The incidence and the risk factors of oculocardiac reflex in cataract surgery: A prospective observational study 白内障手术中眼心反射的发生率及危险因素的前瞻性观察研究

Background: Oculocardiac reflex (OCR) is a cardiac manifestation that accompanies physical stimulation of the eye during various ocular surgeries and may be potentially life threatening. The purpose of this study is to investigate the incidence and risk factors of OCR during cataract surgery. Methods: Adult patients who were planned to undergo cataract surgery under local anaesthesia were enrolled in to the study. Heart rate (HR) and mean arterial blood pressure (MAP) were measured at baseline and at various steps during the surgery. The parameters including various demographic characteristics were compared between patients with and without OCR. Results: One hundred and thirty-six patients (60 males and 76 females) were recruited. Out of them, 49 patients (36.03%) underwent phacoemulsification and 87 (63.97%) patients underwent manual small incision cataract surgery. OCR was noted in 22 (16.18%) patients. Age, presence of co-morbidities and baseline MAP were found to be significantly associated with OCR. Baseline characteristics like gender, HR and duration of surgery were comparable between both the groups. Conclusion: OCR can occur at various steps during cataract surgery under peribulbar block.

Quality of living and visual impact of ICL V4c: A questionnaire-based survey ICL V4c的生活质量和视觉影响:一项基于问卷的调查

Purpose: To evaluate patient satisfaction based on a questionnaire survey on quality of living and the visual impact on the quality of life in patients with moderate to high myopia who underwent implantable collamer lens (ICL) implantation in the South Indian population. Design: A cross-sectional questionnaire-based survey. Methods: Around 125 patients were subjected to two questionnaires: 1) A modified questionnaire on the quality of life impact of refractive correction evaluates the ecstatic state of study participants (SP) after the deliverance of spectacles, the gratified state of expected vision postoperatively, and satisfaction on cost versus benefit of the procedure, improvement in confidence levels of the SP after the procedure, and finally, their inquisitiveness on indoor and outdoor activities; 2) a modified quality of living questionnaire with six categories assess the physical, psychological, self-reliance, social relationships, independence for conveyance, and partaking in recreational activities through telephone or Google sheets. Results: The mean age of the patients was 26 (4.2) years. Around 98% (124 SP) felt satisfied with the expected visual quality after the procedure. About 2% (n = 2) had to wear glasses, and 27% (n = 34) still had difficulty driving due to glare and halos; 96% felt that the cost spent for the procedure was worth enough to achieve spectacle independence after ICL. Furthermore, in 99% (n = 124), physical activities and in 83% (n = 104), self-esteem and confidence levels were increased. Conclusion: Our study highlights the importance of the need for refractive error correction and its impact on social life. ICL has been a life-changing procedure for many young adults.

Treatment modalities of pseudophakic cystoid macular edema 人工晶状体黄斑囊样水肿的治疗方法

Purpose: To evaluate the clinical profile of cystoid macular edema (CME) following cataract extraction and to assess the response to various treatment modalities. Methods: This was a prospective analysis of all patients diagnosed with CME after cataract surgery conducted in the period from July 2018 to July 2019. Patients were divided into three groups based on their degree of visual impairment. Patients whose visual acuity (VA) ranged from 6/6 to 6/18 were allotted Group A and were treated with topical prednisolone acetate 1% and topical ketorolac tromethamine 0.5%. Group B included patients whose VA ranged from 6/24 to 6/36 and were treated with periocular injection of 40 mg triamcinolone acetonide (TA). Patients with VA <6/60 were allotted Group C and were treated with intravitreal injection of 4 mg TA. Best-corrected visual acuity (BCVA), intraocular pressure (IOP) and optical coherence tomography (OCT) findings were recorded at 1-month follow-up. Results: A total of 53.3% developed CME in the first 4–6 weeks post-operatively. CME was more common in patients who underwent extracapsular cataract extraction (ECCE). A total of 75% of patients in Group A, 86.36% in Group B and 66.66% in Group C showed complete resolution of CME at 1-month follow-up. The difference between the pre-treatment and post-treatment macular thickness measured by OCT was statistically significant (P < 0.05) for all groups. Conclusion: CME occurring after cataract surgery usually presents within 4–6 weeks of surgery. Topical non-steroidal anti-inflammatory drugs (NSAIDs) and periocular steroids are effective in the management of pseudophakic CME. However, the choice of treatment is individualised to the patient's needs.

Subconjunctival loa loa in an urban clinic 城市诊所的结膜下血肿

An uncommon filarial infestation in our geographic location is Loiasis. However, we reported a case of microfilariae Loa loa worm in the subconjunctival space. A 69-year-old male, a painter by occupation who lives in Chennai, presented with irritation and watering in his right eye for one month. An adult female Loa loa worm was extracted from the subconjunctival space and was treated with topical and oral steroids under supervision. With the suspicion of clinical manifestation, patients should be questioned about international travel, look for painless swelling in the body that comes and goes, and peripheral smear should not be delayed if there are signs of systemic infestation.

Clinico-pathological profile of tumour and tumour-like lesions of eyelid—A case series and literature review 眼睑肿瘤及肿瘤样病变的临床病理特征——病例系列及文献复习

Eyelid lesions can be benign or malignant and can arise from skin and appendages, subcutaneous areolar tissue, orbicularis muscle, tarsus, modified glands like Moll's glands, and the conjunctiva. Epidemiological data on eyelid lesions are less evident in the existing literature and they represent different ethnic groups. The present case series is a prospective study conducted to understand the clinical presentation and histological appearance of the lesions of the eyelid. In this case series, we report all the consecutive tumours and tumour-like lesions of eyelid, seen at the outpatient department of Ophthalmology in a tertiary care hospital in rural south India over a period of one year from October 2021 to September 2022. There were five cases, all of which were benign, solitary lesions with different cellular origins, namely, epidermoid cyst, intradermal melanocytic nevus, apocrine hidrocystoma, clear cell hidradenoma with foci of trichoepithelioma, and solitary neurofibroma. We conclude that clinico-pathological evaluation of mass lesions of eyelid, not only ensures definitive management but also helps to understand the epidemiology of the disease.
