In this article I will explore IPE, the benefits and challenges and current examples of IPE in midwifery. This article has been independently reviewed prior to publication.

Review of ABM Clinical Protocol #36: The Mastitis Spectrum ABM临床方案#36的回顾:乳腺炎频谱

Recent scientific evidence debunks the concept of milk ducts obstructed by a collection of milk as it is not physiologically or anatomically possible.1 The Mastitis Spectrum is a recently revised guideline which challenges the common belief that ‘blocked ducts’ precede inflammatory mastitis, potentially followed by infective mastitis. The evidence now demonstrates that mastitis encompasses a spectrum of conditions, from mild to severe. Moreover, traditional strategies to augment milk removal to counteract milk stasis and breast massage to relieve ductal obstruction from milk ‘plugs’ lacks physiological validity. The Academy of Breastfeeding Medicine (ABM) Medicine Clinical Protocol #36 details management recommendations for all eventualities of The Mastitis Spectrum and will be referred to as the ABM Protocol in this review. Sepsis is a condition where the body’s reaction to an infection harms its own tissues and organs, putting the patient at risk of shock, organ failure and even death. It is critical to identify sepsis early and provide immediate treatment to avoid these outcomes. Sepsis is a serious life-threatening illness that is very difficult to predict, diagnose and treat. It can be caused by any type of infection: bacterial, viral, fungal or even parasitic. Sepsis prevention is only possible by preventing infections with good and consistent hygiene and avoiding people with infections.1

Skin-to-skin contact (SSC) is a simple intervention that has been practiced for noncomplicated vaginal birth over the decades. Evidence showed that an increased breastfeeding rate is one of the benefits for mothers and babies. Although caesarean section (CS) has increased globally, many mothers and babies have not experienced SSC after caesarean delivery. Therefore, hospitals around the globe have implemented similar projects to enhance the practice of SSC after CS. This literature review will investigate the significance of SSC after CS to breastfeeding rate and the results of SSC implementation in the theatre.

Midwives aspire to support and partner with women in one of the most pivotal times of their lives. Midwifery is unique and purposeful, providing endless opportunity to focus on different aspects of care. This article provides a brief overview of the experiences of women and midwives in relation to obstetric violence. The reflection describes an alternate midwifery research pathway in an attempt to identify and address the systemic issues that contribute to maternal mistreatment. The experiences of the author whilst undertaking a Master of Research is included with the aim to provide information for midwives with an interest in research and practice change.
