Strain and Burden among Caregivers of Stroke Survivors in Punjab: A Cross-Sectional Study 旁遮普中风幸存者照顾者的压力和负担:一项横断面研究

Introduction: Caregivers may feel stressed and burdened while caring for a stroke survivor. However, very few studies have assessed the burden among caregivers of Indian stroke patients. Objectives: The present study was done to assess the magnitude and predictors of strain and burden among caregivers of stroke survivors discharged from a tertiary care hospital in Punjab, India. Method: This cross-sectional study included consecutive newly diagnosed stroke patients from January 2021 till April 2022 at a tertiary care teaching hospital in Amritsar, India. The level of strain and burden among the caregivers was assessed using the Caregiver’s strain index (CSI) and Zarit Caregiver Burden Scale (ZCBS) respectively. A minimum sample size of 110 stroke patients was calculated. Odds ratios were calculated for identifying predictors of strain and burden among caregivers using multivariate regression analysis. Results: Mean CSI for all caregivers was 6.67 ± 1.81. Using a cut-off of ≥ 7, authors found 40.2% of the caregivers to have great strain. Modified rankin scale (mRS) at the time of interview (AOR = 2.77, p value = 0.0063), presence of comorbidities in the patient (AOR = 3.07, p value = 0.023 and caregiver being female (AOR = 2.96, p value = 0.036) to be significantly associated with higher odds of having great strain. Using ZCBS, 18.1% had minimal burden, 53.5% had mild to moderate burden, 24.4% had moderate to severe burden and 3.9% had severe burden. mRS at the time of interview (AOR = 5.38, p value = 0.0048), presence of comorbidities in the patient (AOR = 7.63, p value = 0.0031), caregiver being female (AOR = 9.67, p value = 0.0072) and not being at all confident to care (AOR = 4.94, p value = 0.013) to be significantly associated with higher odds of having great burden, while rural residence (AOR = 0.55, p value = 0.0011) was associated with lower odds of high burden. Conclusion: High strain was observed among 40.1% of the care givers of Indian stroke survivors and 24.4% had moderate to severe burden. Thus, the strain and burden among caregivers of Indian stroke patients is very high.

Tobacco Cessation Centres: Vital Cog in the Fight against Tobacco Epidemic Preparedness towards a future pandemic 戒烟中心:防治烟草流行病的重要一环,防范未来的大流行病

The tobacco crisis, which claims more than 8 million lives yearly and adds 1.2 million more from secondhand smoke inhalation, is a serious concern for world health. The detrimental impact of tobacco usage on health is projected to have an annual economic cost of 1.4 trillion US dollars (health costs plus lost productivity) or 1.8% of the world's GDP. Both smokers and users of smokeless tobacco can profit greatly from quitting. Quitting smoking lowers the likelihood of developing respiratory problems, delays their beginning, and slows the progression of chronic lung diseases. Therefore, the "World Health Organisation (WHO)" and the "Ministry of Health & Family Welfare", Government of India, worked together to open official tobacco cessation clinics throughout India. The goal of tobacco cessation centers was to offer to counsel people trying to stop smoking. This includes monitoring the patient's progress and providing counseling, either by itself or in conjunction with medication.

Assessment and Correlation of Physical Activity and Mental Health Status of Medical Students at One of the Medical Colleges of Karnataka, India 印度卡纳塔克邦某医学院医学生体育活动与心理健康状况的评估与相关性

ntroduction: Physical inactivity and the associated health problems pose a current and growing threat to public health. People with sedentary behaviour are at higher risk of suffering from symptoms of anxiety and depression. Medical students suffer very high levels of psychiatric illness, depression, suicide, dependence on alcohol and drug abuse. Objectives: 1. To assess the level of physical activity and mental health status among medical students. 2. To compare the mental health status in relation to the adequacy level of physical activity among them. Method: Cross-sectional study was conducted among 145 undergraduate medical students. Data was collected using pre-tested and pre-designed proforma. International Physical activity Questionnaire short form (IPAQ-SF) and General Health Questionnaire-12 (GHQ-12) were used to assess level of physical activity and mental health status respectively. Statistical analysis was done using SPSS version 27.0. Descriptive statistics and chi-square test was applied. Results: Among 145 medical students, 60% and 16.55% were found to have moderate and low physical activity. Distress and psychological distress was found to be among 27.59% and 22.75% students. It was found that, 50% of the participants, who were doing low level of physical activity were found to be distressed, which was found to be statistically significant. (χ2 = 10.826, df = 4, p = 0.02859073). Conclusion: Physical activity and mental health are related proportionally. Regular physical activity has the potential to reduce anxiety, distress, and depression. Importance of physical activity, which has positive influence must be emphasised to reduce mental stress among medical students.

Vaccination has been proven to be one of the most efficient and affordable health interventions available worldwide to prevent diseases and deaths. Global vaccination services had good coverage during 2022 yet 20.5 million children are still missing [1] their life saving vaccines. Establishing a dedicated vaccination clinic especially in the private institutions could be one solution to further improve childhood as well as adult vaccination coverage.

Prevalence and Socio-Demographic Factors Associated with Anemia among Females of Age Group 10-45 Years in a Rural Population of Gurugram, Haryana 哈里亚纳邦Gurugram农村人口中10-45岁年龄组女性贫血的患病率和相关社会人口因素

Introduction: Anemia is the most common nutritional deficiency disorder in the world. Its Prevalence is inordinately higher among developing nations, because of unfavorable socio- demographic factors and indigent access to healthcare services. Therefore, it is pertinent to find the prevalence of anemia and its associated factors. Objective: To find out prevalence and socio- demographic factors associated with anaemia among females of age group 10-45 years in a rural population of Gurugram, Haryana. Method: A cross sectional study was conducted in villages under PHC Garhi Harsaru in Gurugram, Haryana among 452 females of age group 10-45 years by two stage random sampling technique. Results: The mean age of the study subjects was 25 ± 6.39 years. The overall prevalence of anemia among women of age group of 10-45 years was found to be 62.39%. Joint family, lower literacy, and low socioeconomic status were found to be statistically significant risk factors for anaemia among study subjects. Conclusion: The prevalence of anemia was very high; therefore attempts should be made to sensitize the rural population through health and nutrition education, information, education and communication (IEC) and appropriate behaviour change communication (BCC) activities.

Public friendliness towards Frontline Healthcare workers in Kolkata, India: Preparedness towards a future pandemic 公众对印度加尔各答一线医护人员的友好态度:为未来的大流行做好准备

Introduction: A new wave of COVID-19 infection has hit China of late and India due to her geographical proximity is at risk. More than 700 doctors in India have died on the frontline during COVID-19. Despite their sacrifices and service, discretionary behaviour by the public have been frequently reported. Objective: To assess the attitude and perception towards frontline Health Care Workers (HCW’s) among residents of a selected zone of Kolkata (India). Method: An observational study of cross-sectional design was performed in December 2022, using a pre-designedstructured questionnaire, that was sent to the registered email address of the families residing in the selected area of a zone located in Southern Kolkataand response from one eligible member per family was assessed. Of 142 families residing in the area, responses from 119 families were obtained. Results: Most participants agreed or strongly agreed to co-operating with HCW’s during door-to-door screening campaigns and supporting their families (94.8% and 83.7% respectively). Majority (74.4%) favoured functioning of private clinics; however, 60.7% didn’t want to allow doctors to work post-COVID recovery. Though 64.9% believed HCW’s were taking adequate measures to contain disease spread, 77.8% wanted to maintain more than required ‘safe distance’ from them and 35.1% favoured keeping them away from neighbourhood. About 67% thought HCW’s were properly trained and equipped to handle COVID-19 cases (47.8%). Participants with age more than 33 years (p=0.030), males (p=0.044) and who sought health advice in last month (p=0.016)were found to have a favourable attitude. Conclusion: The study finds a mixed opinion in the public about HCWs. Considering the difficulties faced byHCWs, which affected the functioning of the healthcare system, this scenario can be tricky while tackling similar emergencies of the future. Hence, measures to reduce societal stigma against HCWs is of great necessity.

Method: An observational study of cross-sectional design was performed in December 2022, using a pre-designedstructured questionnaire, that was sent to the registered email address of the families residing in the selected area of a zone located in Southern Kolkataand response from one eligible member per family was assessed. Of 142 families residing in the area, responses from 119 families were obtained. Results: Most participants agreed or strongly agreed to co-operating with HCW’s during door-to-door screening campaigns and supporting their families (94.8% and 83.7% respectively). Majority (74.4%) favoured functioning of private clinics; however, 60.7% didn’t want to allow doctors to work post-COVID recovery. Though 64.9% believed HCW’s were taking adequate measures to contain disease spread, 77.8% wanted to maintain more than required ‘safe distance’ from them and 35.1% favoured keeping them away from neighbourhood. About 67% thought HCW’s were properly trained and equipped to handle COVID-19 cases (47.8%). Participants with age more than 33 years (p=0.030), males (p=0.044) and who sought health advice in last month (p=0.016)were found to have a favourable attitude. Conclusion: The study finds a mixed opinion in the public about HCWs. Considering the difficulties faced byHCWs, which affected the functioning of the healthcare system, this scenario can be tricky while tackling similar emergencies of the future. Considering the growing public health significance of some forms of caffeine use, such as the consumption of energy drinks with high caffeine content that are currently largely unregulated or combinations of caffeine and methamphetamine, the International Classification of Diseases 11 (ICD 11) separates caffeine from other stimulants. A stimulant and diuretic, caffeine is a bitter alkaloid that is mostly found in coffee, tea, cacao, and kola nuts. As a stimulant of the Central Nervous System(CNS) it boosts the body's flow of hormone like cortisol. Caffeine promotes relative brain hypoperfusion by increasing energy metabolism while simultaneously diminishing cerebral blood flow. Caffeine stimulates noradrenaline neurons, and appears to affect releaseof dopamine locally.Caffeine is quickly and nearly entirely (up to 99%) absorbed into the bloodstream after consumption.

Appraisal of ‘Physical Activity’ Patterns among Medical Students: A Cross- Sectional Study Using International Physical Activity Questionnaire -Short Form (IPAQ-SF) In Lucknow, India 医学生“体育活动”模式的评估——印度勒克瑙国际体育活动简表(IPAQ-SF)的横断面研究

Introduction: The health benefits of physical activity are well established. In India, it is estimated that overall, 39.2 crore individuals are physically inactive. The trends of physical inactivity among Indian adolescent and youth populations are also alarming which implies that a huge population is at risk for developing NCDs. Objective: To assess physical activity levels among MBBS students. Method: The study was conducted at a medical institute in Uttar Pradesh. Sample size of 342 was drawn using stratified random sampling. The data collection was done based on “IPAQ- short form” using self-administered questionnaire. Data analysis was done as prescribed by the IPAQ-SF tool kit. Results: A total of 342 MBBS students participated in the study. Mean age of the participants was 20.86 years (Range 18-26 years). A total of 117(34.2%) medical students were found to be “insufficiently active” (Category-1),134 students (39.2%) were just minimally active (Category -2) and rest 91(26.6%) students were HEPA (Health Enhancing Physical Activity) active (Category-3). Median MET- minutes/week and Inter Quartile Range (IQR) for vigorous physical activity was 0(0-640), for moderate activity it was 60(0-320) and for walking it was 462(297-924). Overall total physical activity MET- minutes/week and IQR was 942(401-2066). The median and (IQR) values for sitting were 10(8-12) hours/day. Conclusion: The study reveals that a noteworthy proportion of the participants do not meet the recommended levels of physical activity, indicating a pressing public health concern. Findings necessitate the importance of behaviour change activities to promote physical activity in the study population.

Introduction: Multi skilling and expansion of the team members involved in operationalizing Sub-Health Centres (SHCs) to Sub-health Centres-Health and Wellness Centres (SHC-HWCs) in India tends to be a challenge. New roles are being assigned to Community Health Officer (CHO) in an exponential manner. Objective: To identify the team-based interpersonal challenges encountered by Community Health Officers (CHOs)in terms of human relations in professional forefront and extract probable solutions from them for overcoming these challenges. Method: It was a sequential type of mixed method done in two-step process, quantitative survey of the defined indicators at SHC-HWCs followed by group discussion among CHOs for qualitative data. Quantitative survey was aimed at finding utilization of services. (Triangulation of both the data) whereas qualitative survey was aimed at finding out interpersonal challenges faced for adequate service delivery. Authors targeted to collect data from all the SHC-HWCs (N=53) in Dahod block for quantitative and qualitative survey. Results: The service utilization related to Maternal health and child health; childhood and adolescent health were found in range of 90-95%, followed by community participation (70%). The qualitative analysis of data revealed that CHOs were overburdened by their superiors and perceived lack of co-operation, dedication, and accountability from fellow team members. Conclusion: Utilization of certain services like non-communicable diseases (NCD), palliative care, mental health was found to be suboptimal. Significant lack of motivation and coordination were possibly some of the contributors.
