Objective: To analyze the therapeutic effect of costochondral graft (CCG) in constructing nasal tip cartilage composite for rhinoplasty. Methods: 84 rhinoplasty patients who were admitted to the hospital between September 2021 and September 2023 were selected and randomly grouped into a study group and a reference group of 42 cases each. The study group was prepared with CCG to construct a nasal tip cartilage composite and the reference group was filled with silicone material to compare the effectiveness of plastic surgery. Results: The plastic effect scores and the perioperative indexes of the study group were all higher than those of the reference group (P < 0.05). Before treatment, there was no difference in the nose-shaping indexes between both groups (P > 0.05). After treatment, the nasal plasticity indexes of the study group were all better than those of the reference group (P < 0.05). The complication rate of the study group was lower than that of the reference group (P < 0.05). Conclusion: The preparation of nasal tip cartilage composite using CCG improved the therapeutic effect of rhinoplasty, improved the perioperative indexes and nasal contouring indexes, and had fewer complications.

Cryptococcus neoformans can cause detrimental cryptococcosis and other severe complications, especially in immunocompromised populations. Disseminated cryptococcosis in organ transplantation patients is extremely rare and often involves lungs, brains, skin, nails, and other organs. In this paper, we report a rare disseminated cryptococcosis case involving skin and lung that presented with recurrent dark brown abscesses and nodules in an immunocompromised female after kidney transplantation. The K-Set detection result of Cryptococcus capsular polysaccharide in serum is positive. The identification of pus culture by Autof MS 1000 indicates that the patient was infected with Cryptococcus neoformans var. grubii. In addition to treatment using liposomal amphotericin B, fluorocytosine, and fluconazole, local extraction of pus and simultaneous injection of liposomal amphotericin B into skin lesions were performed. This comprehensive treatment method proved effective, as the patient recovered from fever and skin symptoms. The innovative treatment may contribute to helping more disseminated cryptococcosis patients.

Lichen myxedema (LM) represents a group of rare skin diseases, which falls under chronic mucinoses. LM, previously described as localized scleromyxedema, however, is not associated with systemic involvement. Among the subtypes, atypical LM is rare, with few reported cases associated with multiple myeloma (MM). We present a case of MM, positive for lambda chains, with acute-onset clinical symptoms, and diagnosed with atypical LM. The patient was managed with topical corticosteroid with improvement of lesions after one month of treatment.

Background: Atopic dermatitis is a chronic, inflammatory, and pruritic dermatosis of high prevalence, especially in childhood. a relevant public health issue. Atopic dermatitis is associated with several factors, including genetic, psychological, infectious, food, and environmental, and may be associated with other manifestations of atopy, such as asthma and rhinitis. Objective: To determine the epidemiological profile and the prevalence of personal and family history of allergy in children with atopic dermatitis at a referral service, comparing it to existing literature. Methods: We conducted a cross-sectional and descriptive hospital-based study involving children of both genders under 18 years with clinical diagnosis of atopic dermatitis (Hanifin and Rajka criteria) seen at the Pediatric Dermatology Outpatient Clinic of Hospital Municipal Universitário de Taubaté (HMUT), SP-Brazil, from October 2018 to April 2019. Epidemiological data and data related to personal and family history of allergies were collected from the medical records. Microsoft Excel 2019 was used for compilation and data analysis. Results: Of the 440 consultations in that period, 35 (7.9%) were for atopic dermatitis, of which 23 of them were females (65.7%) and 15 were phototype IV (42.9%); their mean age was 7.7 (standard deviation = 4.3), and the duration of disease ranged from 2 months to 14 years (mean 5.3 years; standard deviation = 4.3). Among the 35 patients, 31 (88.6%) manifested the condition by the fifth year of life. Personal history of allergies was observed in 27 individuals (77.1%), with a predominance of allergic rhinitis and in older age groups. Conclusion: This study, as a pioneer in the region, contributes to the epidemiological profile of patients with atopic dermatitis from the Pediatric Dermatology Outpatient Clinic of HMUT, revealing an early onset and a higher prevalence in females and individuals with higher phototypes. In addition, this study also demonstrated the prevalence of personal and family history of allergies in these patients, consistent with literature.

Omalizumab (Xolair®) was approved for the treatment of chronic idiopathic urticaria in 2017. Furthermore, consistent with international guidelines published in 2018, the Urticaria Clinical Practice Guidelines by the Japanese Dermatological Association provide detailed instructions for the use of omalizumab. However, there are no reports summarizing the efficacy of omalizumab in chronic idiopathic urticaria in Japan. The subjects of our study were 105 patients treated with omalizumab 300 mg/month for chronic idiopathic urticaria at Tokyo Medical University Hospital from June 2017 to March 2021, inclusive of 23 male and 82 female patients, with a mean age of 45.4 ± 16.0 and disease duration of 46.9 ± 99.4 months. Three months after treatment, omalizumab was discontinued in 63 patients with a Urticaria Control Test score of 16. The mean number of dose completions was 6.4 ± 4.1. There were nine cases of relapse in which seven of the nine cases discontinued omalizumab within six doses. Most of the relapsed cases improved with re-administration of omalizumab, but it was again discontinued in five cases. Omalizumab has lesser side effects than steroids and cyclosporine, and the criteria for its use were revised in 2018, requiring an allergist or dermatologist to be available to treat adverse events such as asthma and anaphylaxis at the facility or in collaboration with neighboring medical institutions. Omalizumab can also be used in dermatology clinics. The drug is an effective treatment for severe chronic idiopathic urticaria and is anticipated to be used by more dermatologists.

The drug is an effective treatment for severe chronic idiopathic urticaria and is anticipated to be used by more dermatologists.","PeriodicalId":299678,"journal":{"name":"Dermatological Health","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-06-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121772486","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Malignant fibrous histiocytoma is the most common soft tissue sarcoma in adults but is rare as a primary cutaneous tumor. It has a male predilection with a peak incidence between the fifth and sixth decades of life. Malignant fibrous histiocytoma predominantly occurs in the extremities. We report a case of malignant fibrous histiocytoma presenting in the hallux of the right foot.

Background: Psoriasis is a chronic recurrent inflammatory dermatosis occurring in genetically predisposed individuals. Its dramatic nature of the lesions, successive flare-ups, and skin discomfort may affect the quality of life of patients and result in a depressive tendency. Objective: To assess the impact of psoriasis on the quality of life of patients and the relationship between disease severity and quality of life. Methods: A descriptive cross-sectional study was carried out in Bamako from September 3, 2018 to August 30, 2019. The Dermatology Life Quality Index (DLQI) and Psoriasis Area Severity Index (PASI) were used to assess quality of life and factors of disease severity. Inclusion was based on clinical and histopathologic criteria. Questionable cases were excluded from the study. Results: A total of 106 cases of psoriasis were identified out of 24,000 consultations, i.e., a hospital frequency of 0.44%. Of these cases, 54 were included. Males accounted for 70% of the cases, and the average age was 37. The distribution of the social repercussions of the disease was as follows: disability (68.52%), non-participation in ceremonies (20.37%), stigmatization (18.52%), isolation (12.96%), work stoppage (11.11%), and non-sharing of meals (9.26%). However, the reception by caregivers was satisfactory in 96.30% of cases, and 85% of patients scored ≤ 10 on DLQI, suggesting that they had good quality of life. Among the patients with psoriasis, 35.04%, 12.96%, and 50.00% had mild, moderate, and severe disease, respectively. Conclusion: Quality of life is multifactorial and has no direct association with the severity of psoriasis. Dermatologists must take into account the dimensions of symptoms and psychological state when managing patients with psoriasis.
