Unified Denovo Multipurpose Analyzer 统一的 Denovo 多用途分析仪

Tooth movement is affected by various factors like magnitude, direction, and the duration of the forces applied; the direction of force plays the most pivotal part. Orthopedic appliances have long been used to apply extraoral traction forces to entirely limit or redirect the growth of the maxilla and mandible. Here, we present a very simple and efficient device. The multipurpose analyzer (MPA) is used to locate the center of resistance of maxilla and maxillary dentition, giving a more accurate direction of force vector in facemask and headgear cases. It is also used as chairside device clinically to measure facial heights, nasolabial angle, mentolabial angle, and mandibular plane (MP), as well as to measure cephalometrically MP, Frankfort horizontal (FH)–MP, palatal plane (PP)–MP, occlusal plane (OP), and MP during analysis.

Accuracy of 3D Printing in Orthodontics: A Systematic Review and Meta-analysis 三维打印在牙齿矫正中的准确性:系统回顾与元分析

To assess and compare the accuracy of 3D-printed digital models, orthognathic surgical splints, retainers or aligners, and implant surgical guides printed using various 3D printing technologies. The search comprised prospective and retrospective studies related to the accuracy of 3D-printed models, splints, implant surgical guides, and retainers/aligners in patients undergoing orthodontic and orthognathic surgical procedures. The outcomes were assessed in terms of linear measurements, degree of fit, and positional deviations in comparison to conventional plaster models and surgical acrylic splints. The methodologic quality of the articles and the level of evidence were assessed using the QUADAS-2 tool. A meta-analysis was performed to compare the accuracy of models printed by different technologies to plaster models using the random-effects model. Twenty-one retrospective studies were included. Quality assessment of all included articles showed moderate risk of bias and no article was excluded. The systematic review showed that there were no significant differences between printed surgical splints, retainers/aligners compared, and the control group. Meta-analysis of six eligible studies showed that printed models had a general trend toward overestimation of linear measurements (mean difference = 0.22 mm; 95% confidence interval = – 0.09 to 0.36 mm, I 2 = 85%). Measurements made on digital light processing-printed models were significantly different than those made on plaster models. The mean difference of 0.22 mm in linear measurements made on 3D-printed models and plaster models was clinically acceptable. Among the various printing technologies, PolyJet and fused deposition modeling-printed models showed higher accuracy. The printed splints and retainers/aligners were as accurate and reliable as their respective gold standards. The findings should be interpreted with caution due to high level of heterogeneity. Hard to Soft Tissue Profile Changes in Skeletal Class III Dentofacial Deformities After Maxillary Advancement and Mandibular Setback Orthognathic Surgery 上颌前突和下颌后缩正颌外科手术后 III 类畸齿的硬软组织轮廓变化

To correlate the sagittal hard tissue movement and soft tissue changes among patients with skeletal class III malocclusion following bimaxillary orthognathic surgery. Fifty-nine subjects with skeletal class III malocclusion treated consecutively with maxillary advancement and mandibular setback were assessed for skeletal and soft tissue linear changes using presurgical (T1) and post-treatment (T2) lateral cephalograms. Pearson’s correlation coefficient test was used to correlate hard and soft tissue parameters. The sagittal hard tissue movement and soft tissue changes for all the parameters assessed in maxilla and mandible showed statistically significant correlations. The ratios of changes for the maxillary protrusion [N-A (HP)] to horizontal subnasale (Sn), superior labial sulcus (Sls), and labrale superioris (Ls) were 0.574:1, 0.703:1, and 0.721:1 respectively. The ratio of horizontal labrale inferioris (Li), mentolabial fold (Mlf), and soft tissue (Pg′) to mandibular protrusion [N-B (HP)] were 0.992:1, 0.970:1, and 0.590:1 respectively and to chin protrusion [N-Pg (HP)] were 1.293:1, 1.252:1, and 1.001:1, respectively. The ratios were different for males and females in maxilla and mandible. Sagittal hard tissue movement and soft tissue changes are significantly correlated. The soft tissue movement in the mandible were more proportionate compared to the maxilla. The sagittal hard tissue to soft tissues movement ratios were greater in maxilla and lesser at pogonion and mentolabial sulcus in females compared to males. The ratios obtained can be used for predicting postsurgical soft tissue changes.

To Evaluate the Relationship between Maxillary and Mandibular Base Lengths with Dental Crowding in Different Skeletal Malocclusions 评估不同骨骼畸形中上颌骨和下颌骨基底长度与牙齿拥挤的关系

Introduction: Exploring the relationship between arch dimensions and crowding has long been a topic of interest. Many studies have found significant relationship between the two, while others have suggested that crowding is a multifactorial condition and is not dependent on arch dimensions only. Objective: To evaluate the relationship as well as sexual dimorphism between maxillary and mandibular skeletal base lengths with dental crowding in different skeletal malocclusions. Materials and Methods: The sample comprised 100 orthodontic patients (49 males, 51 females), which were divided into three groups based on the anteroposterior relationship of maxilla with the mandible. Each group was subdivided based on the severity of mandibular and maxillary arch crowding into two subgroups, and each subgroup was further divided into male and female subjects. Measurements of maxillary and mandibular base lengths (Co-A and Co-Gn) and tooth size arch length discrepancies were obtained through lateral cephalograms and dental casts, respectively, and measurements obtained were analyzed statistically. Results: Patients with class IB, IIB, and IIIB had significantly smaller maxillary and mandibular base lengths than subjects with class IA, IIA, and IIIA. A weak negative correlation was found between maxillary base length and maxillary crowding with r = –0.451 and p = .000, while a strong positive correlation was observed between maxillary and mandibular crowding with r = 0.742 and p = .000. Conclusion: An inverse correlation exists between dental crowding and the lengths of maxillary and mandibular bases, regardless of skeletal malocclusion.

Influence of Three Different Vertical Positions 
of MARPE Expander on Maxillofacial Complex:

Objective: The study was conducted to analyze the stress distribution and displacement due to three different vertical positions of the mini-implant assisted rapid palatal expansion (MARPE) appliance. Methods: A three-dimensional finite element meshed model was created of the maxillofacial complex with the help of cone beam computed tomography (CBCT) digital imaging and communication in medicine (DICOM) data of a patient with transverse maxillary deficiency using materialise interactive medical image control system (MIMICS) software. Three different groups with varying expander heights were used to study the expansion effects on the maxillofacial structures. von Mises stress and displacement at various skeletal and dental points were evaluated using ANSYS software. Results: In the midpalatal suture, the posterolateral portion of the hard palate and the maxillary and pterygoid bones showed the greatest amount of stress distribution and displacement pattern. Group 1 showed the maximum amount of transverse displacement compared to groups 2 and 3. Sagittally, all the teeth had a forward displacement. Vertically, groups 2 and 3 showed clockwise rotation of the occlusal plane, while all teeth showed downward displacement in group 1. Conclusion: In conclusion, a V-shaped expansion in the maxilla with a superior and posterior vertex was noted. Group 1, that is, appliance placed in close approximation to the palate, showed greater displacement compared to groups 2 and 3.

Human Unicorns—An Incidental Finding in

Cephalograms regularly disclose clinically noteworthy incidental findings to orthodontists who maintain a vigilant approach. In recent years, we have consistently identified a specific occurrence in the majority of lateral cephalograms, namely, the occipital spur. This refers to an enlarged external occipital protuberance, a protrusion at the rear of the skull which may manifest in either a smooth, crest, or spine-shaped configuration. Research suggests potential links to factors like brain expansion, adaptation to light levels, relief of neck muscle stress, and the influence of modern habits like smartphone usage in External occipital protuberance enlargement. Studies show a gender predilection, with smooth occipital spur more common in women and spine type more common in men, potentially aiding sex determination in forensics. Occipital spur influences on craniofacial measurements and the potential for causing occipital neuralgia, a condition characterized by pain in the posterior scalp. Management options range from conservative measures to surgical intervention, depending on symptom severity. Hence, this literature review compiles information concerning various types, prevalence, suspected etiology, clinical significance, and approaches to managing occipital spurs.

Aim: This article seeks to evaluate the shifts in publication rates and the participation of Indian orthodontists in different quartiles of orthodontic journals before, during, and after the pandemic, spanning the years 2017–2023. Methods: The evaluation involved manually searching through archives of 15 journals divided into quartiles, that is, Q1, Q2, and Q3. Articles with at least one author affiliated with an Indian university were collected and tabulated into graphs. Results: In Q1 journals, “Progress in Orthodontics” achieved the highest Indian publication percentage at 4.66%, whereas the European Journal of Orthodontics recorded the lowest at 0.93%. Among Q2 journals, the Journal of Orthodontic Science led with a maximum publication rate of 35.83%, while the Korean Journal of Orthodontics had the least, accounting for only 0.36%. In Q3 journals, the highest contribution of 31.16% was observed in APOS Trends in Orthodontics, while International Orthodontics had the lowest contribution at 8.24%. Conclusion: The examination of 15 orthodontic journals spanning the 7 years from 2017 to 2023 revealed that the publication trends remained largely unchanged in the majority of journals across all three time periods: pre-COVID-19, during the pandemic, and post-COVID-19. Notable exceptions included a slight increase in publications observed during and after the pandemic, specifically in the American Journal of Orthodontics and Dentofacial Orthopedics (Q1 journal), Journal of Orthodontic Science (Q2 journal), APOS Trends in Orthodontics (Q3 journal), and International Orthodontics (Q3 journal).

Objective: To test the accuracy of an artificial intelligence (AI) algorithm in predicting the favorability of maxillary canine impaction as compared to the conventional manual tracing method using orthopantomograms. Materials and methods: Orthopantomograms of 437 canine impactions were included in this study. Six parameters (sector classification, three angular, and two linear) on orthopantomograms were used to assess the severity of the maxillary canine impaction. The most advanced convolutional neural network model implemented using MATLAB program was used in assessing the favorability of maxillary canine impaction. The outcome of the parameters assessed by the implemented AI model and conventional manual tracing were compared. Receiver operating characteristic (ROC) curves, diagnostic accuracy, sensitivity, specificity, and positive and negative predictive values were used to assess the performance of AI model in comparison to manual tracing for the angular and sector parameters. Paired t-tests were used for linear measurements. Results: The overall clinical performance exceeded 90% for all the angular parameters including sector classification except for the angle between long axis of canine with lateral incisor which had a specificity score of 55%. The value and the area under the ROC curve were more than 0.9 for all the parameters. The distance from the canine cusp tip to the occlusal plane and midline was statistically significant between the groups ( p = .000). Conclusion: The proposed AI algorithm had higher accuracy in predicting the favorability of eruption in maxillary canine impactions compared to conventional manual tracing.

Objectives: To evaluate the efficacy of water irrigating devices, powered toothbrushes, and active repeated oral hygiene reminders on oral hygiene of fixed orthodontic patients. Materials and Methods: This was a randomized, examiner-blind clinical trial involving fixed orthodontic appliance patients. Patients were recruited within six months of bonding on both the arches. Eighty patients were randomly assigned to four groups; groups 1 and 3 subjects received standard oral hygiene procedures and used manual orthodontic toothbrushes, and subjects in groups 2 and 4 used powered toothbrushes and water irrigators for oral hygiene maintenance during the study period. In addition, repeated oral hygiene reminders via SMS were sent to subjects in groups 1 and 2, whereas groups 3 and 4 subjects received only oral motivation. The principal investigator assessed the plaque index (PI) and gingival index (GI) at baseline (T0) and after 2, 4, 8, and 12 weeks, that is, T1, T2, T3, and T4, respectively. Results: Group 2 and group 4 subjects showed statistically significant reduction in PI ( p < .001) and GI ( p < .001) scores at the end of study interval; however, the magnitude of decrease was less in group 4 in comparison to group 2. Intergroup comparisons between group 1 versus group 3 and group 2 versus group 4 were made to evaluate the efficacy of reminders. Statistically significant reduction was observed in group 2 as compared to group 4 at T4 ( p < .001) when compared to T1 ( p < .87). Conclusion: Using powered toothbrushes and oral irrigators resulted in a statistically more significant reduction in plaque and gingivitis scores than conventional procedures. Active reminders had a catalytic effect in improving the oral hygiene.

Success and Failure of MSE II Assisted MARPE: Comparative Evaluation of Two Cases MSE II 辅助 MARPE 的成功与失败:两个病例的比较评估

Mini-implant-assisted rapid palatal expansion (MARPE) offers a less invasive option for management of skeletal maxillary transverse discrepancy in advanced stages of palatal sutural maturation that cannot be optimally addressed by conventional rapid maxillary expansion techniques. This article briefly discusses two cases of transverse skeletal maxillary discrepancy matched for age and sutural maturation but with varied outcomes. One of the cases was successfully managed with MARPE using the maxillary skeletal expander II (MSE II) appliance, while the other failed. This communication also aims to highlight the possible cause–effect relationship and the caution required in case selection for such techniques. Since the technique is still in its infancy, MSE II should be avoided in palatal region with bone density below 1,000 HU to prevent inadvertent failure.
