Background. The role of the acute care nurse practitioner (ACNP) evolved as a response to a reduction in the health care workforce and the increased acuity of illness in hospitalized patients. Objective. This article reviews the purpose of the ACNP role at a university medical center in the specialty area of cardiology. An overview of role development as well as expansion of the role in the division of cardiology at The Mount Sinai Hospital in New York City, where there are currently more than 200 NPs now employed, is discussed. The responsibilities of the Cardiology Telemetry NP service are reviewed, including discussion of physician satisfaction with the collaborative clinical decision-making process. Conclusion. The role of the cardiology NP has evolved as a result of a focused effort at expanding the care provided by NPs to patients in cardiology. Clinical Implications. Strategies used by this service may be used in other departments to promote the development of the ACNP role, including garnering administrative support, promoting collegial practice relationships with medical departments, demonstration of advance knowledge and skills, and professional role modeling.

Background. With an increasingly elderly population, we are seeing more patients with multiple comorbidities unable to have surgical repair of their aortic valves. Transcatheter aortic valve implantation (TAVI) is now an option available for these patients, and the cath labs need to get prepared. Objective. To provide the nurses who will soon be offering TAVI in their cath labs an inside look at our program to help them prepare for this new innovative procedure. Methods. Through a description of our practice and sharing my perspective of our TAVI program, cath lab staff will have a good idea of what to expect and how to prepare for a TAVI program. Conclusion. Sharing experiences is a positive way to initiate new programs in any hospital setting.

The U.S. health care industry is standing on the precipice of an ever widening and deepening chasm. The breadth and depth of the health care chasm is affected by economics fueled by the unsustainable costs of providing services to an aging population and not having the means to pay for the costs. Reimbursement changes are driving physician-hospital integration primarily through physician employment models. Population growth primarily due to the baby-boomer population who start turning 65 years of age on January 2011 will increase demand for cardiac and vascular services. Timely and convenient access to services will be problematic as the number of physicians required may be stretched to capacity coupled with the likelihood that many cardiologists take early retirement given reimbursement changes. Augmenting the reimbursement challenges is the ever shrinking number of workers per Medicare beneficiary. Current legislation does little to address these challenges and may actually exacerbate them.
