Prevalence of Sexual Dysfunction in Women With Depressive Disorder at a Tertiary Hospital 三级医院抑郁症妇女性功能障碍的患病率

Background: Sexual dysfunction in female with depressive disorder is highly prevalent and strikingly understudied. Epidemiological and clinical studies have shown association between depression and impairments of sexual function. Despite high prevalence, there are limited studies regarding the prevalence of sexual dysfunction among female with depressive disorder in Nepal. Aims: This study was conducted to find out the prevalence of sexual dysfunction in female with depressive disorder in Nepal, their association with treatment status, and the level of psychological distress caused by it. Methods: A descriptive cross-sectional study was carried out among female with depression at Tribhuvan University Teaching Hospital, Nepal. The participants were grouped as drug naïve and under medication. Data were collected using semi-structured proforma, Beck Depression Inventory, Kessler 6+ Psychological Distress Scale, and Female Sexual Function Index. Data were tabulated and analyzed using Statistical Package for Social Sciences version 23. Results: The prevalence of sexual dysfunction was 58.82%, being 60.78% in drug-naïve cases and 56.86% in females suffering from depression and receiving medications. This study showed statistically significant association between sexual dysfunction and psychological distress but not with age, duration of marriage, number of children, occupational, and financial status. Conclusions: The study reported high prevalence of sexual dysfunction among female with depression. All domains of sexual function were affected and showed significant association with psychological distress. Therefore, assessment of sexual dysfunction in every female with depression is required to address the problem timely and efficiently.

Common Pitfalls in the Management of Vaginismus in Couples With Subfertility in India 印度不孕夫妇阴道痉挛管理的常见陷阱

Vaginismus is described as an involuntary pelvic muscular spasm that occurs due to “fear of pain” associated with vaginal penetration which prevents a woman from experiencing penovaginal sexual intercourse despite her wish to do so. In the context of heterosexual couples with subfertility, untreated vaginismus may result in unconsummated marriage and/or reduced quality of sex life and/or can affect a woman’s perception about her potential of femininity and motherhood. Though vaginismus is an easily diagnosable and treatable condition in many instances, it often gets ignored or inadequately/inappropriately managed. There is paucity of practice-informed clinical data on management of vaginismus in couples with subfertility that is contextualized to the Indian setting. The present viewpoint is based on real-world clinical scenarios who presented as couples to a tertiary fertility center with unconsummated marriage as their chief complaint. The intent of this viewpoint is not only to highlight the common pitfalls in the management of vaginismus but also showcase a clinically proven interdisciplinary management approach for assessment and management of vaginismus and any associated sexual problems.

Pornography—Is It Good for Sexual Health? A Systematic Review 色情--对性健康有益吗?系统综述

Aim: The topic of pornography use is controversial. It is important to understand how young people use pornography and determine whether pornography use has adverse effects on health and well-being. Methods: A comprehensive systematic literature search was conducted in 4 electronic databases (PubMed, Google Scholar, CINAHL plus, and Cochrane library) with appropriate MeSH terms “sexual health” and “pornography” and Boolean operators “AND” and “OR,” using SPIDER search strategy tools (sample, phenomenon of interest, design, evaluation, and research type). 11 articles were proceeded with systematic review after critical appraisal following PRISMA guidelines. Results: The major findings of the study imply that traditional and unimaginative activities depicted in some pornography and sexualized media are harmful because they impose restricted and circumscribed concepts of sex and sexuality. As a result, sexism, sexual objectification, neoliberal sexual consumerism, and sexual variety are reproduced and reinforced rather than promoted. FPU is linked to better levels of sexual comfort and self-acceptance and reduced levels of anxiety, shame, and guilt over sexual behavior. Pornography consumption has also been linked to increased arousal and orgasm responses, a greater interest in sex, acceptance of various sexual acts, and more sexual experimentation. Conclusion: Watching pornography may be a healthy phenomenon if it is occasional, not impairing the personal and social life; however, it can become pathological if watched excessively and impairs the individual’s functioning.

Internet Pornography Use Among Medical Students in India: Extent and Effect 印度医学生使用网络色情的程度和效果

Background: Medical students are more susceptible to stress associated with long working hours, arduous studies, and living alone away from the home, which may affect their psychological well-being. They may indulge in the use of internet and pornography to ameliorate their stress. Internet is one of the main sources of pornographic content as it is easily available, affordable, and accessible. Objective: The current study seeks to quantitatively estimate the extent of internet addiction and pornography use among medical students in India, and its effect on their general well-being. Methods: We conducted a cross-sectional study in a medical college in North India involving undergraduate (including interns) and postgraduate medical students. Those using internet for porn use were assessed using a semi-structured sociodemographic questionnaire, General Health Questionnaire 30, WHO Quality of Life Index (WHOQOL)-BREF, and Internet Addiction Test (IAT). Results: A total of 393 medical students who agreed on their pornography internet usage were recruited for the study. Problematic pornography internet usage (moderate and 0severe IAT scores) was seen in 11% (80) of the participants. Male and married students were more likely to be problematic users. The mean duration of pornography use was 8.02±6.32 h/week. The time spent on pornography use positively correlated with IAT and General Health Questionnaire scores, whereas it negatively correlated with WHOQOL-BREF scores. Conclusions: Internet pornography use is significantly prevalent amongst medical students in India. As the duration of pornography use increases, general health measures and quality of life decrease, which may impact their well-being and academic performance.

Adverse Effects of Plasticizers and Pesticides on Female Reproductive Health 增塑剂和农药对女性生殖健康的不利影响

Central neuroendocrine system regulated by hypothalamus, controls most of the body homeostasis involving processes, like metabolism, reproduction, stress responsiveness, growth, and energy balance mainly through hormonal signals. Plasticizers and pesticides interact as endocrine disruptors with endocrine hormones causing adverse effects which tend to destroy the body homeostasis. Exposures to these compounds during critical developmental stages such as puberty and pregnancy (prenatal or perinatal) influence neurodevelopment, social behavior of the growing fetus and causes sexual dimorphism. Plasticizers and pesticides systemize its effects on adulthood either by mimicking, antagonizing, or having an impact on steroidal activity also along with hormonal disruptions. The aim of this review is to address some of the effects of plasticizers and pesticides exposure on female behavior. In this review, we are discussing the remedial nutritional choice to control the plasticizers and pesticides mediated endocrine disruption.

Studies on Sexual Behavior During COVID-19 Pandemic: A Bibliometric Analysis 新冠肺炎流行期间性行为的文献计量分析

Sexual relations suffered from a drastic change because of the pandemic of the COVID-19 and its controlling measures. We performed a bibliometric review of articles conducted on sexual behavior through this COVID-19 pandemic. We performed a search in the Scopus database on August 14, 2021. The search was carried out by the mentioned search terms: “sex* act*” OR “Sex* Behav*” AND “COVID-19.” A total of 230 published articles was identified in the Scopus database, out of which 130 research works were funded and 23.91% (n = 55) articles had international collaborations. R Stephenson published the highest number of papers (n = 4) whilst SK Kar had the maximum number of citations (n = 39) and the highest number of citations per article (13). Universidade de São Paulo and the University of California were the most productive organizations whilst the USA outnumbered the other countries. Sexually Transmitted Infections (n = 13) and Elsevier (61) published the highest number of papers as a journal and publisher, respectively. It is the first bibliometric review on studies that assessed impact of the COVID-19 pandemic on human sexual activities during which identified that more than half of the articles were funded which is a promising sign for the researchers from this field and for those who are keen to work in this field.

Marriage is an anthropological, cultural, and legal institution, that establishes socially sanctioned rights and obligations between individuals. In many cultures, marriage forms the basis for acknowledgement of sexual relationships. However, sexual violence and physical aggression within marriages have traditionally formed a grey legal area. Marital rape refers to “forcible sexual assault or violence by one spouse towards the other.”1 In other words, it’s the act of sexual intercourse with a spouse without his/her spouse’s consent. Though historically establishing sexual relationship between the married couple was considered as a “right” in many societies, the context of consent becomes equally important as among nonmarried individuals. Twentieth century onward, there has been growing international conventions and voices against sexual and intimate partner violence in marriages (more specifically for sexual violence against women).2 However, in spite of the known devastating consequences of any form of forcible sexual encounter, marital rape has remained under the shadow of legal ambiguity in many nations, outside the criminal law and widely tolerated. Marital rape is mostly, but not exclusively, experienced by women. It tends to form a vicious cycle of abusive relationships between the couple, perpetuating chronic violence. This also varies based on sociocultural and political ideologies. For example, the interpretations of the institution of marriage, traditional ways of viewing male and female sexuality, and cultural expectations of relationship dynamics among the husband-wife dyad have led to concerning reluctance of classifying nonconsensual marital sex as a punishable crime. These doctrines started getting challenged in the West between 1960s and 1970s during the “second wave feminism” that focused on gender respect, autonomy, and right to self-determination (concerning all matters of a women’s own physical self and identity).2 However, marital rape has been overlooked in literature and policies throughout centuries and “marriage” being used as a common exemption/defense in sexual assault cases. This has also led to invalidation of the experiences of marital rape survivors, reduced help-seeking, and persistent trauma.

Profile of Patients Availing Marital and Psychosexual Clinic Services During the Pre-COVID and COVID Era: A Study From a Tertiary Care Center 前新冠肺炎和新冠肺炎时代接受婚姻和性心理诊所服务的患者概况:一项来自三级护理中心的研究

Aim: To assess the profile of patients availing the marital and psychosexual clinic (MPC) services through the telemode at a tertiary care center after the imposition of lockdown and compare the same with the profile of the patients attending the outpatient services, in-person MPC services during the prelockdown period. Materials and Methods: It was a retrospective study in which comparisons were made between patients availing the MPC services from March 24, 2020 to March 23, 2021 with the profile of patients during the period of March 24, 2019 to March 23, 2020 (ie, COVID period—lockdown). Results: A total of 112 patients availed the MPC services during the COVID study period compared to 307 patients in the pre-COVID period. However, when the proportion of MPC cases among the total walk-ins was compared, during the COVID period the proportion of MPC cases increased by almost 1.75 times. During the COVID time period, there was an increase in the patients who were significantly younger (P = .002), females (P = .003), more from urban locality (P = .035), and being referred by self or caregiver (P = .001). In terms of specific diagnosis, as compared to the pre-COVID timeframe, significantly higher numbers of patients were diagnosed with premature ejaculation (PME) and PME + erectile dysfunction while the services were being provided through the telemode. Conclusion: The present study suggests an increase in the proportion of patients seeking help for marital and sexual issues while the services are being provided by telemode.

Psychiatry Disorders and Sociodemographic Profile of the Victims of Assault Attending the Teaching Hospital—Cross-Sectional Study 在教学医院接受攻击的受害者的精神疾病和社会人口特征——横断面研究

Background: According to World Health Organization, proportion of women experiencing either physical or sexual violence ranges between 15% and 17%. In India, one-third of women population in 15 to 49 age group experiences sexual assault at least once in their life, predisposing them to develop psychiatric disorders. Aims and Objectives: To assess prevalence of psychiatry disorders among victims of assault attending tertiary care center. Methodology: It is a retrospective, record-based study which was conducted among 216 victims of assault attending one-stop center. Study was started after obtaining Institutional Ethical Committee clearance and permissions from concerned authorities and confidentiality was maintained throughout the study. Data was analyzed by frequency, proportion, and chi-square tests using SPSS version 16. Results: Out of 216 victims, 50% were physically assaulted and 50% were sexually assaulted. Among 108 sexual assault cases, 81.5% victims were raped, 2.7% were sexually harassed, 1.9% was sexually abused, and 13.9% children were sexually abused. A total of 30.1% had adjustment disorder, 11.6% had dysthymia, 8.3% had mild depression, 5.6% had moderate depression, 0.5% had obsessive-compulsive disorder, 0.5% had psychosis, and 43.5% did not have any psychiatry disorder. Conclusion: Majority of the sexually assaulted victims belonged to age group 16 to 20 years and had adjustment disorder, whereas physically assaulted victims belonged to age group above 36 years and had dysthymia and depression. Special services like medical care, counseling, legal aid, and so on should be provided to victims free of charge. Awareness and sensitization programs should be done through active community participation for the welfare of children and women.
