Exploring Stakeholder Perspectives on the UK’s Regulatory Tools for Accessible Housing: Lessons for Canada 探讨利益相关者对英国无障碍住房监管工具的看法:加拿大的经验教训

Abstract Accessible housing is an urgent issue for countries experiencing rapid population aging. For over 20 years the United Kingdom’s (UK) building code (Part M) and Lifetime Homes Standard have been researched as leading practices to increase accessible housing stock and allow older adults to stay in their communities. This review compiles key stakeholder groups’ perspectives on these regulatory tools. Five main themes and ‘lessons learned’ were derived to anticipate potential challenges and inform proactive response for planners and policy makers in Canada and other countries when developing their own accessible housing regulation.

Developing and Testing A Model of the Assisted Living Environment. 开发并测试辅助生活环境模型。

The assisted living (AL) environment plays an important role in supporting residents' satisfaction and helping them to age in place. The AL environment is multidimensional and has many interrelated components including staffing (e.g. direct care workers, nursing, activity staff), services provided (e.g. medical, mental health, pharmacy), amenities offered at the setting (e.g. beauty salon, library, exercise facilities), and the physical environment. Evidence suggests that aspects of the AL environment can enhance or detract from the physical function, well-being, social engagement, and behavioral outcomes among residents. The purpose of this study was to develop and test a multidimensional AL environment measurement model that includes indicators of staffing, services, amenities, and the physical environment. Baseline data was used from a study testing the Dissemination and Implementation of Function Focused Care in AL. A total of 54 AL facilities across three states were included in the sample. Settings ranged in size from 31 to 164 beds with an average size of 82.2 (SD=26.2) beds and the majority were for profit facilities (n=41, 74.5%). Structural equation modeling was used to test the proposed model. Results showed that the model fit the data (χ2/df=1.861, p<.05; CFI=.858, RMSEA=.126). Having a comprehensive AL environment measurement model will advance future research that explores the impact of the environment on resident outcomes. Findings from this study will inform interventions and programs designed to modify AL environments to optimize residents' satisfaction with AL.

Profiles of Expectations Regarding Aging After Relocation to Assisted Living. 搬迁到辅助生活区后的养老预期概况。

This study classified assisted living (AL) residents by expectations for future physical, cognitive, and socioemotional health in later life. The authors analyzed cross-sectional data from 202 AL residents. Study participants completed the 12-item expectations regarding aging survey. A K-means cluster analysis identified three subgroups: "lower expectations" (n = 55), "higher expectations" (n = 70), and "expecting adaptiveness" (n = 77). Subgroups varied by select demographic, health, and relocation-related factors. Results suggest that, despite similarities in physical and cognitive functioning, AL residents reported different expectations for health. Socioemotional functioning may help explain observed discrepancies between current health and future health-related expectations.

Institutional Environment of Old Age Homes and Its Relation to Adjustment of Older Adults: Evidence from Pakistan 养老院的制度环境及其与老年人适应的关系:来自巴基斯坦的证据

Abstract The study investigated the role of institutional environment in adjustment of older adults living in old age homes of Punjab, Pakistan. The study was conducted on a sample of 270 residents, living in 18 old age facilities across Punjab. The data was collected through a structured interview questionnaire and was analyzed through SPSS (v. 22). The multiple regression analyses suggested that institutional environment along with number of children and number of visitors was a significant predictor of adjustment of older adults. Interventions such as higher support from staff and residents, autonomy, and engagement can help older adults to adjust better in old age homes.

Identifying Key Components of Paper-Based and Technology-Based Home Assessment Tools Using a Narrative Literature Review 使用叙事性文献综述识别基于纸张和技术的家庭评估工具的关键组成部分

Abstract The built environment of an older person’s home can reduce or promote the possibility of a fall or other injury. A user-friendly tool can help the residents to evaluate their home environment within the context of their specific functional and mobility challenges, so that needed modifications can be identified and completed before an event causing injury occurs. The increasing use of telehealth and other digital technologies has paved the way for technology-based person-initiated home assessments that may allow people to evaluate and improve their homes proactively. The purpose of this literature review was to systematically analyze existing published paper- and technology-based home assessment tools to understand the structure and key characteristics of existing tools and to understand the limitations in existing home assessment tools. This review identified and compared 24 studies based on structure, content, and implementation. While a vast variety of tools and methods currently exist for conducting in-home or remote home assessment for older adults, there is a lack of a comprehensive and user-friendly technology-based tool that older adults can use to conduct an environmental assessment of their home in relation to their functional abilities, such that modifications may be identified that can prevent falls and other injuries.

Creating a Sense of Place after Homelessness: We Are Not “Ready for the Shelf” 无家可归后创造一种地方感:我们还没有“准备好上架”

Abstract Increasingly, older adults’ perspectives are informing aging in place and Age-Friendly Cities (AFC) research, policy, and practices. However, rarely are the voices of older people who are experiencing or have experienced homelessness included. Guided by the concepts of place and home, this qualitative study used semi-structured go-along interviews and thematic analysis to explore how seven older adults (aged 50+) with homeless histories created a sense of place, and the facilitators and barriers they encountered in their place-making process after being housed. The analysis revealed two main themes: home as safety; and sense of place as purpose. Although most expressed gratitude for the safety their housing provided, it was not enough to feel in place. Sense of place was created through engaging in meaningful activities that provided them with a sense of purpose. Sense of place as purpose was facilitated by three subthemes (self-determination, employment/education, and technology), and three barriers (discrimination, transportation, and income). Recommendations are provided which can inform the development of more inclusive housing design and programming, and foster AFC for all older adults, including people with homeless histories.

Abstract Home adaptations are integral for inpatients with fall-related fractures, especially during discharge. However, it is unclear as to how patient-implemented home evaluations can be prioritized with limited professional rehabilitation resources. This study examines the associations between predisposing, enabling, and needs factors with the implementation of home evaluations in the eastern district of Tokyo. Being a female and lower comprehension in cognitive function was significantly associated with the implementation of home evaluations. It may be necessary to consider patients’ cognitive function when deciding who to target for housing evaluations and to seek strategies to best estimate their needs.

However, it is unclear as to how patient-implemented home evaluations can be prioritized with limited professional rehabilitation resources. This study examines the associations between predisposing, enabling, and needs factors with the implementation of home evaluations in the eastern district of Tokyo. Being a female and lower comprehension in cognitive function was significantly associated with the implementation of home evaluations. Later-Life Migration in The Netherlands: Propensity to Move and Residential Mobility 荷兰的晚年移民:流动性和居住流动性

Abstract As the populations age, there is a growing potential for later-life migration. In this study, the residential moving behavior of older adults in the Netherlands is examined. On the basis of pooled data from the Housing Research Netherlands (HRN) survey, we were able to reveal which factors are likely to influence considerations about moving and actual mobility. The results support the premise that residential mobility decreases with age. The strongest predictors of actual mobility were factors associated with the dwelling, while the neighborhood, particularly the extent of social cohesiveness, played a substantial role in explaining the older adults’ propensity to move.

Abstract With the increase in life expectancy in the United States comes an increase in years lived with a disability and poverty. This necessitates age-friendly, affordable housing that is appropriate for older adults with disabilities. Board and care homes provide housing and services for older adults with disabilities. This scoping review maps the literature on board and care homes in the United States following the age-friendly communities framework. A synthesis of articles (n = 10) revealed that board and care homes, while probably affordable, can be unsafe; variable in services provided; are mostly unregulated; and may lack opportunities for socialization.

Abstract The double societal hit of dementia and infectious disease outbreaks like the COVID-19 pandemic have raised a convergence of concerns for the future of care settings for people living with dementia. It is estimated that 5.4 million Americans have some form of dementia and these numbers are expected to rise in the coming decades, leading to an unprecedented demand for memory care housing and services. In searching for innovative options to create more autonomy and better quality of life in dementia care settings, repurposing existing structures, in particular vacant urban malls, may be one option for the large sites needed for the European model of dementia villages. These settings may become sustainable Dementia Friendly City Centers, because with enclosed mall construction, the internal infrastructure is in place for lighting and HVAC, with varied spatial configuration of public spaces. This paper describes the community-based focus group research with medical providers who addressed designs for adaptive reuse of an existing closed mall case study site. Focus group outcomes included discussions on four central themes about the adaptive reuse including: applicability of the concept, spectrum of programs, economic feasibility and onsite medical care.
