Introduction

Foot casting is essential in orthotic treatments. Several materials and procedures are involved in this process. There is no evidence of the reliability of casting techniques, as these procedures are based on the subjectivity of the podiatrist more than objective references. The aim of this study was to demonstrate the efficacy of a foot casting technique using plantar pressure sensors.

Material and method

The study was conducted on 9 people with 15 pronated feet (FPI > 6). A total of 3 types of foot weight-bearing casting were taken (relaxed position, controlled position, and controlled position using load sensors). The Heel Symmetry Index and 1st-5th metatarsal head height difference was calculated and compared in the 3 types.

Results

The comparison of Heel Symmetry Index in the different groups showed significant differences (P = .001). The 1st-5th metatarsal head height difference variable showed values of P < .001 in the different groups.

Conclusion

Foot casting by controlled plantar sensors achieved a more balanced cast with greater heel symmetry, and less difference in the 1st-5th metatarsal head heights.

Infiltraciones con plasma rico en plaquetas en la fascitis plantar, una revisión de la literatura 足底筋膜炎富血小板血浆浸润,文献综述

Background

The plantar fascia is a dense connective tissue structure located on the plantar aspect of the foot, covering muscular structures. Sometimes this structure is inflamed, by multifactorial etiology, and plantar fasciitis appears. When this condition becomes chronic, degenerative factor appears and renamed fasciosis. One of the invasive treatments are injections of platelet-rich plasma, which consists of being draw blood and centrifuge, thus obtaining a platelet concentrate, which is injected in the damaged zone. It contains many properties among which include the high content of growth factors present in platelets, which is what gives it character regenerative tissue.

Methods

A bibliographical search was made in the medical databases: Pubmed, and Science Direct. The terms used to realize this search were “plantar fascia anatomy”, “plantar fasciitis”, “platelet rich plasma” and “plasma rich in growth factor”. For the discussion section a new search was realized with the boolean terms were “platelet rich plasma” AND “plantar fasciitis”. For this section 10 papers were found.

Results

Comparing the recent studies have been able to observe the effectiveness of this technique applied to plantar fasciitis, since literature showed an important decrease of pain 6 or 12 months after application of platelet rich plasma injections.

Conclusion

Platelet-rich plasma injections reduce effectively pain caused by the plantar fascitis, due to the mechanism of repair of damaged tissues. An ideal quantity to reduce the pain of an effective way it would be from 2 to 3 ml.

Barefoot running and minimalist running have gained considerable popularity in the last years in the running community. This popularity has been achieved by bookseller Born to run, some specific scientific publications, main stream media coverage and websites devoted to the benefits of barefoot running. However, in the last years, much more evidence has been produced in this topic. This article summarizes the current evidence available on barefoot versus shod running at three different aspects of running: economy running, biomechanical differences and injury rates between both trends. The available current evidence about barefoot or minimalist running versus shod running suggests that there are no systematic benefits of one over the other in any of three different aspects reviewed. Clearly, it seems that the theoretical benefits attributed to barefoot and minimalist running are not supported by the current evidence available.

Epidermoid cysts of the sole of the foot are quite rare lesions which must be differentiated from other more common subcutaneous pathologies located on the sole. These lesions are usually slow-growing and unilocular and rarely affect the dorsum of the foot. A case of neglected bilocular epidermal cyst lesion located on the sole with invasion the dorsum of the forefoot is presented. Suspected diagnosis of these lesions should be made with the support of additional imaging tests and diagnostic confirmation should always be obtained after surgical removal and subsequent histopathological study.

Cambios en la postura del pie tras la actividad deportiva en ciclistas de montaña masculinos: estudio piloto 男性山地自行车运动后足位变化:初步研究

Purpose

The aim of this study is to determine whether there are changes in foot posture measured with Arch Index, Foot Posture Index, Navicular Drop Test and Foot Function Index after a session of 3 h of mountain biking in men.

Methods

The sample consisted of 19 male cyclists, and the average age of participants was 42.47 ± 7.75 years. The Body Mass Index was 25,84 ± 2,17. Subjects completed the Foot Function Index, and then they were measured the Foot Posture Index and Navicular Drop Test, and ink footprints were taken to determine the Arch Index with AutoCAD®. Then they practiced 3 h of activity on a mountain biking route. After cycling, they were remeasured the Foot Posture Index, Navicular Drop Test and ink footprints were taken for the AI.

Results

All values were higher in the post-activity, as being all of them but the Navicular Drop Test, greater on the right foot. However, only the Foot Posture Index variable presented statistically significant differences between pre- and post measures (P < .05). Also significant was the variable Arch Index pre and post only in right foot. We did not proceed to analyze data Foot Function Index, because most subjects obtained a score of 0.

Discussion

Based on the data obtained, it can be affirmed that the position of the foot sample of cyclists studied was neutral with a tendency to pronation after physical activity, as pronated legs are more capable to adapt at the sports.

Barefoot running and minimalist running have gained considerable popularity in the last years in the running community. Complicaciones de la osteotomía de Weil: análisis retrospectivo de 25 pies intervenidos en 21 pacientes 威尔截骨并发症:21例25英尺手术的回顾性分析

Introduction

Weil osteotomy is one of the most common surgical techniques used for lesser metatarsophalangeal joints. However, this procedure is not absent of complications. The present study analyzed retrospectively surgical complications associated with Weil osteotomy in a case series of patients.

Patients and methods

Surgical records were retrospectively reviewed for patients consecutively operated with a Weil osteotomy (simple or three step modification) from January 2012 and May 2015. Surgical complications such us transfer metatarsalgia, recurrence of metatarsal pain, floating toes, delayed unions or non unions as well as any complication noted in the record.

Results

It was included a total of 25 feet operated in 21 patients. Simple Weil osteotomy was performed in 8 feet (32,0%) and three step modification of Weil osteotomy was performed in 17 feet (68,0%). Three cases had transfer metatarsalgia. Twenty-two cases (88%) had floating toes at 3 months postoperative. There were no differences in floating toes between the simple and the tree step modification of Weil osteotomy. There were 7 casesof dorsal pain in the late postoperative period that were diagnosed as dorsal interosseous intertarsal compression syndrome.

Discussion

The present study shows valid information about the most common complications associated with the Weil osteotomy.

Revisión sistemática de pautas analgésicas para el postoperatorio en cirugías de pie y tobillo 足部和踝关节手术术后镇痛指南的系统综述

The podiatric surgery can cause postoperative pain. After of the operation the pacients must to start a painful walking which can cause antalgic’ wrong positions. The analgesic drugs are a useful tool for surgeons and patients. Thank to them we will get a convalescence with less pain and therefore with more satisfying recovery.

This study is a systematic review of the treatments postoperative analgesic after foot and ankle surgeries. The tarjet of this study has been to extrapolate a farmacologic treatment whose analgesic power was for above of others without to increase the unwanted side effects.

For this job we have compared results of thirteen studies previously selected, therefore we have considered among other variables the size of the used sample, study's type, pain scale used and route of drug administration.

Finally we have extracted a results to help to surgeon to improve his treatment and to improve the analgesia of the foot operated patients.
