Fanconi syndrome is a renal proximal tubule defect that causes reabsorption defects of electrolytes. The clinical features of Fanconi syndrome are amino aciduria, proteinuria, hypophosphatemia, metabolic acidosis, and glycosuria. In children, it is usually resulting from a genetic defect, such as cystinosis, galactosemia, tyrosinemia, hereditary fructose intolerance, and Wilson disease [1]. However, in adults, it is usually resulting from medications, toxins, and kidney diseases such as light chain proximal tubulopathy and primary amyloidosis [1]. Ifosfamide is a chemotherapy agent that is well known in the literature to cause Fanconi syndrome. Herein, we present a case of a woman with cervical cancer who developed ifosfamide-induced Fanconi syndrome after her fifth cycle of chemotherapy.

Pulmonary metastasis is not uncommon in the follow-up of patients with primary malignancy, but the expectoration of metastatic tumor is extremely rare. Metastases from renal and breast malignities may rarely lead to tumor expectoration with less common colorectal carcinomas. In this case report, expectoration of a metastatic tumor in a patient who had diagnosed with colon adenocarcinoma and pulmonary metastasis was presented. We named this quite rare condition as “metastasoptysis”.

Metabolic status is dependent on the anthropometric profile; indeed, many obese subjects are "metabolically normal" (MNO or Metabolically Normal Obese) while conversely, other obese or even non-obese subjects are "metabolically abnormal". The topography and the function of the fatty deposits seem to play a determining role in explaining this apparent paradox. In this study are examined the carbohydrate, lipid and liver balance of overweight subjects according to their anthropometric indices. It turns out that generalized obesity where the distribution of fat is diffuse as well as gynoid obesity (gluteo-femoral) are correlated at an almost normal metabolic profile compared to android obesity characterized by the metabolic dysregulations.

Primary renal squamous cell carcinoma (SCC) is an extremely rare primary malignancy of the kidney withonly few cases that have been reported in the literature. The indolent course of the disease and the lack of specific clinical and radiological features may be re - sponsible of the delay in the diagnosis leading to an advanced stage of disease at diagnosis with poor outcomes. We present the case of a 65-year-old man with long standing renal stone disease and recurrent pyelonephritis in his history, admitted for management of locally advanced renal tumor with distant metastases where histology of biopsy revealed SCC. We report this case to underline the rarity of the renal location as primary site of squamous cell carcinoma, the clinic-radiological and histopathologic features, in addition to treatment management and prognosis.

The chronic use of opioids may result in opioid induced consti- pation and disordered esophageal motility (achalasia). Patients who experience achalasia will experience nausea, vomiting, anorexia and weight loss which could complicate treatment of their opioid disorder. Naloxegol (a partial μ antagonist) is FDA approved to treat patients with opioid induced constipation. We report a single case of severe opioid induced achalasia successfully managed with naloxe- gol for 30 days.

Intra-Arterial Chemotherapy in Metastatic Bladder Cancer Refractory to Standard Therapy: Two Case Reports. Int J Case Rep Ther Stud 1: 004. Abstract Bladder cancer is the commonest malignancy of the urinary tract. Unfortunately most patients develop metastases during the course of their medical history. In this setting, chemotherapy treatment re- mains the only therapeutic option. Today, there is not standard therapy over the second line chemotherapy. We reported our experience with intra-arterial chemotherapy in two patients with metastatic bladder cancer liver progression disease after standard chemotherapy. The schedule used consisted in epirubicin 35 mg/mq and cisplatin 42 mg/mq administered into hepatic artery proper by bolus injection through a catheter inserted in the femoral artery with the Seldinger method, with cycles repeated every 21 days. Intra-arterial chemotherapy provided a good liver disease control, allowing to treat disease progression with radiotherapy, without important adverse events. Interesting, intra-arterial che- motherapy was well tolerated despite age, previous treatment and related residual toxicities.
