Influence of led radiation on the staphylococcus aureus sensitivity to antibiotics led辐射对金黄色葡萄球菌抗生素敏感性的影响

We studied the influence of LED radiation of red + infrared (MEDOLIGHT Red device) and blue + infrared (MEDOLIGHT BluDoc device) ranges on clinical isolates Staphylococcus aureus and collection test strain Staphylococcus aureus ATCC 25923 sensitivity to antibiotics. The dependence of radiation on the studied objects depending on exposure, wavelength and frequency were defined. The result of photo modification on the state of microorganisms was increase of their sensitivity to some of the tested antibiotics. Based on the data obtained, we developed an algorithm and clinical recommendations for the use of LED radiation in the complex therapy of purulent-inflammatory diseases.

Low intensity laser effects on factors of congenital immunity and cytokine balance in secretions of female reproductive system affected with chlamydia infection 低强度激光对衣原体感染女性生殖系统分泌物中先天免疫因子和细胞因子平衡的影响

Therefore, we defined the objective of our research: to study the factors of uterine cervix tissue im- munity for women with Chlamydia infection and to estimate the effect of low-intensity laser therapy (LILT) on these factors due to a wavelength of 632 nm and a power of 30 mW.Materials and methods. Clinical immunologic studies of cervical secretion were carried out on 184 women of reproductive age 25,9 ± 4,22 in 2016–2018. Chlamydia infection Chlamydia trachomatis was found by the methods of real-time polymerase chain reaction (Rt-PCR) tests. Code by Classifier of the International Standard Classification of Diseases and Related Health Problems 10th Revision is А 56.0, Chlamydial infections of the lower sections of the urogenital tract. Urethra and canalis cervicis uteri scrapings were used as the material for detecting C. trachomatis. All patients with chlamydial infection of the lower part of the urogenital system were divided into two groups: a group of 100 women who were treated by using the local influence of the low-intensity laser of 632 nm as a part of complex treatment, the comparison group (basic treatment) consisted of 84 women who took azithromycin of 1 g once a day for the 1st, 5th, 10th days of treatment. 50 healthy women without any pathology and infections of the genitourinary system made the control group. A system of cytokines was studied by interleukin's level IL-8. IL-1 α, IL-1 β, TNF-α and IFN-γ content in cervical mucus.Results Course. The results of the study showed a high clinical and immunological efficacy of treating chlamydial cervicitis using a low intensity laser with a wavelength of 632 nm. An imbalance of cellular factors of innate immunity revealed before starting a therapy: a decrease in the oxygen-dependent metabolism of neutrophilic granulocytes of the cervical secretion.

Improvement the quality of life of patients with trophic ulcers of venous etiology in the application of photodynamic therapy 应用光动力疗法改善静脉源性营养溃疡患者的生活质量

Purpose. To analyze quality of life dynamics of patients with trophic venous ulcers of the lower limbs after the treatment with the use of photodynamic therapy. Materials and methods. Quality of life research was conducted in 57 patients with trophic ulcers of venous etiology. The first group included 30 patients who received standard treatment, the second group — 27 patients, in which standard treatment was combined with a course of photodynamic therapy. To assess the quality of life of the patients in both groups, a questionnaire was conducted using the EuroQol-5D-5L survey system and the visual analogue scale EQ-VAS. The quality of life assessment was carried out at the beginning of treatment, 10 days and 6 weeks after starting treatment. Results. The results indicate a significant improvement in the quality of life in both groups, but the degree of improvement in this indicator in group 2 was 20.7 % higher.An analysis of the dynamics of improvement in quality of life showed an increase in the mean of the indicator 10 days after the beginning of treatment in patients in the first group of 26.7 %, the second group by 43 %, and in 6 weeks, its increase was achieved in patients in the first group by 32.7 %, in the second group — by 53.4 %. Conclusions. The use of photodynamic therapy in the treatment of venous trophic ulcers significantly affects the quality of life, which is manifested in the anti-inflammatory effect and reduction of pain syndrome in the shorter term (by 76.2 %), compared with patients in the first group.

Experience of treatment of acute cholangitis with the use of anti-bacterial photodynamic therapy 抗菌光动力疗法治疗急性胆管炎的体会

Introduction. Acute cholangitis is a life-threatening disease characterized by an ascending inflammatory and infectious process in the biliary tree. Biliary decompression and systemic or local antibiotic therapy are key points in the treatment of acute cholangitis, regardless of the etiology of the disease. At the same time, an evergrowing antibacterial resistance requires the search for new ones, including non-pharmacological methods of combating infectious agents, one of which is photodynamic therapy. The aim of the work is to assess the effectiveness of the use of antibacterial photodynamic therapy in the complex treatment of acute cholangitis. Materials and methods. The paper presents the experience of using antibacterial photodynamic therapy in the complex treatment of acute cholangitis in 7 patients with obstructive jaundice. Standard methods of percutaneous transhepatic cholangiosis and conservative treatment of acute cholangitis were supplemented by intraductal antibacterial photodynamic therapy according to the technique developed by the authors. Photolon (Belmedpreparaty, Republic of Belarus) was used as a photosensitizer. The radiation was supplied through a quartz-polymer fiber with a diameter of 400 μm. The radiation wavelength is λ = 660 nm, the radiation power is 50 mW. The exposure time is 5–7 minutes. Results. The bactericidal effect of antibacterial photodynamic therapy was established, expressed in a statistically significant decrease in microbial contamination in bile already on the 3rd day of the study with its complete eradication on the 6th day of the study (p < 0.05). The average treatment time for acute cholangitis was 11.2 ± 2.4 days. Findings. Thus, the use of bile duct drainage in combination with intraductal antibacterial photodynamic therapy allows to stop the cholangitis phenomenon, reduce the economic costs of treatment, as well as the subsequent rehabilitation of this category of patients.

Artificial sources of light used in photomedicine: history of discovery 光医学中使用的人造光源:发现的历史

Introduction. The history of the development of photomedicine over the past one and a half years is directly related to the history of the development of artificial light sources. And the medical direction of using these sources, as befits the light, has always been extremely bright. The purpose of this study was to familiarize doctors, scientists, engineers working in the field of photomedicine, with the creators of artificial sources of the light that is used for the rehabilitation, diagnosis, treatment and prevention of the most common human diseases. Materials and methods. Sources of information were archival documents of scientific libraries, scientific journals, and conference proceedings. The search depth was almost 160 years. Results. As a result of this work, the names of three outstanding inventors of the first incandescent lamps born in the same 1847 and giving people artificial sun were mentioned again (Russian engineer Alexander Nikolaevich Lodygin, Russian military engineer Pavel Nikolaevich Yablochkov and American electrical engineer Thomas Alva Edison) . The work of the first arc lamps was based on research by the Russian inventor V.V. Petrov and the English naturalist G.Devi. As a result of complex experiments, the Russian physicist S.I. Vavilov discovered in 1924 that the efficiency of luminescent substances is much higher than the efficiency of incandescent lamps. The Nobel laureates of 1964 for fundamental work in the field of quantum electronics, which led to the creation of generators and amplifiers of a new type - masers and lasers, became the American physicist C. Townes and Soviet physicists N.G. Basov and A.M. Prokhorov. In 1960, the laser era began with the light arm of Theodor Harold Meiman, who created the first ruby-crystal laser operating at a wavelength of 694.3 nm. Then A. Javan, W. Bennet and D. Herriot created a gas (helium-neon) laser. In 1962, a semiconductor laser was created by a group of American (B.Lex, U. Dumke, M.Naten) and independently Soviet scientists (B.M. Vul and others). A carbon dioxide laser (molecular) was created in 1964 by K. Patel. A dye laser was created in 1966 by P. Sorokin and J. Lancard. The “father” of LEDs is considered by the whole world to Nick Holonyak, who in 1962 created the world’s first GaAsP luminescent diode emitting in the red spectrum. Findings. In the process of evolution of artificial light sources (from incandescent lamps, fluorescent lamps to lasers and LEDs), phototherapy methods were also improved.

The history of the development of photomedicine over the past one and a half years is directly related to the history of the development of artificial light sources. And the medical direction of using these sources, as befits the light, has always been extremely bright. The purpose of this study was to familiarize doctors, scientists, engineers working in the field of photomedicine, with the creators of artificial sources of the light that is used for the rehabilitation, diagnosis, treatment and prevention of the most common human diseases. Materials and methods. Sources of information were archival documents of scientific libraries, scientific journals, and conference proceedings. The search depth was almost 160 years. Results. As a result of this work, the names of three outstanding inventors of the first incandescent lamps born in the same 1847 and giving people artificial sun were mentioned again (Russian engineer Alexander Nikolaevich Lodygin, Russian military engineer Pavel Nikolaevich Yablochkov and American electrical engineer Thomas Alva Edison) . The work of the first arc lamps was based on research by the Russian inventor V.V. Petrov and the English naturalist G.Devi. As a result of complex experiments, the Russian physicist S.I. Vavilov discovered in 1924 that the efficiency of luminescent substances is much higher than the efficiency of incandescent lamps. The Nobel laureates of 1964 for fundamental work in the field of quantum electronics, which led to the creation of generators and amplifiers of a new type - masers and lasers, became the American physicist C. Townes and Soviet physicists N.G. Basov and A.M. Prokhorov. In 1960, the laser era began with the light arm of Theodor Harold Meiman, who created the first ruby-crystal laser operating at a wavelength of 694.3 nm. Then A. Javan, W. Bennet and D. Herriot created a gas (helium-neon) laser. In 1962, a semiconductor laser was created by a group of American (B.Lex, U. Dumke, M.Naten) and independently Soviet scientists (B.M. Vul and others). A carbon dioxide laser (molecular) was created in 1964 by K. Patel. A dye laser was created in 1966 by P. Sorokin and J. Lancard. The “father” of LEDs is considered by the whole world to Nick Holonyak, who in 1962 created the world’s first GaAsP luminescent diode emitting in the red spectrum. Findings. Low-level laser therapy in rehabilitation of patients with ischemic heart disease: review of evidence-based studies 低水平激光治疗缺血性心脏病患者的康复:循证研究综述

Cardiac rehabilitation is one of the main achievements of cardiology and physical rehabilitation medicine in the second half of the 20th century. Based on the methodology of evidence-based medicine, cardiac rehabilitation in a short time evolved from a “fictional” “alternative” unproven treatment direction to completely uncontested and proven multidisciplinary programs. This article presents contemporary findings on cardiorehabilitation and analysis of evidence-based experimental and clinical studies of low-level laser therapy efficiency in patients with ischemic heart disease. Aim. To search for evidence-based investigations on application of low-level laser therapy for rehabilitation of patients with ischemic heart disease and to analyze their major propositions and recommendations.Materials and methods. PEDro, EMBASE, PubMed and Cochrane library electronic databases were searched for randomized clinical trials, systematic reviews, meta-analyses and clinical protocols using the key words within 2000–2019 years. Results and discussion: experimental and clinical investigations were carefully considered and the following findings were analyzed: action mechanisms of low-level laser therapy, its positive influence on myocardial remodeling, infarction area reduction, restenosis prevention and presented cardioprotective effect. More attention was focused at clinical evidence-based studies on application of photobiomodulation in order to increase the efficacy of surgical and percutaneous myocardial revascularization, and in non-invasive cardiology and physical rehabilitation medicine. Conclusion: regular analysis of performed studies and realization of new evidence-based investigation should be conducted to evaluate the efficiency of low-level laser therapy in rehabilitation of patients with ischemic heart disease.

Surgical tactics for victims of gunshot wounds of the diaphragm specialized stage of medical care in conditions of a modern armed conflict 现代武装冲突条件下横膈膜枪伤受害者的外科治疗策略

Introduction. The diaphragm gunshot wounds are serious combat injuries. The main functions of the diaphragm are to change the intra-abdominal pressure and the outflow of lymph and blood from the abdominal cavity due to the constant contraction and relaxation. Therefore, when it is injured at the same time with a powerful painful impulse, cardiopulmonary disorders quickly arise. Purpose: To improve the results of surgical treatment of patients with gunshot wounds through the introduction of new video endoscopic technologies at the stage of specialized surgical care. Materials and methods. The article analyzes the features of specialized surgical care in 64 patients with gunshot wounds who were treated at the surgical clinic of the Military Medical Clinical Center of the Northern Region (III level of medical care). New minimally invasive methods are proposed to improve the outcomes of surgical treatment of victims with diaphragm gunshot wounds at level III of health care delivery. Results. Thus, the use of video thoracoscopic technique in the surgical treatment of the diaphragm wound and its suturing in the proposed method allowed to improve the results of treatment by increasing the average value of diaphragmatic excursion in deep breath in the main group to 3.73 ± 0.31 cm, whereas in the comparison group 2.21 ± 0.38 cm. The severity of the pain syndrome on the of Visual Аnalogue Scale 5 days after surgical treatment was 5.2 ± 2.3 points in the injured main group, 6.7 ± 2.1 points in the comparison group. The ratio of vital lung capacity to the required vital lung capacity in the main group was 75.3 ± 2.2%, in the comparison group 64.1 ± 1.7%. Conclusions. The use of video thoracoscopy increases the efficiency of the diagnosis of gunshot wounds of the diaphragm. The use of laser imaging and fluorescence diagnosis of diaphragm wounds ensure the adequacy of the removal of paravulary necrotic tissues. The suturing of the wounds in accordance with the anatomical and functional structure of the diaphragm with the use of plaques improves the immediate postoperative results of surgical treatment.

Hyperuricaemia is included in the risk factors for arterial hypertension. Arterial stiffness is one of the risk factors for arterial hypertension. The purpose of the study: to evaluate the effectiveness of low level laser therapy for the correction of hyperuricaemia and stiffness of the arterial wall at the comorbidity of hyperuricaemia with arterial hypertension. The study included 92 patients: 1st group — 42 patients with arterial hypertension, 2nd group — 50 patients with arterial hypertension in combination with hyperuricaemia. The low level laser therapy was performed by the Mustang 2000 apparatus with a wavelength of 635 nm. Correlation relationship between the level of uric acid and pulse wave velocity. After low level laser therapy, the level of uric acid decreased in the 1st group by 3.4 %, in the 2nd — by 19.1 %. In the 2nd group, the decrease in uric acid after low level laser therapy was 15.7 % more intense than in the 1st group. After low level laser therapy there was a pulse wave velocity decrease in both groups. Thus, the effectiveness of low level laser therapy for the correction of arterial hypertension and the reduction of arterial wall stiffness with arterial hypertension comorbidity with hyperuricaemia are proved.

Experimental justification of the use of electromagnetic radiation of the light range and tissue growth factors in the treatment of the failure of intestinal anastomosis 利用电磁辐射的光范围和组织生长因子治疗肠吻合失败的实验证明

The research work reflects the results of an experimental study, the purpose of which was to develop a method of sealing the small intestinal anastomosis under peritonitis. The experiment was performed on 20 white Wistar male rats weighing about 250 grams. The possibilities of using physical (light of a certain wavelength) and biological (tissue growth factors) methods in the prevention and treatment of acute inflammatory reactions and stimulation of reparative processes are shown. It is noted that a promising direction for further research on methods for the prevention of insolvency of small bowel anastomoses is the use of low-intensity electromagnetic radiation in visible spectrum in combination with the application of tissue growth factors.

The article discusses the psychosomatic approach to the treatment of pain syndromes in patients with discogenic dorsopathy, based on the combined use of kinesitherapy and psychological correction of anxious depression using transcranial exposure using neuroacoustic stimulation on individually rhythms and meso-encephalic modulation using magnetically and photostimulation. It was established that the treatment of pain syndromes in patients with discogenic dorsopathy based on the proposed method reduces the subjective assessment of pain on the VAS scale by almost 30 % and the level of depression by 20 % in the A. Beck psychometric table compared to the control group.
