anterior Crossbite, Using Removable Appliances in Mixed Dentition: 5-Year Follow-Up 使用活动矫治器矫正混合牙列的前交叉咬合:5 年随访

The anterior crossbite (ACB) is a malocclusion where the upper anterior teeth are occluding lingually with respect to the lower anterior teeth. Moyers describes anterior crossbites as dental malocclusions resulting from abnormal axial inclinations of the anterior teeth. This type of malocclusion can be dental, functional or skeletal and can occur in the primary, mixed and second dentition. Diagnosis is made using diagnostic aids. While the treatment was carried out through interceptive orthodontics, with the use of different removable appliances. The objective of the study is to present the diagnosis and treatment for 5 years with the use of removable appliances. The results were favorable, being able to uncross the bite in 4 months, it was maintained throughout the dental replacement. One of the conclusions reached is that the treatment time is slightly longer since the use of preventive and interceptive orthodontics takes more time because these treatments are performed during the growth of the patients.

The flabby ridge is commonly observed in the anterior region of the maxillary arch in fully edentulous patients. It is characterised by non-adherent hypertrophic mucosal lesions overlying the underlying bone, often with excessive bone resorption. Management of the flabby ridge is essential prior to the placement of a definitive prosthesis, whether by surgical resection or the use of specific prosthetic methods. This management aims to ensure stability and optimal retention of the prosthesis. In order to determine the type of intervention to be chosen, a thorough interview and clinical examination are carried out. Several parameters are considered, such as the volume of the flabby ridge, the degree of underlying bone resorption, the type of prosthetic rehabilitation planned and the presence of any contraindications to surgery. These factors will guide the decision on the most appropriate treatment. In this article, we focus on a non-surgical approach to the management of the flabby ridge, which involves the selection of a specific type of impression. This impression technique aims to record the ridge in its static state, without compression or displacement, while ensuring precise muco-dynamic recording of the adjacent tissues.

Unveiling Traumatic Facial Wounds: A Comprehensive Study on Epidemiology, Diagnosis, and Therapeutic Approaches 揭开面部创伤的面纱:流行病学、诊断和治疗方法综合研究

Introduction: Dilapidated facial wounds are shredded injuries with loss of tissue substance. A surgical emergency characterized by complex anatomical clinic types that may expose vital organs. The objective of this study is to investigate their epidemiological, diagnostic, and therapeutic aspects. Material and method: This epidemiological, retrospective, descriptive, and cross-sectional study was conducted using patient records from individuals who experienced maxillofacial trauma resulting in decaying facial wounds between March 2017 and June 2021 at the Maxillofacial Surgery Department in Libreville, Gabon. All confirmed cases of traumatic dilapidated facial wounds were included, while patients with non-traumatic wounds were excluded. The analyzed parameters encompassed sex, age, etiology, clinical signs, and treatment. The criteria for patient assessment of the plasty outcomes were categorized as very satisfactory, satisfactory, not satisfactory, and disappointed. Results: A total of 33 cases were included in the study. The male-to-female sex ratio was 3.1:1. The average age of the patients was 29.8 ± 14.9 years. The majority of cases (36.4%) were attributed to road traffic accidents (AVP). Among the affected areas of the face, the lips were most frequently involved. Local plasty was performed in 24 patients, while 9 patients underwent regional plasty. The treatment outcomes were deemed satisfactory by 75.8% of the patients. However, 21.2% of the patients experienced functional sequelae as a result of the injuries. Conclusion: Young males are commonly afflicted by severe lesions, often leading to bone fractures. The application of local plasty has proven to yield favorable results in effectively managing these complex injuries.

Determinants of Dental Care Utilization, Unmet Dental Care Need, and Barriers among Women of Reproductive Age in the United States 美国育龄妇女牙科保健利用率、未满足的牙科保健需求和障碍的决定因素

Introduction: Dental care is integral to general health but is often isolated from the healthcare system nor discussed as part of overall health. The barriers to accessing needed dental care vary among socioeconomic and demographic groups. This study examined the disparities in dental care access and identified the socioeconomic and demographic factors associated with dental health needs among United States women. Methods: Using data from the 2017-2020 National Health and Nutrition Examination Surveys (NHANES), we conducted a Chi-square to assess the differences in the proportions of women who reported not getting their needed dental care and computed a weighted multivariate logistic regression to examine the factors associated with access to dental care use, unmet dental-care need, and reported reasons for unmet need. Results: Race, income, and education played a significant role in surveyed participants regarding dental care use and unmet dental care needs. Non-Hispanic Blacks and other minority racial-ethnic groups, including multi-racial groups, were likelier to have never used dental care than non-Hispanic Whites. Furthermore, non-Hispanic Whites with less than a high school education were likelier to have never used dental care than those with a high school education. Groups more likely to report an unmet need were women with PIR < 1.00. Moreover, between 1.00 and 1.99 compared to PIR ≥ 2.00 and women without health insurance compared to those with health insurance. Conclusion: Expanding insurance coverage for dental care and improving access for women with poor health may improve racial-ethnic and education-level disparities, specifically in African American women's unmet dental care needs.

The New Modified Mono-Block (MMBA) has demonstrated effectiveness and versatility in treating OSA patients and boasts its advantages over previously used devices. It is essential to seek medical advice if experiencing any symptoms of sleep apnea, and long-term treatment use can significantly improve symptoms and prevent potential complications.
