This review is devoted to assessing the prevalence of human papillomavirus (HPV) in head and neck tumors (HNC). HPV is recognized as the etiological factor of cervical cancer, but it may be involved in other tumors progression. Of the 28 studies presented, 6 studies showed a low incidence of HPV in HNC tissue (1.6—6.2 %). Most often, HPV was detected in Singapore (90.6 %), Japan (100 %) and France (94.7 %). Most often found 16 types of virus (82 %) and 18 types (43 %). Low-grade HPV type 11 was most often observed in the Russian Federation (19 %). The frequency of occurrence of HPV in HNC varies, which raises the question about impact HPV and co-infection in HNC progression. It makes further research in this area relevant and promising.

Surgical treatment is leading in the treatment of malignant tumors of the salivary glands. The complexity of the anatomical areas, the severity of the tumor process, the close location of the facial nerve branches lead to frequent complications in the postoperative period. The technique of elimination of paresis of mimic muscles with the use of a complex of therapeutic measures, including a course of physiotherapy with the appointment of a magnetolaser and SCENAR therapy developed at the Cancer Research Institute, Tomsk National Research Medical Center of the RAS and the prospect of its use in this pathology. The clinical case of application of this material is described.

The study objective is to evaluate the utility of ultrasound (US) examination in the diagnosis of laryngeal/hypopharyngeal squamous cell carcinoma and in the assessment of tumor spread. Materials and methods . We performed US examination in 100patients (7 females and 93 males) with laryngeal/hypopharyngeal cancer aged between 36 and 85 years. We evaluated vocal cord mobility, condition of the laryngeal cartilages, and tumor invasion to the adjacent tissues within the larynx and beyond it. In all patients, the diagnosis was confirmed by histological examination. Results. Seventy-five patients had primary laryngeal/hypopharyngeal tumors, whereas the remaining 25patients presented with recurrent cancer. Eighty-one participants were found to have laryngeal cancer; of them, 13 patients had tumors in the supraglottis; 67 patients had tumors located in the glottis; and 1 patient had a tumor of the subglottis. Nineteen patients were diagnosed with hypopharyngeal tumors. We have identified the most typical US signs of laryngeal/hypopharyngeal tumors considering their location and compared the results of US examination with histology of surgical specimens. US examination demonstrated a sensitivity of 94.1 %, accuracy of 91.9 %, efficacy of 47.0 %, and positive predictive value of 97.5 %. Specificity was not evaluated since we had no negative results. Conclusion. US examination is a highly accurate methodfor the diagnosis of both primary and recurrent laryngeal/hypopharyngeal tumors.

The study objective is to present the experience of performing extensive cranioorbital resection with subsequent arthroplasty of the hard palate and the dentition of the alveolar process of the maxilla. Materials and methods . In 2018, in the Department of Microsurgery of the P.A. Hertzen Moscow Oncology Research Institute, extensive cranioorbital surgery for recurrent sarcoma of the upper jaw was performed, after the treatment in other clinics. The extensive intervention included the exposure of the dura and the single-stage reconstruction of tissue defect, bone defect of the skull base by using the displaced aponeurotic flap of scalp and microsurgical reconstruction of the soft tissues of the face and mouth left free of the gastro-omental flap. In the early postoperative period, the patient underwent endoprosthesis replacement of the hard palate and alveolar process with a dentition. Results . Extensive intervention with resection of the structures cranioorbital zone and the skull base with a single-stage reconstruction with free gastro-omental flap with subsequent arthroplasty of the hard palate and the alveolar process dental arch allowed to rehabilitate the patient.

The study objective is to analyze the efficacy of a hedgehog signaling pathway inhibitor in locally advanced and recurrent basal-cell carcinoma (BCC) of the head and neck. Materials and methods . We compared the information from international clinical guidelines and two clinical trials (ERIVANCE and STEVIE). We also provide two case reports of a hedgehog signaling pathway inhibitor (vismodegib) use. Results . In the ERIVANCE trial, the objective response rate was 48.5 % in patients with metastatic BCC and 60.3 % in patients with locally advanced BCC. The median progression-free survival was 9.3 and 12.9 months in participants with metastatic and locally advanced BCC respectively. The overall survival reached 33.4 months in patients with metastatic BCC and was not reached in patients with inoperable BCC. According to the results of the STEVIE trial, vismodegib ensured disease control in 93 % of patients with locally advanced BCC and 72 % of patients with metastatic BCC. The median progression-free survival was 24.5 and 13.1 months in participants with locally advanced and metastatic BCC respectively. The majority of vismodegib-realted adverse events in both ERIVANCE BCC and STEVIE trials were grade I/II adverse events. In the two clinical trials, vismodegib at a dose of 150 mg per day during 3 months caused visual reduction of the bone destruction focus, which was confirmed by computed tomography. Patients were recommended to continue therapy with vismodegib. Conclusion . Until recently, there were no treatment standards for late-stage BCC. Currently, hedgehog signaling pathway inhibitors, such as vismodegib, can be considered as a method of choice for patients with inoperable and metastatic BCC, since this therapy demonstrated high efficacy.

Currently, intraosseous dental implantation is one of the most widely used methods to eliminate various defects of the dentition. Despite the achievements of modern medicine, the frequency of inflammatory complications, such as peri-implantitis, in patients after dental implantation remains relatively high. Some authors suggest the presence of similarity of peri-implantitis with squamous cell carcinoma of the oral cavity at the level of pathogenesis, as well as cases of mimicry of these different nosological units are described. In this review, we study the pathogenetic mechanisms of the emergence and development of chronic inflammatory processes on the example of dental peri-implantitis, squamous cell carcinoma of the oral cavity and periodontitis, as well as the comparison of pathogenetic patterns of these processes with the study of the possible relationship. The objective of the review was to provide readers with knowledge about the common pathogenetic mechanisms of different nosological units. First of all, the data are necessary for dentists, who must remember the principle of cancer alertness, carefully carry out differential diagnosis, change the tactics of treatment of peri-implantitis with suspected tumor process.

The study objective is to report a case of mandibular ameloblastoma. Materials and methods . A 30-year-oldfemale patient presented with swelling of the right mandibular region that appeared 8 months ago and has been slowly increasing. The patient had no other symptoms, such as pain, anesthesia, paresthesia, difficulty swallowing, mouth opening, speech disorders, fever, chills, or weight loss. Orthopantomography revealed pronounced bone resorption in the right portion of the mandibular body. Computed tomography showed a cystic cavity in this area. After incisional biopsy, the patient was diagnosed with ameloblastic carcinoma. We performed segmental mandibular resection and installed a reconstructive plate to restore mandibular integrity. We also performed selective cervical lymph node dissection (levels Ia and Ib). Results. One yearpostoperatively, the results were considered aesthetically and functionally satisfactory. Conclusion. We observed an aggressive disease phenotype with extensive localized bone destruction, typical of ameloblastic carcinoma. Extensive excision of the primary tumor and selective cervical lymph node dissection without radiotherapy ensured a relapse-free period during patient’s follow up.

Medullary thyroid cancer is a rare tumor. Due to variability of its biological behavior, treatment of this tumor often is a complicated problem. Clinicians have difficulties with determination of indications for prescription of tyrosine kinase inhibitors. Prescription should be based on scru-tinous monitoring of the patients and consideration of all factors: level of tumor markers, patient’s state, and data form visual examinations.
