The gastrointestinal pathology is one of the most common health problems, with direct implications in reducing the patient's quality of life. The COVID-19 disease, which has become a global public health emergency since early 2020, affects both the respiratory and digestive systems. The profile of the gastrointestinal disorders, thus becomes much more complex, being able to indicate an early infection with the SARS-CoV-2 virus. In this context, the open-circuit pharmacy beyond the important role of treating patients, has active implications in the education and prevent program of spreading the SARS-CoV-2 virus, at least until the until the efficacy of the COVID-19 vaccine is demonstrated.

Gastroesophageal reflux disease is a worldwide prevalent medical condition. It not only impacts the life quality of patients, but also plays a role in the socio-economic aspect in terms of management of symptoms and therapy. Attentive diagnosis is made after a physical examination that excludes other possible conditions such as cardiac or systemic diseases. Medication must be chosen in agreement with other underlying conditions (diabetes, autoimmune or rheumatic diseases etc.) as many interactions may occur otherwise. The role of a pharmacist plays a key role in this step. In the current situation of the pandemic, increased attention is necessary from the specialists´ point of view to fully benefit the patient.

Pharmacist’s role in community pharmacy is to provide counselling for patients with disorders accompanied by mild symptoms that do not require medical consultation, but also is to recommend a referral to a physician when the severity of the situation requires it. The pharmacist also performs counselling regarding posology of the prescribed medicines, and informs patients about potential side effects that may occur. Gastrointestinal disorders are widespread among the population and pharmacists have the role and the responsibility to use their knowledge to dispense appropriate medicines according to every patient’s needs, in compliance with the legislation and recommendations of ANMDM.

Pharmacists are health care professionals and their role is to provide health services, recommendations and support to all type of patients with a diversity of pathologies, including gastrointestinal diseases. In the last years, in countries where previously some gastrointestinal diseases were not so common, nowadays are augmenting, for example Crohn’s disease. In some country as Asia, Africa and Eastern Europe the number of patients with this type of diseases is constantly growing. Considering its high incidence, physical, mental and economic impact, it is vital to prevent as much as possible its apparition. High risk factors are represented by inadequate diet and lifestyle, medication side effects, drugs interactions or administration and dosage errors. The aim of this paper is to analyze the important role of pharmacists in preventing some gastrointestinal effects by offering correct information and advices to patients. This paper includes information related to self-medication, over-the-counter medication, side effects and the importance of microbial flora, equilibrate diet and lifestyle.

The novel coronavirus SARS-CoV-2 is responsible for the emergence of a potentially life-threatening disease (COVID-19) which has put tremendous pressure on the healthcare systems worldwide. Pharmacists, either from hospital or community pharmacies, are playing an essential role in the management of current COVID-19 pandemic as trustworthy partners of physicians, contributing to the reduction of drug-induced adverse events through medication guidance, thereby improving patient compliance and helping them regaining their confidence to overcome the disease. In the management of gastrointestinal symptoms that may or may not be in the context of COVID-19, pharmacists should recommend that patients consult a physician when gastrointestinal pathology is complex and associated with other symptoms.

Drug-drug interactions are a major cause of adverse reactions in polypharmacy, the incidence being directly proportional to the number of associated drugs. In this work, we highlighted the main drug-drug interactions that occurred in the treatment of gastrointestinal symptoms with proton pump inhibitors and histamine H2-receptor antagonists. The two groups of agents, used in gastroesophageal reflux and ulcer disease, can be released and without a prescription on the recommendation of the pharmacist, contributing to the risk of emergence of drug-drug interactions. Drug-drug interactions can occur due to the reduction of the gastric acidity, altering the biotransformation or excretion reducing of the drug co-administrated. pH increased may affect the absorption of some drugs decreasing (e.g. ketoconazole, itraconazole, atazanavir, vitamin B12, magnesium) or increasing the absorption of others (e.g. triazolam, midazolam). Furthermore, an attention to the interaction with narrow therapeutic index drugs (e.g. warfarin, phenytoin, theophylline) is required because of the risk of the toxic or side effects by the accumulation of these drugs.

Effective communication is an essential part of pharmacy practice aimed to ensured counsel patients about their prescribed medicines. This will lead to a rational use of medicine with an improved therapeutic outcome, increasing compliance and decreasing confusion and insecurity. For the purpose of legal regulation of pharmacist-patient communication, professional associations from different countries have developed the codes of ethics for pharmacists. As in other countries, the Code of Ethics of Pharmacists in Romania and Law 81/1997 guarantee respect for the rights and dignity of the patient. Communication is a highly complex area and ability to understand a patient’s feelings could be the key to ensuring high-quality interactions. Developing that relationship can mean addressing patients’ emotional responses to therapy, as well as acknowledging their concerns or frustrations. Through careful analysis of the communication in pharmaceutical practice we can help develop skills, tools and processes to make patient encounters as optimal as possible. This paper analyses some tools to understand a patient’s feelings that could be the key to ensuring high-quality interactions.

Over time, gastroesophageal reflux has undergone an unexpected and at the same time undesirable evolution towards the most commonly diagnosed upper gastrointestinal disease, called gastroesophageal reflux disease. The symptoms of gastroesophageal reflux disease are complex, difficult to treat and often recurrent. In order to prevent the disease recurrences, it is also necessary to establish the treatment in accordance with the gastroesophageal reflux symptoms. Unfortunately, nowadays, more and more patients of all ages go to the pharmacy with typical symptoms of gastroesophageal reflux, this ailment having a negative impact on the quality of life. This article aims to highlight the role of the pharmacist in recognizing the symptoms of gastroesophageal reflux and initiating firstline treatment.

The health and illness concepts have a major significance for both the patient and the healthcare specialists. Therefore, the role of the pharmacist is an extremely important one and, during the pandemic times, the relationship between the pharmacist and the patient has further expanded. The healthcare specialist should personalize the medical treatment plan for each individual patient, in order to obtain maximum efficiency with the lowest possible risk. Approximately 20% of all patients infected with SARS-CoV-2 present gastrointestinal symptoms, such as loss of appetite, nausea, vomiting or generalized abdominal pain. Any treatment recommendation made by the pharmacist should take into consideration general aspects regarding the intensity and the localization of the pain, as well as any chronic or acute treatment that the patient may undergo, all relevant medical history and the immediate prognosis of the disease.

Chronic gastrointestinal disorders are becoming more widespread among the population, therefore physicians and pharmacists are meant to use their knowledge and deliver appropriate treatments according to the needs of each patient, relying on new research results and medical practice acquisitions. Common chronic gastrointestinal disorders are: gastroesophageal reflux disease, dyspepsia, irritable bowel syndrome, coeliac disease, inflammatory bowel disease, with a growing global incidence and prevalence, especially in Western countries, being closely linked to socio-economic progress. The effect of patient counseling is seen in improving the patient`s quality of life. As part of medication therapy management, patient counseling focusing on compliance, diet and medications is an effective tool to improve health-related. Such services improve outcomes in chronic gastrointestinal disorders.
