This paper reviews 'database interoperability' problems and suggests a framework for integrating molecular biology databases. It suggests a component database for gene mutation data which is subject-oriented and dedicated to a particular research laboratory, and outlines how to develop a cooperative environment for molecular biology databases to share information based on a common view. The paper also establishes the concept of image object keying for multi-database querying and it proposes a matching algorithm using gel electrophoresis images. This image object keying method could replace the conventional SQL scripts for multi-database querying.

In this paper, we present some important aspects of software testing. We explain why software reliability is important, especially for medical software products. The automation of testing procedures is important in order to achieve the desired reliability level. For this purpose, intelligent systems can be used to identify potentially dangerous software modules. We present an evolutionary decision-tree approach to the identification of dangerous software modules in a real-world medical application.

Modern medicine generates huge amounts of sensitive data. Data mining and knowledge discovery systems can be used to extract useful information/knowledge in order to to improve future diagnoses and reduce costs. On the other hand, when analysing data, the most recent methods, that extract the most accurate knowledge and decision models, should be used. This can be established with a service enabling quick access to newly developed methods. In this paper, a service for doing this, named IFSIMS (Internet Framework Service for Intelligent Medical Systems), is presented.

Palliative care units are hospital services that assist advanced-state terminal patients. These patients are geographically distributed and some hospital tasks such as the organization of visits, the management of medicines, and the analysis of patients are difficult to manage. Here, a multi-layer computer system is proposed. The communication, information system, and multi-agent layers are described and a prototype of the system is used to deal with the Palliative Care Unit at the Hospital de la Santa Creu i Sant Pau in Barcelona.

The increasing storage of vast amounts of information on computers and the rapidly increasing size of databases stored online has the population (and even many politicians) worried about invasion of privacy. This paper examines some possible ways in which privacy maybe invaded by the use of medical databases, particularly during the analysis and publication of results.

The analysis of hospital costs in public health-care systems is a difficult task that the use of diagnosis-related groups (DRGs) has tried to simplify. After more than two decades, DRGs are still used, though they have had important criticisms. In this paper, COSYS (COsting SYStem), a new computer system that incorporates some AI procedures, is introduced as a tool to study hospital costs and also to validate whether DRGs are close to the reality of a particular hospital. Seven representative DRGs are studied and the conclusions from the study are set out.

Ischaemic stroke is a very common disease that affects blood vessels in the brain, causing cerebral tissue damage. Despite several studies on this subject, the early detection of acute ischaemic stroke is difficult, as well its treatment. In this paper, we present some features and initial results of our research, which includes a new image acquisition protocol and a software tool in order to help physicians to improve the accuracy of the therapy.

The shift from version 9 to version 10 of the "International Statistical Classification of Diseases and Related Health Problems", or ICD (International Classification of Diseases) for short, causes enormous problems for the exploitation of medical data warehouses, since conventional data warehouses do not support the change of the structure of dimensions, i.e. the content and relationships of master data like the diagnostic codes, or other key values. This shortcoming results in a reduction of possible analysis, and unfortunately is the cause of many wrong statistics and analysis results. In this paper, we analyze the problem and show how to superimpose conventional multi-dimensional data warehouses with temporal master data to allow queries spanning multiple periods to return correct answers.

Modern and effective biomedical research needs new and effective methods. Use of 96-well micro-titre plates allows the performance of many experiments and samples under the same conditions. The methods used to determine cell numbers in these plates are all end-point methods. These methods allow only one determination per plate, and the cells are lost. Usually, such assays are performed according to concentration dependance. Other parameters and cell growth dynamics remained unrevealed. To reveal cell growth dynamics, multiple assays should be performed. Such an experimental approach would be expensive and time-consuming, so it is rarely performed. To fill this gap, the Institut fu/spl uml/r Angewandte Mikrobiologie (IAM) and SLT Labinstruments developed GCSS (General Cell Screening System). GCSS is a powerful hardware/software system that enables continuous monitoring of cell growth without any treatment or stain. The method is based on a high-resolution turbidity measurement performed directly on the cell culture plate. The system consists of a reader and an eight-channel photometer, a plate with a new form of wells, an Apple Macintosh computer and the GCSS software. We chose a classic bone-marrow colony-count assay, which is a typical assay scored after seven days of incubation and based on one measurement only. GCSS allowed us to seed bone marrow cells in microtitre plates in a medium with different concentrations of haemopoietic growth factors and to perform multiple measurements. We observed bone-marrow cell growth derived from interferon alpha-treated mice and compare the cell growth from placebo-treated mice. The classical bone marrow assay only allowed scientists to confirm the suppressive nature of interferon alpha on bone marrow cells in-vivo. Multiple measurements with GCSS allowed us to collect cell growth data over seven days of incubation which could not be seen in the colony-count assay. The data collected with GCSS could not confirm the suppressive nature of interferon alpha but revealed that it is a bone-marrow cell activator in-vivo. We expect that GCSS will have an important influence in new biomedical research, and in corrections of established assays too.

Reliable diagnostics for coronary artery disease 可靠的冠状动脉疾病诊断

In the past few decades, machine learning tools have been successfully used in several medical diagnostic problems. While they often significantly outperform expert physicians (in terms of diagnostic accuracy, sensitivity and specificity), they are mostly not being used in practice. One reason for this is that it is difficult to obtain an unbiased estimation of the diagnosis's reliability. We discuss how the reliability of diagnoses is assessed in medical decision-making and propose a general framework for reliability estimation in machine learning, based on transductive inference. We compare our approach with the usual machine-learning probabilistic approach, as well as with classical step-wise diagnostic process, where the reliability of a diagnosis is presented as its post-test probability. The proposed transductive approach is evaluated in a practical problem of the clinical diagnosis of coronary artery disease. Significant improvements over existing techniques are achieved.
