This article analyzes issues of surgical site-associated infections (SSIs). The authors analyze factors influencing SSIs development and discuss measures which could reduce the risk of these infections spread. Compliance with the principles of asepsis, adequate surgical linens and surgical clothing are discussed in more details. Basic requirements for surgical linens – following standards and existing practice – are highlighted as well. In addition, the authors analyze advantages of disposable nonwoven materials used for surgical garments as protection against SSIs both for patients and medical staff.

The present review outlines features of a staged surgical tactics in treating battle and non-battle injuries in military medicine and disaster medicine. The problem of surgical infections in lightly wounded patients and the effectiveness of ozone therapy are also highlighted in the review. The authors introduce the concept of ozone-surgical treatment of wounds which is a combination of its evidence-based methods and acknowledged techniques of surgical care and blockades. The staged regional ozone therapy which is described in the article is an adjuvant surgical aid to prevent wound infection in damaged tissues.

Modern management of wounds of various etiologies is a comprehensive approach which includes correction of patient's general somatic state and adequate local treatment strictly corresponding to the stage of wound healing process. In the outpatient unit, it is a local gentle management of wound defects with proper wound coatings that meet various tasks at various stages of wound healing process. One of the problems of modern wound management is stimulation of epithelialization, namely the restoration of subdermal, dermal and epithelial layers. One of the most promising directions in solving this problem is application of collagen, due to its optimal manipulation characteristics, biocompatibility, controlled biodegradation, ability to form complexes with pharmaceuticals as well as to stimulate regeneration. Collagen coatings may be used both independently and in the complex of local treatment when collagen dressings alternate with other interactive dressings. Such an approach ensures high-quality regeneration of wound defects and prevents complications.

Myelodysplastic syndromes are clonal stem cell diseases characterized by hematopoietic dysplasia taking place in one or more myeloid cell lines, as well as by a high risk of progression to acute myeloid leukemia. This systemic disease is often accompanied by the violated microcirculation in the skin due to thrombosis of the terminal arteries leading to the formation of necrotic zones. The multicomponent nature of the disease requires a multidisciplinary approach to the treatment and staged surgical intervention. In this article, we present an example of a successful complex treatment of a 16-year-old girl with soft tissue necrosis of the perineum which developed because of the myelodysplastic syndrome.

It included surgical treatment of a purulent focus, local treatment with negative pressure, and plastic reconstruction of the anterior chest wall with autografts when the wound process transformed into the reparative stage.

To cover a post-surgical bone defect is an essential part of successful treatment of osteomyelitis of long bones in addition to radical surgical treatment and osteonecrectomy. Autologic vascularized tissues is the best option to fill the infected bone defect. However, if a pathological focus is localized in the distal part of tibia, the deficit of surrounding soft tissues makes the replacement of infected tibial defect a difficult task. One of the ways to solve the discussed problem is application of de-epithelized part of fasciocutaneous sural flap with a distal feeding pedicle. The article describes specific features of this surgical technique and outcomes of treatment in three patients with post-traumatic osteomyelitis (type III by Czerny – Mader classification). Our experience allows us to recommend this technique for treating an infected cavity in the medullary canal of the distal part of tibia.

Knitted wound dressings based on synthetic polymers (primarily lavsan) have some advantages over dressings based on natural materials (cotton, etc.) – first of all, they cause less irritation of wound surfaces due to the absence of microvilli. However, during storage one can see changes in the properties of lavsan dressings, i. e. the material becomes more rigid. Under the present work, it has been shown that these changes are caused by the significantly increased crystallinity of lavsan fibers during exposure to natural environment. To reduce lavsan crystallinity, it is recommended to process the material with mixtures of polyethylene glycols which are popular ointment bases. Heat enhances the effect and returns the original degree of softness to knitted lavsan dressing materials.
