Root perforation is an artificial opening occurs in the pulp wall creating communication between the pulp and the exterior. Traumatic (iatrogenic) perforations are due to lack of attention given to details of dental anatomy and failure to consider its variations by the clinician. Perforation of a pulp chamber floor usually occurs due to inattentive access opening or the bur is not properly angulated in relation to the long axis of the tooth

Introduction: Ingestion of a rubber dam clamp is a very rare condition that occurs due to careless manipulation of a rubber dam insertion especially in pediatric dentistry. Case report: The present paper reports a case in which a rubber dam clamp was accidentally swallowed by child undergoing pulpotomy in his upper right deciduous second molar, which entered esophagus and got stuck supraclavicular. Discussion: Ingestion of foreign materials may cause serious complications. Owing to the shape and sharpness of the instrument, there are chances of perforation. Once the instrument is lost in the oropharynx, it is very important to determine whether the instrument has entered the digestive tract or respiratory tract. Conclusion: Dentist should be able to manage an emergency situation, in which patient accidentally swallow a dental instrument.

Foreign bodies in root canals are rarely seen, and usually objects are accidently lodged and broken in root canals by the patients themselves. The occurrence of a foreign body, such as a metal screw, staple pin, sewing needle, pencil lead, bead or toothpick in the root canal system, makes the eradication of microorganisms impossible. Due to difficulties of eradicating microorganisms, foreign bodies may become sources of infection. These objects must be removed. This case report describes a rare clinical case in which a sewing needle, inserted into the root canal by the patient, was removed the orthograde approach with the aid of ultrasonic devices.
