Ultrastructural studies on implants failure with immediate or late loading 植入物即刻或延迟加载失效的超微结构研究

Objectives. The purpose of the present study was to evaluate by scanning electron microscope (SEM) and energy dispersive spectrophotometry (EDS), the degree of bone mineralization of the tissues on the surface of failure dental implants with immediate or late loading. Materials and method. In the study, 8 dental explants from 8 clinically healthy, non-smoking patients were taken. All implants were inserted by the same dental surgeon, 4 of them were immediate loaded and 4 were late prosthetically loaded by the same dentist. The ablation of the implants was performed as atraumatically as possible and they were collected in sterile containers and sent to the BIOMAT Research Center, where they were subjected to SEM and EDS analyses. The ratios between the chemical elements calcium and nitrogen (Ca/N), phosphorus and nitrogen (P/N), respectively calcium and phosphorus (Ca/P) were calculated for each of the 8 samples and were statistically analyzed. Results. If we compare the degree of coverage of the implants with bone tissue in different phases of mineralization, we find that the bone structures occupy a larger surface area of the implants in the cases with immediate loading compared to the cases with late loading. Thus, out of 4 immediately loaded implants, 2 were completely covered, one showed very little exposed areas, and the fourth showed alternating covered and uncovered areas. Regarding the late loaded implants, three showed alternation between covered and uncovered areas and only one was completely covered with bone. Conclusions. The degree of coverage of the explants with bone tissue was better represented for the immediate loading cases. The degree of mineralization of the bone tissue covering the explants was higher for cases with immediate loading. To confirm the obtained results, it is necessary to expand the study on larger batches of samples.

SEM and EDS comparative studies about the homogeneity of some cements used for luting of posts 用SEM和EDS对比研究了几种粘结剂的均一性

Objectives. The aim of the study was to evaluate comparatively by scanning electron microscope (SEM) and energy dispersive spectrophotometry (EDS) the physical and chemical homogeneity of some luting materials used for cementation of three types of posts. Materials and method. We selected 12 irretrievable monoradicular teeth extracted in our dental offices. After extraction, all teeth were prepared for cementation with 4 different cements (NANOCORE DUAL, CEMBEST, PANAVIA SA and CEMENT-ONE) of three types of posts: prefabricated titanium post, FibreKleer 4x fiberglass post (Pentron) and post cast from NiCrMo alloy. Afterwards, the samples were embedded in resin and prepared metallographically for examination with the Phenom ProX scanning electron microscope equipped with an energy dispersive spectrophotometer with X Rays. Results. SEM studies provide valuable ultrastructural information regarding the degree of filling with luting materials. The four studied cements do not present an inhomogeneous composition in the EDS analysis, not having significantly increased values between the 2 analyzed points for each of the 12 samples taken in the study. Conclusions. Among the four luting materials studied comparatively, PANAVIA SA seems to best fill the existing spaces between posts and the limits of dental preparations, being closely followed by CEMENT-One and CEMBEST. In last place is NANOCORE DUAL, which presents discontinuities in the mass of material and at the border with the preparations.

The modern dental caries adhesive direct restoration requires a working protocol that includes stages and techniques that must be strictly followed to ensure the correct morphological and functional reconstruction, as well as an increased longevity of the restoration and implicitly of the respective tooth in the oral cavity. Failures in achieving these goals are represented by the occurrence of recurrent caries, secondary or residual caries, coronary fractures, leading to pulpal and periodontal complications. They can be due to both incorrect therapeutic maneuvers and other causes, for which the dentist is not responsible, such as manufacturing defects of dental materials that are not visible during inspection or the patient's attitude towards oral hygiene.

Statistical study concerning doctor’s opinion in using the Diode Laser in Endodontics 二极管激光牙髓治疗中医生意见的统计研究

Our questionnaire-based study with 14 items proposed an investigation of the dental practitioners' opinions and knowledge of lasers, especially on the diode lasers application in Endodontics. The number of respondents was 104, with a higher percentage of 44.2% between 23-30 years old and 61.5% women. 86.5% of the participants worked in the private dental system. 80.8% of the respondents stated that they do not use the diode laser in their daily practice. Only 43.3% considered diode lasers useful in the sterilization of root canals, and 87.5% of respondents considered the sodium hypochlorite the most effective antiseptic. Among those who do not use laser systems, 76.9% were complaining about the high costs of this equipment. 65.4% of the respondents stated they never used the diode laser during endodontic treatment, and the most frequent use was for sterilizing the infected root canals. 87.5% did not know which wavelength to use in practice. Asked if they would like to learn how to use the laser system, 76.9% (80) of respondents answered affirmatively, and 45.2% of study participants said they wanted to purchase a laser system in the future. There is a great need to educate dentists to use diode lasers and make them aware of their advantages. The only problem at present is the need for more affordable laser equipment.

FEA on the biomechanical behavior of immediately loaded implants with different sizes 不同尺寸即刻加载种植体生物力学行为的有限元分析

Objectives. The purpose of this research was to study, in the case of immediate loading, the biomechanical effect produced by the length of the implants. Material and method. The study material was a CBCT analysis performed on a patient from one dental office in Bucharest. An segment of edentulous mandibular bone was selected from the CBCT, which was processed with Mimics Innovation Suite, respectively Mimics and 3-matic. After processing the bone segment, two implants of the same manufacturer, with identical design, but different length – 10 and 13 mm respectively, were selected from the BIOMAT database. To simulate immediate loading, the bone-implant interface was not blocked and the mandible was defined with properties that characterize a bone with moderate density. A perpendicular masticatory force of 200N was applied to each of the two implants. The software ANSYS calculated the minimum, maximum values and their geometric means for the possible stresses produced on both the shorter implant (10 mm) and the longer implant (13 mm). Results. In the case of short implants, higher average stresses develop along the entire length of the implant, towards the vestibular bone plate, while in the case of long implants the higher stress seems to be cantoned towards the apical side. Conclusions. The present study shows that, in the case of immediate loading, the use of longer implants (13 mm) reduces by more than 50% the geometric mean of the stresses to which the bone-implant interface is subjected than in the case of the use of shorter implants (10 mm). In both types of implants, higher stresses occur at the level of the screw fixing the abutment in the implant.

Objectives. The aim of the present study was to analyze comparatively, through the finite element method (FEM), the biomechanical behavior of different types of devices used to increase the resistance of restorations on endodontically treated teeth, upon the experimental application of a force of 350N. Materials and methods. We studied comparatively three types of posts, respectively: fiberglass post, cast post from Cr-Co-Mo alloy and prefabricated titanium post. Three irretrievable extracted teeth for periodontal reasons were endodontically treated and then specifically prepared for cementation of this devices. The teeth were scanned at a radio-diagnostic center, and the obtained DICOM files were processed with MIMICS and 3-matic and then subjected to finite element analysis with ANSYS. Results. From the 48 simulations carried out for the case of all-ceramic crown coverage, the use of titanium post is associated with 25 situations in which maximum or minimum values appear, the use of cast post presents 12 situations in which maximum or minimum values are developed, the use of fiberglass posts is associated with 10 simulations where maximum or minimum values occur, and in one case the deformations are equal. Regarding the use of the zirconium oxide crown, the results are comparable. Conclusions. The presence of zirconium oxide crowns generally produces less valuable stresses and deformations at the level of the components. The stress and strain values are lowest when using fiberglass posts, followed closely by cast post and far behind by titanium post. In all simulated situations, the limit values of stresses and strains are mostly recorded at the tooth root, which is susceptible to fracture in case of parafunctional forces, followed by the three devices themselves and the covering crowns.

A systematic review of sinus floor augmentation complications. Does graft type influence the complications rate? 窦底增高并发症的系统回顾。移植物类型是否影响并发症发生率?

Background. Sinus floor elevation is considered a safe procedure to properly augment the height of the edentulous crest. Often, complications may arise due to multiple causes and can lead to a poor outcome of the graft and implant’s osseointegration. A careful surgical approach, a good knowledge of possible obstacles, and careful treatment planning can reduce the risks of complications, as well as their impact on the surgical outcome. This study aims to make a literature review of 40 articles, analyzing the incidence and type of complications related to maxillary sinus graft surgeries and graft materials. Material and methods. A total of 40 articles published between 2015 and 2021 were selected for a systematic literature review on maxillary sinus floor complications. The studies were selected from 2 different internet databases: PubMed and Science Direct. The sinus lift complications were counted and their incidence was organized upon the surgical technique and the timing of occurrence (intraoperative, postoperative). It was also analyzed if the graft material influences the complication rate. Results. In a group of 1757 sinus augmentation surgeries performed on 1605 patients, 363 complications were found. Sinus membrane perforation occurred in 242 cases, 29 procedures resulted in partial or total graft loss, there were 26 postoperative cases of sinusitis, 24 sinus infections, 16 wound dehiscence, 6 bleeding complications, 5 lost implants, and 2 oro-antral fistulas. Conclusion. Sinus lift complications are sometimes inherent circumstances of the procedures, but they can also be prevented through an accurate technique and preoperative plan. The type of bone graft does not influence the incidence of surgical complications.

Objectives. The aim of this study was to evaluate the prevalence of selected bacterial species from periodontal pockets samples in a group of young adult periodontitis patients and explore the additional value of the Real-Time Polymerase Chain Reaction (RT-PCR) assay. Material and Methods. Patients with periodontitis diagnosis and good general health were included in the study after a complete periodontal examination and following the inclusion/exclusion criteria. Samples from the deepest periodontal pockets were collected, and RT-PCR assay was performed in a private laboratory in order to identify eleven bacterial strains associated with periodontitis. The patient’s report provided the bacterial pathogen load quantified according to the detection threshold and classified as negative, weakly positive, positive, or intensely positive. Results. The RT-PCR assay detected periodontal pathogens, and their prevalence could be calculated based on the test report. The red complex bacteria were detected in all patients with the highest counts. Alongside the orange complex bacteria were also detected in periodontitis patients. A. actinomycetemcomitans remained undetected in our study. Conclusions. The RT-PCR technique proved to be a useful and easy approach for identifying and quantifying subgingival periodontal pathogens in periodontitis patients. RT-PCR testing may facilitate accurate diagnosis and prognosis as well as successful treatment.

Periodontitis is a major public health concern because of its high prevalence and due to the significant impact on the masticatory function and aesthetics. Periodontitis can cause social inequality and greatly reduce quality of life. If a proper periodontal treatment is implemented, effective personal plaque management, and a thorough supportive phase, the majority of patients with periodontitis can preserve their natural teeth for an extended period of time. The subgingival mechanical instrumentation is the gold standard of periodontitis treatment and it determines a significant change in the subgingival microbiota. Due to the fact that in some clinical situations subgingival instrumentation is not totally efficient different locally delivered antibiotics can be used as adjunctive therapies to periodontitis treatment. The present article aims to provide information with respect to some locally delivered antibiotics used as adjunctive therapy in periodontitis treatment.

Dental bleaching is a common procedure, requested and accepted by patients with aesthetic requirements. The SEM images were taken at different degrees of magnification, always looking for cracks, porosities, or other changes at this level, which could be related to the sensitivity that appeared after such a procedure. The standard bleaching procedure was applied to the labial surface of the tooth. Biolase Laser white 20 bleaching putty was used, which contains 45% hydrogen peroxide and then activated with 940 nm laser light. The microscopic SEM images of the labial faces on which the 45% concentration hydrogen peroxide type bleaching gel was applied showed that they do not present any structural damage to the enamel.
