Post-infectious immunosuppression may cause repeated hospitalization after COVID-19 pneumonia. The results of the observational study of the Raphamin use for immunotherapy in the recovery period of COVID-19 pneumonia are presented. Aim: to estimate efficiency of Rafamin usage in patients who have completed the course of inpatient treatment for coronavirus pneumonia. Material and methods. Thirty patients aged 18 to 80 years after hospital treatment of COVID-19 pneumonia were included and randomized into 2 groups (1: 1). All patients received anticoagulants, antioxidants, metabolic drugs. Patients of the 1st group (n=15) were additionally prescribed Raphamin for 5 days. The primary endpoint was the number of repeated hospitalizations due to consequences of COVID-19 and/or new cases of acute respiratory infection for 28 days of follow-up. In addition, dynamic of immune and inflammatory markers (absolute lymphocyte count, C-reactive protein), proportion of patients with immune dysregulation, and duration of symptoms associated with COVID-19 were assessed. Results. The number of hospitalized patients in group 1 was 0 (vs 5 in group 2, p=0.0421). Study therapy reduced the risk of repeated hospitalizations in 1.44 times [relative risk 1.44;95% confidence interval 0.91–2.28];duration of breathlessness decreased from 24.5 to 15.3 days (p=0.0108), and duration of fatigue reduced from 23.6 to 16.8 days (p=0.0452). The proportion of patients with immune markers normalization was 2 times higher than in the comparison group on 14 day of observation. Conclusion. The immunomodulatory therapy may be recommended during the recovery period of COVID-19 pneumonia. © Eco-Vector, 2022.

The problem of COVID-19 vaccination in patients with autoimmune inflammatory rheumatic diseases is discussed. There is an increased risk of viral infections in these patients. Attending physicians should provide patients with rheumatic diseases with complete information about the risks and benefits of COVID-19 immunoprophylaxis. The use of immunosuppressive drugs, rather than the diseases themselves, can reduce the level of post-vaccination immune response. This requires choosing the optimal time for carrying out COVID-19 vaccination in this group of patients. Objective. To describe the clinical and epidemical characteristics of a new coronavirus disease 2019 (COVID-19) in people living with HIV, for HIV infection implies the development of an immunosuppressive condition that may exacerbate the course of COVID-19. Material and methods. The research is based on retrospective and current epidemiological situation of HIV and SARS-CoV-2 infections in the Southern Russia regions during 2020 and survey of the patients with the co-infections concerning epidemiological, clinical, and laboratory diagnostic information. We collected all data from 15 regional centers for AIDS prevention and control in the Southern and North Caucasus Federal Districts. The survey sample consists of 121 patients. Statistical computation is done with Microsoft Office Excel 2010. Results and discussion. HIV patients of various age and social characteristics are involved in the COVID-19 epidemic process. Within registered HIV and SARS-CoV-2 co-infections all patients have apparent clinical symptoms. Asymptomatic cases are not presented. Mild cases prevail in the sample (48.8%). The frequency of severe and extremely severe was significantly higher in people living with HIV/AIDS on ART more than 2 months against naive PLHIV or using ART up two one month (р<0.05). © 2022 by the authors.

Features of the course and medical and social aspects of a new coronavirus infection in the Republic of Tajikistan 塔吉克斯坦共和国新型冠状病毒感染的病程特点及医学和社会方面

The aim - to assess some medical and social aspects of the epidemic process during the first wave of a new coronavirus infectious disease - COVID-19 in the Republic of Tajikistan. Material and methods. The retrospective study was conducted on the basis of an epidemiological analysis of official statistics as part of the epidemiological surveillance of COVID-19 from April 2020 to April 2021. Results and discussion. At the beginning of April 2021, a total of 13 308 cases of COVID-19 were registered, of which the proportion of recovered was 99.3% (13 218 cases), and the number of cases with a fatal outcome was 0.68% (90 cases;the average age of the deceased was 62.3±0.07 years). The peak of infection during the first wave occurred in May and June 2020, when the average daily increase was 97 people. For 2 months of the epidemic in the republic, 44.6% of the total number of patients with COVID-19 became infected, and the number of deaths reached 52 people or 57.7%. Among the patients, men prevailed (65%). The largest number of deaths (76.7%;n=69) was among older people with comorbidities (diabetes mellitus, cardiovascular disease, chronic lung disease, metabolic syndrome, etc.). An analysis of the age structure showed that the main proportion of cases fell on the age group of 40-60 years (42.6%). It was found that a significant proportion of patients with COVID-19 was detected in the Sughd region (33.0%) and Dushanbe (30.1%). Conclusion. The COVID-19 pandemic showed that the national healthcare system of the Republic of Tajikistan was not sufficiently prepared for such a development of the COVID-19 epidemic process. There was an acute shortage of medical and preventive specialists in the republic. Given the current situation in the Republic of Tajikistan, within the framework of epidemiological surveillance, the features of the course of the COVID-19 epidemic process were analyzed, adequate emergency measures were developed and proposed to limit the spread of the virus and reduce the negative impact of COVID-19 on public health. The number of beds has been reasonably expanded, the capacity of the laboratory service has been increased, mass vaccination of the population has been started according to epidemic indications. © 2022 Geotar Media Publishing Group

Material and methods. The retrospective study was conducted on the basis of an epidemiological analysis of official statistics as part of the epidemiological surveillance of COVID-19 from April 2020 to April 2021. Results and discussion. At the beginning of April 2021, a total of 13 308 cases of COVID-19 were registered, of which the proportion of recovered was 99.3% (13 218 cases), and the number of cases with a fatal outcome was 0.68% (90 cases;the average age of the deceased was 62.3±0.07 years). The peak of infection during the first wave occurred in May and June 2020, when the average daily increase was 97 people. For 2 months of the epidemic in the republic, 44.6% of the total number of patients with COVID-19 became infected, and the number of deaths reached 52 people or 57.7%. Among the patients, men prevailed (65%). The largest number of deaths (76.7%;n=69) was among older people with comorbidities (diabetes mellitus, cardiovascular disease, chronic lung disease, metabolic syndrome, etc.). An analysis of the age structure showed that the main proportion of cases fell on the age group of 40-60 years (42.6%). It was found that a significant proportion of patients with COVID-19 was detected in the Sughd region (33.0%) and Dushanbe (30.1%). Conclusion. The COVID-19 pandemic showed that the national healthcare system of the Republic of Tajikistan was not sufficiently prepared for such a development of the COVID-19 epidemic process. There was an acute shortage of medical and preventive specialists in the republic. Given the current situation in the Republic of Tajikistan, within the framework of epidemiological surveillance, the features of the course of the COVID-19 epidemic process were analyzed, adequate emergency measures were developed and proposed to limit the spread of the virus and reduce the negative impact of COVID-19 on public health. Features of the COVID-19 in lung tissue damage admitted to hospital 入院肺部组织损伤的新冠肺炎特征

The COVID-19 pandemic continues to spread despite all the efforts of the medical community. The aim of the study – to identify the features of a new coronavirus infection (COVID-19) with lung tissue damage in men and women receiving inpatient treatment. Material and methods. The method of continuous sampling selected 225 patients admitted to inpatient treatment. The patients were divided in 2 groups: the 1st group – men (n=85), the 2nd group – women (n=140). Statistical processing was carried out using the IBM SPSS Statistics 22.0 program. Results. In a comprehensive study of the features of lung tissue damage by the new coronavirus infection SARS-CoV-2 in men and women was found that women received inpatient treatment more often, after ineffective outpatient treatment, the general serious condition at admission was more typical for women who had a positive result of a PCR-test for the detection of SARS-CoV-2 (p=0.038). In the presence of general clinical manifestations complaints of loss of sense of smell were more often presented by women (p=0.020). According to laboratory studies in the indicators of general clinical and biochemical blood tests, on average men often have higher levels of creatinine (p<0.001), women more often have elevated levels of ESR (p=0.003), erythrocytes (p<0.001), CRP (p=0.042) and decreased hemoglobin (p=0.029). Conclusion. In the first wave of the Wuhan pandemic of the SARS-CoV-2 virus strain women prevailed among patients admitted to inpatient treatment with lung tissue damage after ineffective outpatient treatment (p=0.038). There was a commonality in age and clinical manifestations, of the complaints in women, only loss of sense of smell was more common (p=0.020). The erythrocytosis detected in women, increased ESR and CRP, lower hemoglobin and creatinine levels are not specific for COVID-19. There were no statistically significant differences in the terms of hospitalization and the fatal outcome of the disease between men and women. © Eco-Vector, 2022.
