Optimizing Workplace Safety: A Comprehensive Analysis Of Accident Risks Through FMEA And RCA Methods 优化工作场所安全:通过 FMEA 和 RCA 方法全面分析事故风险

Occupational health and safety (OHS) is the key to the company's growth in modern industry. Awareness of OHS has a positive impact on employees financially and non-financially. Indifference to OHS can be fatal, causing harm. OHS includes the establishment of a safe work environment through various approaches. Work accident data at PT. XYZ reaches 36% of the workforce, not yet meeting the government's zero accident policy—workshop at PT. XYZ has 9 types of OHS Risk. This study will use an assessment method using the Failure Mode and Effect Analysis (FMEA) method. After identifying and assessing these risks, improvement proposals will be made using Root Cause Analysis (RCA). Judging from the risks that need to be overcome immediately from the RPN value obtained, namely operating a gas-cutting machine with a value of 71.94, the eye is exposed to iron splashes during the turning process with a value of 64.51. Finally, interference with the eye during the welding process with a value of 60.11 is broken down through the RCA method to find the root cause of the problem. Worker safety is a critical factor in the continuity of an oil palm industry because the progress of the palm oil industry is also influenced by the availability of safety guarantees for its workers. Based on the results of the study, it was shown that the highest percentage of accident risk assessment was at the sterilizer work station (boiling FFB) using the HIRARC method, namely in the activity of pulling the cable sling from the winch and then attaching the hook to the lorry from the transfer carriage with 25 the risk of accidents occurring with a percentage of 33.33 % and categorized into extreme, for the second highest accident risk assessment, namely in the activity of pulling a lorry using a winch and in the activity of opening the sterilizer door and lowering the sterilizer bridge with a work accident risk of 16 times with a percentage rate of 21.33% and categorized into high incident and the risk of work accidents, namely in the activity of releasing the hook and carrying it then installing it to the last lorry and in the activity of pulling and carrying sling cables from the winch with a risk of accidents of 9 times the incidence of work accidents with a percentage rate of 12.00% and categorized into incident medium. Keywords: Minimizing the Risk of Occupational Accidents and the HIRARC Method

Material Management System for the Waste Processing Industry at PT. Pallet with Economy Order Quantity (EOQ) Method PT.Waste Processing Industry 的材料管理系统采用经济订货量(EOQ)方法的托盘系统

PT. Pallet is a waste processing company that uses plastic waste as the main material to make plastic pallet products. Based on initial observations, this company experienced problems in material management, thereby reducing cost-effectiveness and efficiency. The purpose of this study is to identify problems in material management and the procurement of raw materials for plastic waste used in the production process of making plastic pallets, as well as to find out some of the causal factors for the occurrence of deficiencies/excesses in the procurement of raw materials for production waste. This research uses a descriptive quantitative approach, and the researcher acts as a research instrument or data collector. The analytical method used is Economy Order Quantity (EOQ), Safety Stock, and Reorder Point (ROP). The EOQ method has a lower total inventory cost. After calculating using the EOQ method, it is clear how much material must be ordered, the time to place the order and the total costs that must be incurred. The number of economic orders in the 3 months was 1,153.57 kg in October, 1,153.02 kg in November and 1,262.22 kg in December. The safety stock needed to expedite the production process is 394,345 kg in October, 376,306 kg in November and 99,652 kg in December. The results of the EOQ calculation produce a lower frequency of reorders, namely 203 times in one month, compared to the frequency of orders without EOQ, namely 378 times in one month.

Industrial company PT. Berlina Tbk. currently engaged in manufacturing processing of plastic pellets into plastic packaging products. From the data the author has obtained for 1 month from 21 August to 22 September 2022, the production process at PT. Berlina Tbk. has a bottle product defect in 500 ml plastic packaging quality. From 21 August to 22 September 2023, this production resulted in 6,421 (2%) defects from the total machine production of 287,897 (100%) 500 ml plastic bottles. This study uses a qualitative descriptive method by conducting field research activities (field research), observations, and interviews and by studying books, journals, literature, and other references related to this research. The analysis used in this study is the FTA analysis (Fault Tree Analysis) and the FMEA method (Failure Mode and Effect Analysis). From the research to overcome and reduce the three most significant types of defects using the FTA (Fault Tree Analysis) method, it was found that the causes were influenced by several elements, such as workforce, machines, and materials. With the discovery of the most extensive defect, the researcher has made a proposal based on the RPN (Risk Priority Number), which is then analyzed using the FMEA (Failure Mode And Effect Analysis) method, namely the submission for dirty material defects by checking in the storage room for raw materials, then the type of dirty oil defect by checking on the compressor air section so that during the blowing process it avoids dirty oil. The proposal from the last researcher is a type of folded defect to periodically check the cutting temperature to see if there is a shorted thermocouple.

Comparison of Clusterization of Higher Education Institutions in Regions XII Maluku and North Maluku based on the Science and Technology Index (Sinta) 根据科技指数(Sinta)比较马鲁古第十二区和北马鲁古区高等教育机构的集群情况

This research aims to analyze the comparison of clustering results of universities based on data sourced from the Science and Technology Index (Sinta). The analysis results will serve as a guide for all higher education institutions in the XII Maluku and North Maluku regions to improve their performance in the three pillars of higher education to achieve better clustering. This study was conducted by extracting Sinta data from the years 2019 to 2021. The research results show that based on the Total All Score, the universities in Maluku are ranked in descending order from the highest to the lowest score. The main cluster is occupied by Pattimura University with a total score of 16.04, followed by State Polytechnic of Ambon with a total score of 13.45. In the middle cluster, we have Indonesian Christian University of Maluku with a total score of 9.19, and in the basic cluster, we have Darussalam Ambon University with a total score of 6.62, Maluku Husada Health Sciences Institute with a total score of 4.79, Doktor Husni Ingratubun Tual University with a total score of 2.5, and Institute of Technology and Business, Ambon School of Computer Science with a total score of 2.37. As for North Maluku, the universities are ranked in descending order from the highest to the lowest score. The main cluster is led by Universitas Kharun with a total score of 14.01. In the middle cluster, we have Muhammadiyah University of North Maluku with a total score of 11.62, and Halmahera University  with a total score of 9.45. In the basic cluster, we have Nuku University with a total score of 5.45, Pasifik Morotai University with a total score of 5.3, Hein Namotemo University with a total score of 4.63, and Bumi Hijrah University Tidore  with a total score of 3.31

PT. Pallets is a manufacturing company that processes plastic waste into products that have value, namely plastic pallets. A plastic pallet is a flat city-shaped container used as a base for storing and transporting goods in logistics warehousing. This study aims to improve the quality of plastic pallet welding products of the HDX1210R type at PT. Pallets. This study uses the Six Sigma method with the DMAIC (Define - Measure - Analyze - Improve - Control) approach with the help of fishbone diagrams, Pareto diagrams, and FMEA. From the company's historical data from March 2022 to March 2023, 3 types of defects were obtained, with loose welding as the highest type of defect, with a percentage of 89% of the total product defects. The overall DPMO average value is 49908.1405, and the average sigma value is 3.191. From FMEA calculations, the highest RPN value is obtained for frequently changing over machines with a scale value of 480. The proposed improvements can be made by changing the hitter model and size, modifying the mold (pallet molding) by adding the welding area on the pallet and making SOPs for producing HDX1210R-type pallet welding.

This study aims to identify strategic positions through Internal and External (IE) analysis at UD. Reza Jaya and Formulate what business strategy should be carried out by UD. Reza Jaya to be able to win the business competition. This study uses a quantitative approach with descriptive analysis. The location of research was carried out at the Building Store, UD. Reza Jaya Besur Village, Sekaran District, Lamongan Regency. The research was used from April-May 2023. The data collection method is by making a reservation or studying directly on the research object. The data used are interviews and secondary data documentation. SWOT analysis is used for the advantages and disadvantages of UD. Reza Jaya, both internally and externally. The results showed that internal and external factors indicated the position of UD. Reza Jaya is currently in quadrant I, namely, showing the "grow and build" position. And the SWOT analysis produces eight alternative strategies with specifications through SO, WO, ST, and WT strategies. Proposed improvement priorities used to improve the company's business processes can be carried out by applying the concept of the Business Model Canvas. The results of the QSPM analysis show the formulation of the most likely alternative strategy to be carried out by UD. Reza Jaya is a Product Development Strategy with the highest score of 11.80000. While the second alternative, namely the horizontal integration strategy, only gets a value of 10.37000

Application of Fuzzy Time Series Method Cheng Model in Forecasting Stock Prices PT Bukit Asam Tbk 模糊时间序列法程模型在预测 PT Bukit Asam Tbk 股票价格中的应用

Investment in stocks is one type of investment that can get huge profits, but there are also great risks. So it is necessary to analyze in advance before starting an investment in stocks, in order to avoid losses. One way is to forecast the stock price using fuzzy time series Cheng. The data used is weekly period stock price data from PTBA in January 2020 - December 2022, which can be categorized as a form of time series. From this research, the forecasting value for the next period is Rp. 3797. Which results in a MAPE of 4.2%, which means that FTS Cheng method is very good to use in forecasting the share price of PT Bukit Asam Tbk, because it produces a MAPE value <10%, and produces an RMSE of 158 rupiah, which means the average of the difference between actual and forecast values.

This research focuses on analyzing the types of conductors and electrical installation safety at Sanctoo Suites & Villas Singapadu, where this research aims to determine the type of conductor, current carrying strength (CRC) flowing in the conductor, protection MCB, contactor, TOR, ELCB, and Surge Arrester installed already meets PUIL standards.After this research is completed, it is hoped that this research can provide consideration to the company that the importance of the electrical installation system, especially the installation of electric power for safety and comfort, by using the installation of electrical equipment according to standards.

The Belawan Main Kesyahbandaran Office is a Technical Implementation Unit under the Directorate General of Sea Transportation, Ministry of Transportation which has duties in accordance with the Minister of Transportation Regulation Number PM 34 of 2012 concerning Organization and Work Procedures of the Belawan Main Kesyahbandaran Office which has the task of carrying out supervision and law enforcement in the field of shipping safety and security, as well as coordinating government activities at the port. In this study the authors analyzed the Leave Submission Procedure at the Belawan Main Kesyahbandaran Office directly, using several methods to obtain data from the office such as conducting interviews and observations. In taking leave, employees of the Belawan Main Kesyahbandaran Office must comply with the applicable flow or procedure and also the Operational Standards. In addition, there are also several obstacles in applying for employee leave which can cause leave applications not to be ACC by superiors / leaders
