Abstract Purpose of Review An emerging subset of dismal sinonasal cancers are those characterized by the loss of a SWItch/Sucrose Non-Fermentable (SWI/SNF) complex unit, such as the SWI/SNF-related Matrix-associated Actin-dependent Regulator of Chromatin (SMARC), which includes two main subtypes: SMARCB1- and SMARCA4-deficient sinonasal carcinomas, ultimately leading to four distinct SWI/SNF-deficient sinonasal tumors. These cancers are rare entities and low treatment responsive malignancies. In fact, they are poorly differentiated and usually detected at a late stage, when invasion of facial and cranial regions had already occurred. Recent Findings From a histological standpoint, SWI/SNF-deficient sinonasal carcinomas belong to the group of sinonasal undifferentiated carcinomas (SNUC); however, their distinctive features disclose a special category for these cancers. The identification of biomarkers and signaling pathways has led to the development of emerging therapies, such as immunotherapy and personalized treatments. Finally, we report preliminary findings on 3D in vitro models of sinonasal cancers, as a multidisciplinary tool that could empower the understanding of SWI/SNF-deficient cancer biology. Summary Here, we review the current knowledge about histological and molecular features of SWI/SNF-deficient sinonasal cancers, with a focus on treatment options and multidisciplinary research perspectives. The possibility of studying SWI/SNF-deficient sinonasal tumors in-depth would be fostered by the establishment of tumor cell lines.

Abstract Purpose of Review Discuss current literature and clinical experience related to vocal hygiene, with special consideration for the professional and performing voice user. Recent Findings Use and non-use-related vocal hygiene factors that may impact professional and performing voice users include hydration, laryngeal irritants, phonotrauma, and voice rest. Key considerations include monitoring both systemic and surface hydration, maintaining awareness of key signs and symptoms of reflux, avoiding phonotraumatic patterns and/or learning to pace voice use when high-impact vocal behaviors are required. Complete voice rest is better suited to treat acute injury and not practical for day-to-day vocal management. Summary Vocal hygiene is one tool within voice therapy that plays a key role in vocal injury prevention and rehabilitation when paired with direct voice therapy. Vocal hygiene education should be individualized, guided by the tenants of meta-therapy and motivational interviewing to increase self-efficacy for change and adherence to recommendations.

Abstract Purpose of Review To consider ethical aspects of rhinologic surgery including surgical and medical treatment options, new technologies and techniques, informed consent and patient choice. Recent Findings The ethical principles of autonomy, justice, beneficence, non-maleficence and fidelity are fundamental to the practice of medicine as well as to the clinical research that informs new treatment options. Here, they are discussed in the context of treatment of benign rhinologic disease. Summary Surgeons are ethically bound to act in the best interests of the patient, informing and involving them in the clinical decisions and avoiding harm. This requires full and honest discussion with patients about the available treatment options, their potential benefits and risks and cost-effectiveness of newer techniques and technologies.
